Employee was walking down #1 Entry when EE tripped over gob on the mine floor, fell toward the rib, and struck EE's right fifth digit on an installed rib bolt, causing a laceration. Showing all 37EE rolled left ankle while dismounting the Co. #402 roof bolting machine in the #5 entry due to an unseen 4"x4" hole in the mine floor. EE reported the incident later in the shift and continued working. That night they sought medical attention and was diagnosed with a sprained ankle. EE was fully released to work on 02/12/2025.
Employee was walking down track entry when EE tripped over gob along the mine rib causing EE to fall into the rib. As EE was falling, EE stuck arm out to brace fall causing EE to strike right forearm against a rib bolt plate causing a laceration. EE received 6 sutures to close wound and received a full release to return to work on EE's next scheduled shift on 5/28/2024.
The employee was climbing a ladder to begin welding. The ladder was erected on a sloped surface with uneven legs. Once EE reached the 4th step, the ladder fell sideways causing EE to fall to the concrete below. The employee sustained a few abrasions and contusions to lower back and rib area. EE missed zero days of work but is still on physical restrictions.
Employee was exiting the maintenance scoop, and as EE was twisting while exiting, EE felt pain in EE's left knee. Based on the MRI Results received by the operator on 12/13/2023, the employee has a torn meniscus and is off work. This is being claimed as Lost Workdays.
Employee was calibrating CO Detectors at the # 1 Section tailpiece, when they stood on the tailpiece to calibrate the CO, they slipped and grabbed a belt chain, jolting their left shoulder. Based on the MRI results received by the operator on 10/11/2023 this is now being claimed as a Lost Workdays Incident.
Employee was walking down a steep grade on the section, to change a broken torque shaft carrying a tool bag on EE's left shoulder and a new torque shaft on EE's right shoulder when EE stepped in a hole in the mine floor causing two fractures of EE's left foot. A closed fracture at the distal fibula and a closed fracture on the cuboid bone. This is a lost workdays incident.
WALKING DOWN STEPS TO OPEN GATE FOR VISITOR AND NOTICED AFTERWARDS ANKLE BEGAN TO HURT.
Employee was in the process of changing an O-ring in the tilt jack of a D8R Dozer, EE had gathered tools and was climbing between the blade and the machine when EE slipped which twisted EE's left knee. On 12/1/2021 the employee received the results of an MRI which revealed a tear of the PCL. No surgery is required. There were NO Lost or Restricted Work Days.
Injured employee was moving C/M cable out of the roadway when a rock broke loose from the mine roof. As EE turned to run away, employee felt pain in the right knee. The operator received the results of an MRI on 11/8/2021 which reported a fibular ligament tear. This is now being reported as a Lost Time Incident.
Employee was walking through parking lot when EE slipped on ice and fell on EE's left side. Operator was notified on 6/8/2021 that the employee had a partial tear of EE's left shoulder. Employee began missing work on 6/10/2021 for corrective surgery of this injury.
Employee fell approximately 3' from a 4' ladder and landed on EE's left side. Employee had a can of starter fluid in their jacket pocket. Employee sought medical treatment on January 13th, 2020 and was diagnosed with a fractured rib and which time this became a reportable incident. No lost workdays.
FOOT CAUGHT ON CENTER BLOCK AND FELL ON KNEES STOMACH AND CHEST KNEES AND ELBOWS SCRAPPED AND BACK HURTING
Employee was exiting the mantrip after arriving to the slope bottom when EE placed EE's left foot on the barrel connected the throw arm of the switch causing EE's ankle to roll which resulted in a fracture of the distal end of the left tibia. This incident is being reported as a Lost Time incident.
Employee slipped and fell in an area with a slight incline which resulted in a dislocated right elbow. Employee received medical treatment and returned to work on EE's next scheduled shift without restrictions.
Employee slipped on a patch of ice while walking to a company vehicle which resulted in a sprain/strain injury to the left knee. On 5/22/2019 the employee was placed off work due to a surgery that was performed at which time this became a Lost Time Incident.
Stepped out of guard shack and tripped over a rock and hurt hand and side
The employee was walking behind a moving scoop when the scoop batteries struck the top of a broken area of track rail and caused the rail to raise and shift onto the employees left foot. This caused the employee to fall and twist ankle. The employee was diagnosed with a fractured metatarsal and a dislocated metatarsophalangeal joint (big toe).
The employee was standing on the bottom belt at the top of the slope trying to feed the new slope belt through a set of roller assemblies used to pull the belt up the slope. A clamp slipped on the belt causing the employee to fall and strike sternum against a piece of metal. The employee went to the hospital and was diagnosed with a fractured sternum.
The employee stated was going down the outside steps at the plate and frame building when foot slipped causing the employee to fall on posterior and sliding down the other 3 remaining steps.
The employee stated ee was stepping up into a lizard truck when ee said ee right knee gave out. Employee sought medical attention on 4/24/2018 and was taken off work. This incident is being reported at Lost Time Accident.
The employee was in the process of pulling down Highline cable down the track to hang when the employee tripped over on the track rail. On 11/10/2017 employee sought medical attention and diagnosed with an effusion of the left knee. The employee was placed on restricted duty. This incident is being reported as a restricted duty.
Employee reported fall on 10/30/17. Stated stood up to walk out and got foot hung in chair wheels. States as falling hard hat fell off and employee landed on back and head bounced on floor. Employee was terminated on 10/30/17 but not due to occurrence was caught sleeping on 10/28 and 10/29/17
The employee stated ee was hanging tracking cable and stepped on some uneven bottom and twisted left knee. Employee finished shift and the next day went to med express and was diagnosed with a strained knee and was placed on restricted duty. The incident is being reported as a restricted duty.
Employee stated was working outby the section dropping cable when employee stepped in a hole and felt back shift. Size of the hole was 8" deep. Employee was placed on restricted duty. On 9-22-17 employee was taken off work by the treating doctor.

Employee sought medical attention and it was determined that EE dislocated EE's right shoulder. Employee was placed of restricted duty. This incident is being reported as restricted duty.
Employee stated EE was in the process of retrieving highline on the # 1 Section when the incident occurred. Employee stated EE's foot slipped of a 3 inch ledge and slid under the scoop bucket, in that process the scoop bucket smashed EE's left foot. Employee sought medical attention and was placed on 1 week restricted duty. This incident is being reported as restricted duty
SECURITY GUARD WAS WALKING OUT OF GUARD SHACK LOCATED AT BOTTOM OF HILL AND EE STEPPED THROUGH THE DOOR THRESHOLD TO THE PORCH AND TWISTED ANKLE.
Employee stated EE was walking intake escape way on 2 West Panel, when EE stepped in a hole and twisted left ankle. Employee later sought medical attention and was diagnosed with a sprain and was prescribed crutches and a air splint if needed. Employee return to work on the next scheduled shift. This incident is being restricted duty.
On 08/22/2016 Employee stated EE was in the process of fire bossing when EE was crossing a over cast. In that process, employee went to walk down the steps and slip and twisted EE's left knee. Employee finished EE's shift. On 10/15/2016 Employee missed a day of work due to EE's knee hurting. This incident is being reported as a Lost Time Incident due to the employee missing.
The employee stated that EE was exiting # 126 scoop charger to put it on charge. As EE exited the scoop EE felt a sharp pain in lower back. The employee later sought medical attention on 08/01/206 and was diagnosed with a lumbar strain and was taken off work. This incident is being reported as a lost time injury.
The employee stated that EE was working on a broken belt and as EE crossed the #2 section tailpiece EE stepped down and twisted EE's ankle. EE sought medical attention and was prescribed crutches and walking boot if needed, but no restrictions. EE returned to work the next shift at full capacity. This is being reported as a medical due to the prescription.
On 01/28/2016 employee stated was carrying a bag of rock dust on right shoulder, the employee was walking and stepped into a rut causing EE to twist right knee. On 10/19/2016 employee had to have surgery on right knee to repair a torn meniscus. This incident didn't become reportable until 10/19/2016. This incident is being report Lost Time Incident.
