Showing all 3Employee had cleaned VSI deck off, when employee started down stairs carrying a water hose employee slipped and fell. During the fall the valve on the hose contacted the employee on the right side of face.
EE was walking out of primary feed hopper, at the top of the hopper while exiting the employee fell back ward. During this the employee hurt right knee. It is not know if EE struck an object or twisted knee while falling back. The employee fell back approximately 6 feet. The hopper has raw feed material in it and could have been the cause for slipping and falling.
