Taken to Doctor as precautionary measures. Diagnosis contusion to side. Returned to work with restrictions. Reportable due to restrictions.
Employee was entering MCC room. When switching his weight to his right leg his foot slipped out from under him. He placed his right arm out to catch his fall causing the injury. Returned to work without restrictions 3/24/15. Returned to doctor for evaluation. Reportable as of 4/22/15 for medical treatment. Physical Therapy ordered for tendinosis.
Operator was walking away from loader. He turned to the right, twisting his right knee. Was seen at ER, prescription medication given. No restrictions, no days lost. Reportable due to prescription.
Employee was climbing down steps of loader, while stepping off last step his foot was on uneven ground, which caused him to twist ankle. Employee was taken to doctor. Sprained ankle and prescription medication given. Employee's work activities were restricted to prevent aggravation of injury.
