Showing all 10Employee was stepping onto the pedal of a bicycle when foot slipped causing the bike and self to fall.
Employing was hydro-blast under Precipitator #6. Positioning platform shifted due to back-thrust on blasting shotgun. Employee lost footing and struck elbow on guardrail.
Driver (Sun Coast employee) fell from 2' height when handrail on a tank platform/walkway driver leaned on broke. Driver fell to the ground and broke upper arm. The handrail was constructed of wood and was in a weathered condition.
Employee leaned on handrail of walkway and it broke, causing EE to fall several feet to the ground and break EE's arm. Accident originally reported to OSHA under SER 56522 before it was know this was a MSHA-regulated location. Reporting now due to oversight of reporting to MSHA at the time.
GRM in Area 340 went to install a valve on a vacuum flash pump when left foot slipped resulting in EE tearing calf muscle. EE was monitored daily for one week, then was seen every other day for the second week. In the third week, on 10/17 EE had swelling in the lower leg & saw orthopaedic doctor and was diagnosed with a tear. Physical therapy was ordered twice weekly.
Employee was walking between the ditch and the ESP MCC when employee slipped in the wet ESP dust and twisted ankle. Medical evaluation resulted in prescription medication and a (R) fibula fx.
Supervisor was walking in Area 345 when he tripped over slightly uneven grating falling forward & catching himself with his hands. Received laceration to the left palm as a result of fall.
GRM reported going to take some steam racer doors off #3 cyclone. Enroute, while walking up first flight of stairs, after climbing about 7 steps, he felt a pop in the left knee. Iced x 25 minutes for minimal swelling. Taken to Dr. for x-ray. Restricted with climbing activities.
Employee was climbing down a ladder from an elevated platform when he lost his balance and fell approximately 5 feet to the floor.
