While performing a pre-op inspection on a mobile screening plant, EE slipped off EE's step because EE's boots were covered in mud. EE caught self from falling but twisted EE's wrist in the process.
EE was doing maintenance on front-end loader. EE missed a step coming down the ladder and fell on back. Safety took EE to a hospital. The hospital did a CT scan that found 4 compression fractures in EE's vertebrae and 1 broken rib. EE was able to go home that day and is seeing a spine and orthopedic specialist.
EE was standing on the tracks of the excavator to fuel. EE gave a thumbs up sign to another operator in the area. The fuel truck driver thought the signal was to ee and the fueling process was complete. The fuel truck driver began to pull away while fueling was still in progress. The operator jumped off the tracks (approximately 2 ft). EE fell onto right shoulder.
A truck entering the site was on the scale and while the driver was checking in with the scale operator something in the engine ignited starting a fire. Our employee, while attempting to put the fire out with a fire extinguisher was blown backwards when a tire blew. The employee's left shoulder was hurt when he fell backwards.
After moving rocks in a track hoe. The employee parked the equipment. While climbing down from the equipment he twisted his right ankle when he stepped onto the ground.
