Gateway North Mine Coal

A contractor and employee were involved in a verbal argument that turned physical while in a mantrip. The boss stopped the mantrip and both the employee and contractor rolled out of the man trip and continued fighting until the other employees in the mantrip separated them. IE sustained 16 stiches in the ear and a spot on the face that was glued.

IE stepped over a pile of coal and fell on left knee. IE now has compartment syndrome and swelling in left calf. Off work for 2 to 6 weeks.

IE was changing scoop battery, and the nip was tight, so IE claims they twisted harder and felt pain in their right thumb. Incident happened around 12:15 AM on the 5th. IE was on midnight shift. IE worked at full duty until 11/25/24 when at a follow up appointment, they were given restrictions and a thumb splint. IE is still working but light duty.

IE was hanging tie wire for bolter cable across entry #3 in unit 2. The ends of the tie wire were bent and when IE let go of the wire it flexed and hooked under their safety glasses, striking them in the left eye. IE received a prescription eye drop and returned to work.

IE lifted cable & reported back pain and was evaluated with no acute injuries and a back strain. IE was given a full release and could perform all job duties. IE was moved to an alternate role for training for upcoming vacancies. Despite being released, an MSHA inspector believed this met the criteria of RWC. We disagree but are reporting the case.

IE was bolting on left side of unit 1 in entry 5 installing hog panel. When IE lifted the sheet of hog panel over the canopy it hung up causing them to strain their right shoulder. IE continued to work full duty until 11/26/24, when they were seen by a doctor and given restrictions until MRI could be performed.

Carrying sheet of hog panel over the roof bolter to put on ATRS and IE hurt right shoulder. On 12/31/2024 IE went to doctor and received restrictions that we could not accommodate with EE's normal work duties.

IE got off mantrip ran into a piece of low hanging hog panel. Jamming neck and having shoulder pain.

Hands and elbows have started hurting and going to sleep from repetitive motion. No actual incident. IE first day missed from work was 4/14/2025.

IE was loading pinner when a roof bolt plate slipped in IE's arms and Kevlar sleeve rolled up and the plate cut IE on left forearm. Unit 3 room #4, 1+80.

IE had an audiogram and the miner incurred a reportable hearing loss. Miner was over the threshold limit.

IE complains of numbness in both hands on a regular basis. IE also complaining about having trouble squeezing for long period of times and cramping. Employee continued to work regular duty until surgery on 11/1/2024

A scoop was driving through a wing curtain and caught it on the canopy. IE asked for the scoop operator to stop and shut down the machine, but IE hurried in too quickly to free the curtain before the emergency brake had time to fully set. The scoop was on a slight incline and rolled a few inches onto the IE's right foot. IE suffered two broken metatarsals.

IE claims right and left hand go numb. No actual incident. IE first day missed was 6/5/2025.

IE was walking in uncleaned x-cut between entry #4 and #5 in Unit 2. IE stated there was no incident EE just heard a pop in EE's left foot.

IE was loading 2 x 4 on to the battery scoop. As IE was pulling on board their right ring finger was caught in between the end of the board and scoop battery.

IE was changing battery on ram car. IE unplugged from the new battery and was throwing the slack out of the way of dead battery. IE felt a pop in right shoulder.

Employee was conducting maintenance and was replacing a coil spring when the trailing arm slipped out of employee's hand and struck employee's middle finger causing injury

A continuous miner operator had completed a cut and was in the process of tramming to the next place. While backing the miner out, the operator pinned self between the coal rib and the tail of the continuous miner. EE was able to free self using the remote control.

IE was hit by a rock in the lower right leg above the ankle, clearing rock of the duct work of a miner. The IE continued to work thinking the injury was not anything serious, but recently claimed it has been getting worse. The IE was seen by an Orthopedic doctor and was taken off work on 09/01/23 for a small tear in the calf muscle and placed in a walking boot.

IE stated has been experiencing tingling and numbness in both hands. The tingling and numbness seem to get worse while roof bolting. IE said started experiencing these symptoms about six months ago. No incident triggers these symptoms, they come on from repetitive motion and use of the hands and arms.

The IE was driving a diesel scoop on the Main East travel road. The IE encountered a piece of large coal in the roadway. The diesel scoop successfully navigated over the rock, however, due to the size, the machine dropped immediately, which then projected the employee into the canopy of the diesel scoop. EE sustained injuries that include jamming EE's neck to due to the impact.

The IE was hanging roof bolter cable on the left side of U3. IE was concentrating on hanging and securing the cable to the mine roof, EE was unaware of the slack pile of roof bolter cable laying on the mine roof. IE cont. in the hanging of the cable, unknowing to EE, EE had stepped into the slack cable pile. The IE rolled EE's rt ankle, causing a sprain to the ankle.

The IE was moving power on the left side of Unit 3. The IE was working out of the high voltage cable tub, aiding in the installation process of hanging the high voltage cable to the mine roof. While pulling and lifting the cable out of the tub to place it into position to be secured to the mine roof, the IE felt a pop in the right bicep.

While installing the outside roof bolt, a piece of coal measuring 3.5' x 1.5' x 1' broke loose from the roof, near the rib that was not cut down by the continuous miner. This piece of coal struck the roof bolter on the right ankle resulting in a fracture to that ankle.

IE was walking leaving Unit 2 RM 1 and was struck by rock that fell from the roof. Small pieces fell first, then a piece measuring (3'.5" L x 2'.2" W x 4' T) struck EE in the hip area. The ER X-rays were negative, and IE was released to work as tolerated. A follow up MRI showed a closed stress fracture of left inferior pubic ramus of the pelvis.

IE was handling cable for the roof bolter backing the machine out of entry 2. The cable became tight and hit the IE below the knees, causing them to fall. The IE reached out with arms to catch themselves as they hit the floor with both hands then rolled on to the left shoulder. As of 9/26/2023 IE turned in voluntary resignation.

Injured employee was walking along the right side of roof bolter heading towards the drill boom. About mid-machine, the IE stepped on a slick rock causing EE to lose balance and fall. When the IE struck the mine floor, EE's chin hit a rock causing a laceration just below the lip.

A contract employee was building a concrete block stopping in Unit 1. The stopping was approximately 75% complete when EE decided to install cap wedges to secure it against the roof. As EE was preforming this process, the top portion of the wall fell over, striking EE on both legs and lower torso.

The IE bent over to pick one side of a piece of wire mesh. As the IE went to lift it over their head, IE felt a pop in the lower part of their back. IE continued to work until 10/16/2024.

IE was roof bolting on Unit 3. As the IE was installing a 6' fully grouted bolt, the bearing plate wasn't orientated in the right direction. The IE reached up with the right hand to position the plate, simultaneously pushing the bolt into the hole, pinching the tip of the right index finger between the plate and roof.

On January 10th, the IE was working in the ME battery barn at 129 x-cut. The IE leaned down to turn a battery charger up, when IE stepped on a rock about the size of a soft ball and twisted left knee. The IE continued to work until May 15th, where the IE underwent surgery to repair the injury to the knee.

A contract employee was spreading rock dusting by hand. As the employee reached to pick up another bag of rock dust from the scoop, a rock fell from between the roof bolts, striking employee on the left forearm and wrist. X-rays show two fractures, one in the forearm and the other in the hand.

The IE was kneeling on one knee marking up EE's section cut map. The IE heard a pop in the roof right above EE and immediately moved to get out of the way. As the IE stood up and stepped away, a rock fell from between the bolts striking the left leg at mid-calf. X-rays showed no broken bones, IE missed work on 12/19/22 making this reportable.

While in the process of loading behind the miner. A piece of coal measuring (14' x 13' x 9') was discharged from the tail end of the miner conveyor tail traveling 10.5 feet, striking the IE's right hand positioned on the bed controls in the cab. This resulted in a laceration to the inside bottom of the little finger that required 7 sutures to close the wound.

IE was cutting down loose screen wire to set a timber. The IE was under the wire when strains that were rusted broke, allowing the rock debris on it to slide off, striking the back of IE's neck and left shoulder. The IE continued to work until 04/03/2023 when doctor placed IE off work

IE was in Unit 1 entry #3, removing a wooden pallet from the scoop bucket. Loose material fell from between roof bolts and struck IE in the right upper portion of their body. First aid was administered, IE was transported out of the mine and taken to the emergency room.

A contract employee was recovering H/V cable into a tub secured to a wheeled trailer in old Unit 2 Panel. As they leaned over the side of the tub to help spool the cable, the wheel of the trailer ran over the left foot. A diesel scoop was towing the trailer at the time of the accident.

IE was pulling 2 sleds with a scoop. Sleds were attached using (2) 6 foot 2" dia. polyester slings wrapped around (2) 1" eyebolts attached to the front corners of the tool sled. As the scoop was turning into a x-cut, this created tension on the R/S sling which caused that eyebolt to break. The broken eyebolt projected into the cab of the scoop striking IE on the R/S of the head.

IE has been dealing with lower back pain on and off for over 12 months. No incident triggered the pain, but it has gradually progressed where a medical procedure was performed on 9/19/22. The IE has been working in the industry for the past 34 yrs. in various roles, that involved lifting, pulling, bending carrying and kneeling which have injured the IE's lower back.

A contract employee was helping move roof bolts from the rear of a roof bolter to the side boom tray. A rock measuring 2'8" X 2'2" X 1'6" (approximately 380 lbs) fell from the roof at a trimmed corner. When this corner was trimmed, an arched kerf was left from the head of the miner. Part of the arched kerf fell and struck the miner. The rock fell between the roof bolts and the rib.

Two mantrips were traveling to Unit 2. A BTV pulled off the travel road to allow the first mantrip to pass. The first mantrip passed by, the BTV flashed lights then pulled out of the crosscut striking the second mantrip. The IC in the second ride experienced some bruising to the left torso. The IC started missing work on 2/24/22. *CT on 3/7/22 revealed fractures of ribs.

The IE was using a hand drill to bore out a broken bolt to a tram lever on a roof bolter. As the IE applied pressure to the drill, the 3/16" bit scattered causing a fragment of metal to become logged in the corner of the right eye. The IE was transported to the Sparta Community Hospital ER where medical treatment was administrated, to care for the eye.

On 12/27/21 while drilling a hole to install a 6' roof bolt. The IC had a premade roof bolt leaning against the drill boom tray. Unknowing the location of the base of the bolt, as the IC lowered the drill boom it struck the bolt causing it to strike the right knee. The IC continued to work until placed on restricted duty by and Orthopedic doctor on 1/4/22.

The IE was loading 6" x 16" roof bolt plates onto the tray of the drill boom. When the IE turned around to go get more roof bolts, IE caught the edge of a plate on the tray with right forearm. The IE was transported to ER at a hospital where IE received 11 sutures.

IE was drilling a roof bolt hole, a hyd. hose on the bottom of the boom was hung under a piece of rock. IE continued drilling upwards, unknowing the hose was stuck under the rock, the hose broke at a fitting as it was stretched, allowing it to swing up striking the IE above the right eyebrow. The IE was taken to Sparta Community hospital where IE received 3 sutures.

On 10/20/21 IE was walking the slope belt during an on-shift examination. IE stated IE stepped down off a manway door (approximately 4' - 5') and IE's knee buckled resulting in pain. The IE continued to work, on 12/29/2021 IE began receiving a physical therapy that per the part 50 guidelines makes this incident reportable.

On 09/22/21 IE was roof bolting on Unit 2. While installing the 3rd row of roof bolts, IE swung the drill boom towards the right rib. The IE set the drill boom. It is believed that the foot of the drill boom was either positioned on a rock or weak floor material. The base smashed the rock/weak floor and lowered the drill boom, striking the IE in the hard hat.

IE reported on 8/28/21 that the pain in both knees has increased over the last 4 months. Orthopedic Doctor recommended the replacement of both knees due to excessive amounts of bending, kneeling & walking IE has experienced working underground. The IE continued to work until 11/29/21 where IE underwent surgery to replace left knee. Right knee replacement will be scheduled later.

IE was installing Rail Bars (secondary roof support) on the Unit 1 travel road at 14 x-cut. After the job was completed the IE mentioned to supervisor that back was sore. The IE finished the shift operating a piece of equipment.

IE walked over to check the CM that was down in E2 at 14+18. As the IE approached the CM, a rock measuring (2.5-4 W x 4 L x 5-1 T) fell out from between the roof bolts. The rock struck the IE's head and right forearm pushing IE forward. To prevent from falling forward, the IE planted right leg in front of self. The rock hit the IE's right knee as it fell to the floor.

IE was bolting in RM3 XCR at the 120' station. After IE installed 2nd bolt in the 5th row of the cut. The IE lowered the canopy to swing the boom towards the ATRS to install the last bolt. A rock measuring (4'8" L x 1" to 4" W x 9" T) broke loose from a brow in the roof sliding across the top of the canopy cantilevering over the edge striking the IE on top of the left foot.

On March 8th, the IE was walking the Main East Belt. During the examination of the belt line, the IE stepped on a rock in the walkway slipping (pivoting motion) and felt a pain in the left knee as a result of this action. The IE continued to work while receiving medical treatment and therapy for the injury. The IE had surgery on 06/24/21 to repair a medial meniscal tear.

IE was working on a brake issue on 135 belt truck. IE had to reach behind wheel to find a leaking brake line. When the left front air bag spring failed, this caused the frame to lower pinching the IE left arm between the fender well and tire.

On 01/26/21 IE injured left shoulder while pulling a piece of curtain off top of scoop to put in dumpster. Unknowing to IE there were RB plates on the curtain that added resistance jolting the IE left shoulder causing a "popping" as it was pulled off. IE had surgery on 05/13/21 to repair a tear in the Rotator Cuff.

Employee reported on 01/07/21 was seen by an orthopedic doctor who performed a nerve induction test on both wrists and diagnosed EE with Carpal Tunnel Syndrome. Employee worked up until 02/08/21, where EE underwent surgery to report the condition. The mine is submitting this as an occupational illness due to the repetitiveness of maintenance personnel.

IE was backing continuous miner out of right side of cut in room 2 at 2+10 on right side of unit 1. Section of rock fell from between bolts striking IE on top of head/right hand. IE was seen by physical therapist on 12/28/2020 due to type of therapy administered during the visit this is considered medical treatment which makes injury MSHA reportable. IE has not missed any work.

Employees were moving up miner cable on left side of Unit 2. IE dropped down part of the hanging cable when the remaining cable came loose, falling on IE's right hand pushing fingers and hand backwards.

Employee stepped in a hole twisting left knee. Employee was diagnosed with a lateral meniscal tear.

Employee reported on 9/11/20 EE had pain in right shoulder, elbow and both wrists and has been experiencing this for a couple of months. There was no incident to trigger this, it's just repetitive use of arms and hands. EE continued to work until 12/14/20 where EE underwent surgery on the right elbow and wrist, with another surgery in 12 wks. on the left wrist.

Employee was walking along slope belt examining when employee felt pain in right knee. Mine has been waiting on Dr. reports.

Empty coal hauler turned in outby of entry #6, continuous miner operator in the face of #6 entry moved behind the tail of the miner to check sights. Empty hauler proceeded to back towards miner for loading, miner operator remained behind tail. Miner operator became pinned between tail of miner and tailgate of coal hauler. Miner operator suffered severe injuries to upper R leg.

Employee reported pain in right elbow on 6/17/2020 to the supervisor. EE was diagnosed with Lateral Epicondylitis.

Employee has worked underground for 13 years. On 4/22/2020 employee was walking a belt line when employee felt pain in right knee. Employee had a right arthroscopy on 6/3/2020. Doctor's notes attached.

Employee has worked underground for 18 years with several of them as a roof bolter operator that requires a lot of repetitive motion. On 2/25/2020 EE reported to the safety department that EE was having pain in the left shoulder. MRI results show tendiopathy, bursitis, osteoarthritis, tear in left shoulder. Surgery performed on 4/14/2020. Doctors notes are attached.

IE was pulling on a blown hydraulic hose on a slinger bucket duster. The hose was in a bind causing IE to pull harder and EE felt pain in the abdomen. IE continued to work up until 01/10/2020 when EE was diagnosed with an umbilical hernia.

IE was standing beside a slack pile of roof bolter cable as it was being reeled up. The cable became tight and made contact with the top of EE's left hand and wrist. EE received a fracture of the scaphoid bone of the left wrist.

IE was helping carry a sheet of hog panel up to their roof bolter. They felt pain in left shoulder. IE continued to work up until 12/20/2019 when they went in for debridement of the left labrum.

IE was removing a steel cover over the foot shaft on a belt feeder. The plate fell and struck IE on top of the left hand. IE received 3 closed fractures to the left hand.

EE was standing under EE's roof bolter canopy when bar slate fell out behind EE. The slate struck the roof bolter then broke up and struck EE on the backside of EE's lower legs. EE received abrasions and contusion to EE's lower legs.

An employee was attempting to remove a miner bit from a bit cone. As employee struck the bottom of the bit a piece from the bottom side of the miner bit broke off and pierced IE in the upper right quadrant. The approximate 1/4 in. piece had to be removed.

IE and another employee lifted a ram car ejector cylinder out of the back of maintenance ride #710 and set it on the mine floor. Employee stated EE had pain in groin area. EE continued to work until 7/29/19. EE was diagnosed with an inguinal hernia on 8/5/19.

IE was roof bolting pushing a roof bolt into the hole. The bolt head was not firmly seated into the drill chuck. EE reached towards the roof bolt and inadvertently hit the drill rotation causing the right hand to be caught between the roof bolt head and the drill check. Fractured lower right arm.

At approximately 11:55am a loud noise occurred that alerted the office personnel. Upon investigation it was discovered the top of the surface bulk hydraulic oil tank had blown off. Investigation revealed the hydraulic oil level was lower than the electric heating element warming vapors to combustible levels. No one was in the area at the time of the event.

IE was loading supplies on the roof bolter tray when IE slipped on a berm/rock and twisted left foot and fell. IE received a fracture to the lower left leg. Left distal fibula fracture.

Employee is a roof bolter who does repetitive work daily. Employee reported to the safety department on 1/29/19 pain in left wrist and forearm. No specific event or incident was reported. Employee continued to work until 2/7/19 when employee was place on restricted duty. Employee was diagnosed with peritendinitis/tenosynovitis. See attached MRI report.

EE is a repairman who slipped on a continuous miner cable and water hose on 1/23/19. EE reported to the safety department on 1/29/19 that EE needed to seek medical attention. Doctors reports and MRI results are attached that reveal degenerative, osteoarthritis, atrophy to EE's left knee and right shoulder and right elbow. Mine has been waiting on Dr reports.

IE was struck by a rock that fell out between roof bolts. EE received a laceration to EE's left ear and a fracture to the second metatarsal of EE's right foot.

EE was hanging roof bolter cable. EE was hanging the cable above their head when they felt a pop in the left shoulder. EE continued to work until 8/8/2019 when they had surgery to repair a tear to the left labrum.

IE was working on a Mini Track using a pry bar. IE felt pain in IE's right shoulder. IE continued to work up until December 21st when IE was placed on restrictive duty for a right shoulder torn rotator cuff. IE is off work awaiting rotator cuff surgery.

IE was installing roof bolts. As they pushed a roof bolt up into the hole the bolt bent causing pressure on the drill wrench. The drill wrench broke striking them in the mouth. Employee received 1 broken tooth and a laceration on their lip requiring 2 sutures.

IE was carrying a wooden curtain board. EE walked into the back of a roof bolting machine and the curtain board struck EE in the mouth. EE received a chipped right front tooth. EE had it repaired on 10/29/2018.

IE stepped out of ram car and tripped over a curtain lying on the mine floor. EE stumbled and struck left forearm on a coal rib. EE received a laceration that took 5 sutures to close.

IE (Temporary Employee) was cleaning roof rock off of a parked coal hauler. A piece of rock approximately 3.5 ft. x 1-2 ft. x 2 in. fell out between the roof bolts striking IE on top of the left hand. IE received a fracture to the middle finger of the left hand.

IE (Temporary Employee) was capping off a hollow block stopping. As EE reached for a wedge part of EE stopping blocks fell striking EE on the right leg. EE received a strain to EE's right knee and is off work.

IE was operating a boom hoist on the back of a maintenance service vehicle. As ee was retracting the hoist rope ee placed right hand between the rope and the hoist arm. IE had right middle finger pinched between the rope and hoist arm. EE received a fracture to the tip of right middle finger.

An unplanned roof fall occurred at x/c 4 of the Unit 1, #3 belt. The fall measured approximately 50' long, 20' wide, 4'to 5' thick. The area was bolted with 4' fully grouted roof bolts. Approximately 50' of belt structure was damaged.

IE had a piece of roof rock approximately 2ft. x 2ft. x 0-6in. fall out between roof bolts. The rock either hit the rib or the ground then made contact with EE's right foot causing EE to twist right knee. EE has a right knee strain.

IE bent down to pick up a battery jumper cable and felt a pop in IE's back. IE was transported out of the mine for medical treatment. IE has been diagnosed with an annular tear and disc herniation of L4-5.

IE was hanging high voltage cable. The cable got hung on itself coming out of the sled causing tension on the cable. This energy pushed IE into the sled twisting right ankle. Employee had scheduled vacation starting the next day. After returning from vacation and still experiencing pain, IE returned to doctor and was taken off work 7/24/17.

IE (Temp. Employee) was examining the roof on Unit #2 when a rock approximately 2' X 4.5' X 1-8" fell out between roof bolts striking IE on the left upper arm. The laceration required 7 sutures to close.

IE (Temp. Employee) was backing roof bolter out from the face using a remote when the IE was struck by top coal/rock on the top of the head and lower left leg. The top coal/rock was approximately 3ft. X 8ft. X 0-3.5in. IE sustained a fractured tibia and fibula of the left leg.

EE was removing a top roller segment from a damaged belt rail. As EE removed the roller the belt rail shifted and struck EE on the front of the right thigh. EE received abrasions and a large contusion to the right thigh.

IE (Temp. employee) was roof bolting on the right side of Unit 2 in Room 2 at 1+20. EE was installing a roof bolt and reached up to rotate the roof bolt plate when EE got right index finger caught between the plate and the mine roof. EE received a facture to the tip of the finger and (3) sutures to close the laceration.

IE (Temp Employee) was changing the battery on the battery coal hauler. The plug wrench slipped and EE lost balance and fell onto outstretched left wrist. Fractured left wrist.

AN UNPLANNED ROOF FALL OCCURRED AT XC128, ENTRY 5, ON THE 1ST NORTH SUBMAIN. THE FALL MEASURED 30' LONG X 19' WIDE X 7.5' THICK. THE AREA WAS BOLTED WITH 6' DOUBLE LOCK BOLTS. NO INJURIES WERE SUSTAINED, APPROX. 40' OF BELT STRUCTURE WAS DAMAGED.

Employee reported pain/discomfort in lower right abdomen on 11/2/2016 to the safety department. EE stated EE is not sure when the pain started but on 11/1/2016 EE was building a hollow block stopping. Employee has been diagnosed with an inguinal hernia and first missed work day was 11/7/2016. Reporting as an illness due to no known event occurring.

Employee alleged Carpal Tunnel syndrome due to working in the coal mine on 10/11/2016 to the Safety department. EE has Carpal Tunnel in both hands and had surgery on left hand on January 13,2017.

IE stepped on a rock causing EE's left knee to buckle and pop. EE continued to work up until January 18, 2017 when EE had surgery to repair an acute lateral meniscus tear of the left knee.

IE was partially struck by a rock that fell out between roof bolts. The rock measured approximately 7'x1.5'-3'x14". EE received a laceration under right eye requiring 6 sutures and a sprained right ankle.

IE was struck by a piece of roof rock. The rock approximately 6" x 8" x 0-2" thick slid off the end of wire roof mesh and struck EE on the top side of EE's left forearm. This resulted in a laceration requiring 7 sutures to close.

Employee was using their hands attempting to slide a rock forward off of wire roof mesh. As employee was doing this a piece of the rock broke off causing employee's left wrist to come into contact with a sharp edge of the rock. This resulted in a laceration requiring 9 sutures.

EE (Temporary Employee) was roof bolting when a piece of roof rock approximately 1ft. x 1ft. x 1-4in. fell and struck him on the right hand. He received a laceration and a fracture to his right ring finger. 4 sutures to close laceration and finger splinted.

Employee alleged Carpal Tunnel syndrome due to working in the coal mine on 12/09/2015 to the Safety department. He had Carpal Tunnel surgery on his left hand on February 10, 2016.

EE stated that his left knee was bothering him on 12/9/2015 as he walked. He reported this to his supervisor on 12/13/2015. He continued to work until he had surgery on his left knee on 3/1/2016. This is being reported as an occupational illness. Attached MRI noted degenerative chondromalacia and degenerative meniscal deficiency. Additional medical information is attached.

THE EMERGENCY HOIST WAS ALARMING AT THE UNDERGROUND LANDING, UPON INVESTIGATION IT WAS FOUND ONE OF THE "SLAVE DRIVE" UNITS HAD MALFUNCTIONED. THE HOIST WAS MADE OPERATIONAL AND BACK IN SERVICE AT 6:50 PM.

EE was hanging up a ram car battery jumper cable when the cable fell from an insulated hook and struck EE on the right knee. EE received a contusion to the right knee.

IE (Temporary Employee) was riding in a mantrip entering the mine. As the mantrip turned the corner at the bottom of the slope the IE's left foot was pinched between the steel plate on the arches and the frame of the mantrip. He received a contusion of his left foot and was taken off work.

IE was roof bolting when his drill steel plugged with wet material. He was striking the steel against his drill boom when he struck himself in the mouth with the drill steel/drill bit. 3 sutures to close the laceration.

IE was loading roof bolts plates onto a roof bolter. The top stack of plates slid off and struck him on top of his left wrist causing a laceration approximately one inch long. 3 sutures required to close laceration.

The employee was struck in the lower leg by a steel plate diesel scoop attachment. The employee was positioned next to the storage stand while spotting for the operator. He sustained a puncture wound of the shin area. X-rays of his foot were negative. He was treated and returned to work.

The employee was moving an air tugger down slope. The tugger wedged between arch structure and belt structure. The employee attempted to free the tugger with a pry-bar. The tugger shifted as the employee attempted to step around it to pry from a different angle. The tugger struck the employee in the lower leg. No medical treatment until 10/12/2014. No work missed until 10/19/2014.
