Fall from machine

EE was getting out of EE's excavator and slipped off the machine. EE tried to catch self and fell to the ground. EE's arm and shoulder started to hurt. EE was taken to be seen at a local urgent care clinic.

Fall to the walkway or working surface

Employee was fueling truck and slipped and fell fracturing their #6 rib

While performing a pre-op inspection on a mobile screening plant, EE slipped off EE's step because EE's boots were covered in mud. EE caught self from falling but twisted EE's wrist in the process.

Worker was prepping for the workday and was seen acting unusual. When approached by a supervisor EE looked as if EE was in pain. Safety responded and learned that EE had heart and previous medical issues. EE was taken to a medical center emergency room and then transferred to a hospital in El Paso. We are waiting for further information.

Miner was installing a rubber guard. The rubber was too large. Miner is right hand dominant. Miner held the rubber in place with right hand and used a utility knife in left hand to cut the rubber. The knife slipped and cut the middle section of ring finger on right hand. Miner received 3 stitches.

While changing fuses in a loader, EE was in the operator seat. EE grabbed chest and fainted. Co-worker laid EE on the catwalk. Another co-worker called 911. EE woke up before co-worker could start CPR. They met the ambulance near the entrance. EE was treated and released from the Medical Center.

The loader operator was cleaning the feeder on the ramp. When he shifted gears the loader reversed and hit the rear end of a dump truck waiting to be loaded.

EE WAS TRYING TO TIGHTENING BOLT ON SCREEN THAT CAME LOOSE. IN ORDER TO GET TO BOLT, HE STEPPED OFF CAT WALK, WITH ONE FOOT ON MOTOR. AS HE WAS TIGHTENING HIS FOOT SLIPPED OFF THE MOTOR, AND F ELL 5 FEET.
