Showing all 3Employee was helping to unload pipe, stumbled, fell on ground, hurting hip. Employee reported incident to supervisor but was OK and continued to work. One week later employee complained of pain in hip. EE was sent to doctor for examination. Dr found no problems prescribed a cortisone pill for one week and sent employee back to work no restrictions or lost time.
Miner exited excavator to break for lunch on 6/29/15 and stepped down into a rut. Miner felt a pop in his back. Miner completed shift. Miner reported to work at 4:00AM on 6/30 and performed normal duties. At approximately 7:00AM on 6/30, miner reported injury to supervisor.
