Employee was removing PTO pump from truck and dropped pump on right little finger causing laceration requiring 5 stitches.

Greaser was stepping down from hydraulic tank to the track on D10R dozer twisted to grab the handrail felt sharp pain in lower back.
Loading spoil in the pit and a rock rolled out of spoil hitting bucket jarring loader operator causing them to twist their lower back.
Powder truck was parked behind employee parking lot. An employee seen smoke coming from the front of truck and went and got help plus fire ext and started putting fire out used a total of 8 fire ext to put fire out and a water truck to sprays water to cool the truck. Fire was contained to the cab and engine compartment. Msha was on site at time of event.
Employee was walking from pickup to the employee change room and slipped and fell. Employee said employee was ok and worked entire shift. Woke up next day 1/9/2025 and called foreman and said was going to the doctor, that the left knee was swollen and hurting.

A school bus that we use for a mantrip caught fire under the hood and burnt no one was injured or hurt
Employee was working on installing a roller frame on the D-11 dozer #040, when positioning the frame they placed a hand in a pinch point and smashed fingers between the roller frame and the bogie.
Employee was operating a rock truck being loaded when a rock unexpectedly flopped out of the loader bucket striking the offside bed railing of the truck bouncing EE around in the cab.

Employee was cleaning up the out slope below the ALMA seam on the left hand side of Mill Seat. When the excavator took off down the slope overturning on its side down the hill. When the excavator came to rest EE took the seatbelt off and fell out of the seat striking EE's head and shoulder.
EE was in the parts yard looking for a replacement hose and stepped on a parts pallet which broke causing EE to stumble and fall.
Employee was sitting on the back of the mechanic truck and stepped down and started to turn around when EE's ankle gave causing EE to lose EE's balance and fall to the ground. EE tried to catch self with EE's right arm causing EE's injury.
Employee was removing a fan shroud from a 993K loader, when employee got left hand caught in between the shroud and the frame of the loader.
Employee was changing the belts out on a excavator. Employee was pulling on the belt tensioner when they felt a sharp pain/burning in lower back. The doctor didn't take them off work until 04/21/2021.
Exiting steps of lube trailer. Stepping off bottom step right foot became stuck in mud while the other foot was still on the step. Proceeded to fall backward hitting left side of torso on large mining truck wheel chock.

EE was entering the mechanic's change trailer when EE tripped on a rubber mat and fell into a supply table. EE sustained a laceration to face and strained left shoulder.
EE was tramming a highwall drill from the south side of Grapevine to the north when the left side track locked up. EE was seated an the sudden stop threw them out of the seat against the door. Head hit door frame causing a headache and shoulder strain as well. Became a lost time accident on 8/17/2020
EE was changing the air conditioning compressor on a Mechanic Truck. When EE removed the air conditioning line there was a pressure build up in the line and it sprayed the Freon Gas in both eyes.
EE had climbed on a D10 dozer and filled it with hydraulic oil and was attempting to dismount. A mist of rain had fallen and even though EE was holding the grab rail, feet slipped . EE fell to the ground hitting right shoulder and knee against the ground.

During workday EE used a sledge hammer, carried boxes of blasting caps and other tasks. When shift was over they were experiencing pain in neck and shoulders. EE said it felt like a pinched nerve. EE did not know which task caused the injury. MRI revealed fracture of C& and C6 spinous process. Sent home with cervical collar. Will see a neurosurgeon. (Edited to fit 384 characters)
Employee was operating a water truck on Grapevine North Contour. Employee was going up an incline to the pit. The truck engine died and began to roll backward. Employee said it rolled about 75 ft. and they steered the truck toward the highwall. The rear tire rolled up on a berm and it flipped on its side.

EE parked the rock truck EE had been operating and was climbing down the steps off the deck when EE lost footing and fell 8.5 feet to the ground and landed on lower back. EE sustained a fractured T-1 and S-1 vertebra. a fractured pelvis, and two fractured ribs. EE was flown to Medical Center and then transferred to another Medical Center for care.

Working on D11R Dozer E-Bar center pin repair with customers job mechanic. Pin was frozen to correct temperature and both were holding each side of the 180 lb. pin when customers job mechanic stumbled. EE took the whole weight of the pin to keep it from falling on self. Felt burning sensation in stomach.
EE was tramming drill to the bench when the final drive stalled causing the chain that holds the dust curtain to crack side window and dust and debris came in the operators compartment. As a precautionary measure the EMT-M flushed ee eyes with eyewash. Due to continued irritation ee went to the doctor on 5/19/17 and was prescribed eye drops the
EE was working on a 785 truck at Grape Vine South Surface removing the main pump with another EE and a field person from JP Tech. Pump was almost out, EE moved to the side and was waiting for pump to come out to the ground. When it did, it struck the pry bar EE had been using. When the pump fell to the ground, it hit the pry bar and the bar slammed into EE's lower leg.
Employee was helping mechanic on a 777 rock truck. Using a pry bar and bar slipped causing employee's right wrist to bend backward too far

Employee was operating a 980 G end loader and backed over a rock. The loader slammed down and the seat suspension bottomed out twice, jolting the employee and straining employee's neck, shoulder, and lower back. Employee continued to work until 5/9/2016 at which time the doctor took the employee off work.
Employee was moving cable trees so Highwall miner could advance to hole #36. While he was moving the cable trees, one tree tipped over and struck the 980H Loader on the off side mirror. The mirror hit the side windshield glass, busting it, and causing the glass to hit the employee. He had cuts on the right forearm and glass in eyes. RTW the next scheduled work day 3/16/15.

EE was removing cutting edge from a D-10 dozer. He was removing what he thought was the 2nd to the last bolt holding the cutting edge on however, the remaining bolt had broken off and when ee removed the bolt the cutting edge flew loose due to dirt behind the cutting edge and the cutting edge came forward and struck him on the forehead. He sustained a laceration requiring stitches
The wrench cable was tangled up on the boom of the Freightliner mechanic's truck. Employee climbed on the bumper of the truck to straighten the cable and lost his footing. He fell to the ground and injured his right shoulder and elbow. He became a lost time on 9/24/2014.

EE was dismounting a 785C rock truck when he lost his footing and fell to the ground. EE was taken to Hospital for treatment and diagnosed with multiple contusions, x-rays and CT Scan were negative for any fractures. EE was taken off work and told to follow up with his family physician.

EE was using a hammer to loosen sprocket from excavator when the sprocket chipped and a fragment of metal entered his right thigh requiring medical attention to remove it.
EE was operating a 830 Rock Truck up a steep incline, the truck went into neutral and started to roll backwards. The truck struck a blade on the dozer that was following behind for safety precautions. An eyewitness to the accident stated that the truck rolled backwards a maximum of 15' before striking the dozer blade. EE taken to Medical Center because of neck pain from seat belt.
Employee was operating rock truck when he leaned over the arm rest of seat to pick up a pencil and hurt his ribs. Taken off work effective 1/18/11.

EE was walking near grader getting papers and did not notice that the blade was not on the ground, when his foot caught under the blade, smashing the steel toe down resulting to a fractuure of his left big toe.
Employee was operating a dozer when the kill hose blew. Employee exited the cab, checked hose and was walking back to cab when his feet slipped while he was holding on to the grab rail and strained his right shoulder. He was placed on restricted duty on 12/4/2010.
EE was operating loader when it slipped off a large rock, causing his right shoulder to strike the dash and windshield. He was taken off work effective 10/28/10.
Drill steel came out of carousel and the EE tried to push it back in place manually, when his thumb got caught between the drill steel and the brace, lacerating the tip of his right thumb.
While installing an o-ring in a 777 rock truck tire, the hydraulic jack employee was using flew out striking employee in hand.
Employee was prying with a crowbar and the bar slipped causing pain in his right shoulder. He continued to work until 6/2/2010 at which time his doctor said he was unable to work. EE will have surgery for a tear in the rotator cuff. He is expected to be off work for approximately 20 weeks.

Employee was helping put teeth on loader-hit tooth with hammer & metal went into lower left leg. Cut a small piece - metal sliver in leg.
EE was walking behind welding truck, stumbled over a chain, fell and hit his head on a piece of triangled metal that was sticking up on a piece of flat metal.
EE went to step off the steam truck he slipped and hit his leg on the bottom step of the truck.
EE was filling up fuel truck. He climbed up ladder to see if truck was full. He was climbing down and he slipped on ladder and fell on ground. He said his right shoulder and neck were hurting. Sent him to hospital (A.R.H.)
