Showing all 19The employee was conduction his work place exam. While inspecting the back he tripped over a rock and fell. When he put out his hand to catch himself, as he fell his right palm landed on a rock cutting though his glove, causing a laceration to his palm.
EE was working in a cribbed manway in the 5050 8N, he started to feel light headed and said he blacked out for a split second and fell approximately 10' to 15' to the landing. EE sustained a bruise to his left leg from the fall.
EE was using a 4' pipe wrench, in the drill doctor shop, to loosen up a part on a jack-leg drill leg. When EE put pressure to the wrench, the wrench slipped causing EE to lose his balance forward and when he took a step to catch himself, he twisted his left ankle.

When ee finished drilling a blast hole in the burn, he started pulling the drill steel out of the hole and the steel puller bounced up allowing the drill steel to come out of the drill causing ee to lose his balance and he fell backwards on his shoulder.

Welder & operator were moving steel plate to a rack. The welder was guiding a sheet into the rack while being lowered by the forklift. The steel made a movement. The welder moved to stay clear and slipped in the mud and strained his left knee. He was examined by site first responders and taken to the local doctor as a precautionary measure. Employee was doing the work place inspection, when he noticed the area was wet with debris. Employee turned around to leave when he lost his footing causing the employee to fall through the mill hole 40'.
The employee was exiting off the bus to start dayshift, when she stepped down and into a frozen tire rut. This caused the ankle to twist awkward and employee fell.

Employee was adjusting the forks on the bobcat forklift, while doing this procedure the employee got a large amount of grese on his gloves. As the employee was pushing the forks into place, he slipped off the forks and his left shoulder hit the frame of the forklift.
Employee was walking on a muck bound draw point hole, when the muck gave away. Causing the employee to fall approximately 10' from his heading.
Employee was working about eight feet off the ground installing steel pipe to pump concrete. He had one foot on an extension ladder and the other on a wooden stull. The stull gave way and the employee fell catching his arm on the pipe on the way down.
In the process of desending drill deck to retrieve gloves, stepped off top deck, missed second step and twisted right ankle.
