EE was loading box trailer with pallet sand. While backing out of trailer truck driver started driving off before load was complete. EE tried to stop but was not able to stop in time causing the forklift to fall out of trailer (remaining upright).

Miner reported to supervisor miner was not feeling well, said they felt dizzy and sick to their stomach. Supervisor had a coworker go check on them, the miner was found passed out and unresponsive. EMS was called immediately, miner regained consciousness quickly and was given water and kept cool until EMS arrived. The miner was treated by EMS for heat exhaustion.

EE was going to 1oo mesh load out scales. When EE stepped out of the truck, EE stepped into a wash out from rain and twisted/sprained left ankle.

EE was picking up a 4 chain hoist that goes on forklift used to lift equipment. When EE picked it up EE felt a sharp pain and burning sensation in the middle of stomach.

Employee reported incident and did not want medical treatment at that time. Employee returned on 10/31/2019 asking for medical treatment. Employee was greasing the skid steer at the fueling conex, while walking between the skid steer and the conex EE struck head on a stop sign. Employee was wearing hard hat.
Employee was working on welding a paddle tree for a scrubber. While welding ee had to roll the tree at some point during rolling it ee injured right elbow. The employee did not know ee injured elbow until later that night.

Employee was opening and closing lids on trucks at load out. The catwalk used for access to lids hung up on a safety rail on the trailer. When the employee tried to pull the catwalk free the employee injured left elbow. Employee stated felt a sharp pain in left arm at the elbow.

Employee was digging in ultra fine pile with 980 loader ee got to much dirt in bucket which caused rear of equipment to raise up, When rear raised employee tried to quickly lower bucket which caused equipment to bounce hard. When equipment bounced employee was also bounced around. Employee did have seat belt on.
Employee had cleaned VSI deck off, when employee started down stairs carrying a water hose employee slipped and fell. During the fall the valve on the hose contacted the employee on the right side of face.

EE was walking out of primary feed hopper, at the top of the hopper while exiting the employee fell back ward. During this the employee hurt right knee. It is not know if EE struck an object or twisted knee while falling back. The employee fell back approximately 6 feet. The hopper has raw feed material in it and could have been the cause for slipping and falling.
EE was operating a Genie 125 Boom when for an unknown reason EE made contact with overhead electrical power lines.
