While operator was climbing down the ladder of his Rock Truck his boot drug on one step and missed the next step causing him to fall approximately 3 feet to the ground.

Ground frozen and covered with approx. 1 inch ice and 1/4 inch snow. Driver got out to check tailgate and slipped - spraining left ankle.

Truck driver was hooking lines onto trailer behind tractor cab. Started to dismount tractor deck, missed step and fell on left shoulder. Driver declined squad but agreed to be taken to ER to be checked out. Driver broke left collar bone, cracked or broke multiple ribs on left side and received scraps to left forearm.

Sands Hill Mining LLC · Fall to lower level, (Not Elsewhere Classified)

Top rail of handrail was removed to provide service truck boom access to replace crusher parts. As the boom was being lowered at job completion victim stood and stepped back; striking middle rail of handrail. He fell approx 11 feet landing on his feet, and then right side. Injury occurred to Rt. ankle, Rt. elbow and Rt. shoulder.
