The employee was exiting a pickup and tripped and fell out onto the ground. The foot was tangled in wiring fed through the passenger door. After medical review it was determined that the right ankle was fractured and that EE would need to wear a boot for three (3) weeks.

Employee reported to line-out meeting at beginning of the shift and was attending a safety presentation that was to be 1 1/2 hours in duration. 30 minutes into the training, the employee was not feeling well and was going to go get some fresh air. The employee stood up and fainted and dropped to the floor. This resulted in a non-displaced hairline fracture of upper jaw.

Employee was evaluating a stuck vehicle and was walking to the rear of the vehicle and tripped on a windrow of rocks. While stumbling forward, the employee planted their right foot and felt a pop in their right knee.

The employee was leaving Mine Medical on her way to the warehouse. The employee was stepping off of the last step of four steps when her knee gave out. She tried catching herself unsuccessfully and fell to the ground contacting her face onto the ground. This caused a chipped tooth, facial abrasions and contusion on her left knee.

While rappelling during a Mine Rescue Training, the employees left knee twisted when they contacted the ground. The employee did not seek any medical attention for several weeks. The ACL was torn and required surgery on 9/16/15. Work activities were not restricted until that date.

An employee stepped in a parking ditch causing his knee to pop and causing pain. The employee was seen by an orthopedic specialist and an MRI was done on 10/17/14. This was the soonest he could be seen. He was released back to work on restricted duty on 10/17/14.

An employee was descending a platform ladder and slipped on the third step down causing him to twist his ankle. The employee resumed work without informing his supervisor of the incident. The employee had the ankle checked the next day and X-rays revealed that it was broken.

The employee stepped onto the deck of an overburden drill that was wet due to rain and slipped. This caused him to twist his knee. The knee required surgery 20 days later.

The employee was walking on the deck of a haul truck with both of her hands full when she tripped and fell. When the employee fell, her head came into contact with the coolant reservoir tank causing a slight fracture to the employee's forehead.
