Employee was coming down from a ladder and missed the last step. When they stepped down to the ground, they rolled their ankle slightly. Employee stated that they did not think it was an issue and filed a report when they could not put much weight on it the next day.

Employee was getting his lunch box out of the pickup truck and got his foot stuck in the mud. When he went to leave he twisted his right knee.
Employee was walking around the connex area when a rock rolled out from under his foot. He fell and hurt his lower back.

Ladder was leaned against a 20 ft wall. Ladder should have been leveled and tied off before worker attempted a small drill hole near the top. Ladder slipped, and worker fell. Broken both wrists/arm, finger, impact left elbow, left knee, left shoulder, and ribs. Face injury, teeth.
Employee reported this injury at least one month late, continued to work. Mentioned headaches, co. immed. scheduled Dr. apt. He fell 8 ft off ladder, in mid/late January. Descending a ladder, bottom slipped, fell to the floor. Got up, said he was fine, continued working. Four Witnesses.
Working on floor, tripped on something (wood or rock) fell and landed hitting his right hand on a sharp rock.
