As the Safety Technician was walking past the power center, EE tripped over a cable causing EE to fall awkwardly on EE's right knee. Started missing work on 5/18/2026.
Employee was stepping over a trailer tongue when they lost balance injuring right knee. EE was seen at the ER where they were taken off work as they were unable to perform their job without restrictions.
Employee was carrying a diesel particulate filter when EE stepped awkwardly causing a jolt to EE's right hip. EE continued working until 4/1/26 when a follow up with a physician gave EE restrictions preventing EE from performing normal duties.
While watering the roadways they stopped the diesel tractor to check the sprays on the water cart. As they were walking to the back of the water cart, they were tripped up by a nylon band laying in the road causing them to fall and twist their right knee. They worked until a week before surgery. Started missing work on 03/30/2026.
Employee was walking while carrying a timber when EE fell. EE's left hand contacted sharp rocks on the mine floor resulting in lacerations requiring sutures. EE was given a full release to return EE's next shift with no restrictions.
Employee tripped over some car cable at the section substation. This resulted in a fall and the employee hitting their nose on the corner of the sub station. They received a broken nose from this.
Employee was walking from shuttle car to help the miner move when they twisted an ankle and fell to the ground. EE was seen at the ER where x-rays were negative for a fracture but employee was taken off work due to pain preventing them from performing normal duties.
The mechanic was standing on the shuttle car fender that was laying on the ground. While they positioned themselves to fix the problem, they slipped on the side of the fender causing pain in left knee. Started missing work on 11/14/2025.
While crossing over a supply trailer the tractor operator slipped and fell onto the ground. The operator worked until needing surgery to repair the left knee. Started missing work on 11/26/2025.
Walking around the tailpiece EE stepped in a hole, slipped causing pain in EE's left knee. Started missing work on 09/24/2025 and returned on 09/26/2025.
Employee jumped off a ledge at the dust hole onto a dust trailer. EE then jumped from the dust trailer onto the top of a diesel scoop and grabbed a trussbolt to catch EE's balance causing EE to strain EE's left shoulder. Started missing work on 09/08/2025.
Employee was helping bolter move when the reel was engaged knocking EE to the ground, landing on EE's left wrist. Finished shift but had pain and swelling the following day. Diagnosed with a fracture of left wrist and taken off work beginning 7/16/25 as EE was unable to perform EE's normal duties.
Employee was replacing some hoist cable pads for the slope hoist rope. When EE went to turn and walk EE's left foot stayed planted on the ground causing EE to twist EE's left knee. EE started missing work on 05/22/2025
While exiting the scoop the scoop operator twisted their left knee. Started missing work on 02/20/2025.
While walking to the opposite operator boom to install a roof bolt, the employee slipped and fell on a rock. The employee caught their body with their left hand spraining wrist. Started missing work on 12/12/2024.
Mechanic exited a ride, stepped over a berm in the road and rolled their right ankle. Started missing work on 12/03/2024. Update: Started missing work again for the same injury on 4/1/25.
Employee was climbing a bank out of the dust hole and slipped falling onto the side of a dust wagon landing on EE's right side under EE's right arm resulting in two fractured ribs.
The employee slipped and fell hitting a screen on the chin causing employee to chip a front tooth. The employee did not miss any work.
The employee exited the shuttle car and tripped over a piece of coal. Falling to the ground employee's rib cage on the left side landed on a chunk of coal that had fallen out of the rib, resulting in a fractured rib. The employee did not miss any work.
Employee was organizing the supplies on the top of the bolter, when sliding off the side of the bolter employee's feet hit the ground twisting left knee. Employee started missing work on 05/29/2024.
Employee was loading cable bolt plates in the tray of EE's roof bolter. Reaching to put the plates into the tray EE slips and smashed index finger between the plates and the steel tray. Resulting in a fracture.
Employee tripped over a tray made of boards and hit EE's right shoulder on the supply trailer. Employee started missing work on 04/22/2024.
EE was walking the top compartment of the slope when EE slipped causing their knee to strike the belt structure.
Employee was walking and stepped on uneven ground causing EE's left knee to hurt, EE continued to work until the doctor took EE off of work on 01/09/2024.
Employee tripped over a cable causing EE to fall down and now has pain in the right shoulder, resulting in EE seeing a doctor and started missing work on 08/19/2023.
Contractor foreman that was over a crew for Dynamic Plant Services, LLC contractor company I.D. #C5098 was getting on the vertical man elevator to come down to the bottom floor when they missed a hand hold causing them to fall to the bottom floor landing on their feet. They had a fractured wrist, foot and pelvis.
Employee was working on a new header when EE's left foot slipped causing EE's left ankle to swell up. EE continued to work until 07-10-2023 when the doctor took EE off of work.
On 4/14/2023, the employee was walking through the airlock on the unit catching EE's foot, EE fell to the mine floor, landing on EE's left shoulder causing pain. The employee continued to work, started missing work on 8/12/2024.
Employee was walking around EE's car when EE tripped on a cable and fell into the car causing EE to fracture two ribs resulting in EE missing work.
Employee was going through a man door when EE tripped and fell on EE's left wrist. EE continued working and went to the doctor on 01/31/2023 and EE said EE has a hair line fracture. EE has not missed a shift of work at EE's regular job.
Employee was walking around the feeder and stepped in a rut twisting left knee. EE has had both knees replaced in the past. EE continued to work until EE was taken off of work by their doctor on 01/17/2023.
Employee was walking and slipped causing EE to hit head on the handrail. EE continued to work until 12/26/22 when the doctor put EE off of work with restriction. EE came back to work on 01/04/2023 full duty. *EE had surgery for the same injury on 6/26/2023.
Employee was getting out of dust hauler when employee slipped and landed on right shoulder resulting in the doctor giving employee restrictions so employee could not work on 12/15/2022.
Employee was walking across the unit, stepped into a hole causing back to hurt resulting in employee being taken off of work by a doctor on 09/06/2022.
Employee was cleaning off the top of the miner when right forearm got cut on some slate when employee slipped. Employee had four stitches and returned to work the same day.

Citation 9534979 was issued during a Part 50 Audit with regard to this injury. The employee slipped and fell on EE's right shoulder as EE was trying to pull a curtain down that was hung over a pallet of concrete blocks.
Employee fell while hooking up chains to move a Connex box. EE bruised their knees, right elbow and hip.

Employee was walking along a belt line when employee tripped and fell forward. While falling employee tried to catch self by reaching for the belt structure with right hand. Employee lacerated little and ring fingers and fractured little finger in the process.
