Showing all 8Injured employee was walking along the right side of roof bolter heading towards the drill boom. About mid-machine, the IE stepped on a slick rock causing EE to lose balance and fall. When the IE struck the mine floor, EE's chin hit a rock causing a laceration just below the lip.
On 10/20/21 IE was walking the slope belt during an on-shift examination. IE stated IE stepped down off a manway door (approximately 4' - 5') and IE's knee buckled resulting in pain. The IE continued to work, on 12/29/2021 IE began receiving a physical therapy that per the part 50 guidelines makes this incident reportable.
Employee stepped in a hole twisting left knee. Employee was diagnosed with a lateral meniscal tear.

IE was loading supplies on the roof bolter tray when IE slipped on a berm/rock and twisted left foot and fell. IE received a fracture to the lower left leg. Left distal fibula fracture.
IE stepped out of ram car and tripped over a curtain lying on the mine floor. EE stumbled and struck left forearm on a coal rib. EE received a laceration that took 5 sutures to close.
IE was hanging high voltage cable. The cable got hung on itself coming out of the sled causing tension on the cable. This energy pushed IE into the sled twisting right ankle. Employee had scheduled vacation starting the next day. After returning from vacation and still experiencing pain, IE returned to doctor and was taken off work 7/24/17.
