Showing all 15Employee was carrying food into the mine office to heat. EE lost balance, stumbled and fell backwards. EE stepped on a water hose lying on floor of porch. EE fell onto a mantrip parked on porch then onto the floor. EE complained of right shoulder pain. EE was treated by MET-M then transported by ambulance to hospital.
Victim had stepped onto the top of a truck tire to gain access to the engine compartment. EE's foot slipped as EE was getting down and EE injured wrist when catching themself. Incurred fractured/dislocated left wrist.

When descending ladder from loader employee sprained his right ankle when shifting weight to right foot as left foot left the last rung of the ladder.
Location: office area. He slipped and fell due to the ice and snow in the office area going out to get in his truck.
Climbing down from hauler, made it down to 3rd step of ladder & then fell to ground.

After working on fender lights of equipment, employee stepped from fender back onto deck, hitting and slightly cutting his face above the right eye. He then fell to the ground.
Employee stated he was cleaning windows on equipment and stepped back too far, fell to ground.
