Showing all 38Getting rope off of cable, EE hit an airline, the force of the air knocked EE to the ground injuring EE's right knee. EE was taken off work on 5/18/2026 for surgery.
As EE stood up getting off the mantrip, EE felt a sharp pain in EE's right knee. EE went to take a step and EE's knee failed causing EE to fall to the ground.
Standing on a ladder knocking a Huk bolt loose on the wear-liner of the rotary breaker chute when the ladder slipped causing EE to fall over the guardrail onto the ground.
Walking back from #2 entry at crosscut #42 to the longwall face, they slipped in a wet product from the pumpable cribs, twisting left ankle.

The employee was walking and slipped on slickline, twisting lower back. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Left side bolter lost suction bolter on right side. Employee went to help, on the way back around, on the back side of the bolter, they slipped on a muddy rock and hyperextended their right knee. Was able to finish shift. They continued working and was taken off work on 7/20/2023 by doctor.

Putting in belt structure during belt move, EE stepped in hole and felt left knee pop.
Walking at Power Boxes, EE stepped on the miner cable with left foot. Lost footing and twisted left knee, lost balance and fell to the ground.
Loading top of Head Drive with crib block. Carrying two cribs on EE's left shoulder, EE lost EE's footing causing EE to fall forward. The cribs slid forward from EE's shoulder hitting EE's right middle finger against the side of the drive.

Was getting out of scoop when EE's left foot slid under the control panel in the cab and stepped out of the cab with EE's right foot. EE's right foot rolled and EE lost EE's balance falling to the ground with EE's left foot still in the cab under the control panel.
While walking by the miner their feet slid out from under them, when they reached out to catch themselves with their left hand, wrist hit a miner bit on the head of the miner.
Helping remove an impact roller from the magnet area. As EE was walking away with the roller, EE tripped on a waterline and fell with the forearm hitting the bracket that the roller mounts in. Received stitches to the arm.
EE exited the road grader to clean off the camera. When walking around machine, EE slipped catching themselves with left arm on the grader. EE felt pain in the shoulder area. EE was taken off work May 1st for surgery.
While washing the Ram Car at the shop, employee stepped over the center section of the car and slipped in the hole on the bed of the Ram Car, and fell striking head on the bed of the car. EE was taken to hospital and received staples in the head.

While packing steel bar, employee tripped causing the piece of material to fall on employee's hand, cutting the index finger on employee's right hand. This caused employee to receive stitched.
EE was changing out bret-by on the shear carrying two pieces at a time from the head drive to the mid-face. While getting through the head gate shields with the bret-by ee fell forward, leg stayed between the shield foot, causing a pop along with pain in right knee. *Became medical treatment due to surgery on 5/26/17*
Employee got down off the dozer at the edge of the clean coal pile and was walking toward the office, employee stepped in a rut and felt pain in the right knee. Employee had surgery to repair the knee on May 17, 2017.
Trip / Struck against. On Nov 29th, 2016 the injured miner was walking to the mantrip that was parked in the crosscut between entry #1 and entry #2. Miner stepped on uneven footing and stumbled about 10 feet, falling into the seat rail of the mantrip. Striking left ribs and cutting left arm. Resulting in fractured ribs.
EE was walking the long wall face. When EE crossed shield to get to the tailgate entry, EE fell, striking arm on a sharp rock, causing a laceration.
He was using a stepladder while sealing a stopping. He reached over to get more plaster out of the bucket, the ladder tipped over; he then leaped off and landed on his feet. Felt pain in his mid-back.
A Pillar Innovations employee was working in the 5 Left Mains. The employee must mount and dismount the belt line which is elevated approximately 3' 1/2" from the ground. The employee stepped off the belt line and on to an uneven surface, rolling his ankle. Employee suffered an ankle sprain, the doctor did assign an air cast to be worn out of the medical office.
Coming out of a Kennedy, went to step down off bank and slipped on loose rock, threw off his balance and landed on his shoulder.
Employee was working on a shield hauler, while walking around the left side of the machine, he stepped on a rib rash and rolled his left foot inwards and heard a pop.
