Employee was bolting top and dropped wrench. EE stepped down off of the platform to get the wrench and slipped on the muddy bottom. EE's knee went one way and EE's body went the other.

Showing all 7Employee was in the process of doing pre-shift examination on the off side of #12 belt drive. As employee was walking around the drive, employee stepped on something in the mud, twisted left foot, ankle and knee
EE climbed the ladder to get back in position to operate the loader. As EE was climbing, EE missed a step & fell forward which caused EE to reach out to the hand rail to catch self. When EE grabbed the handrail EE had pain in left shoulder & the pain has continued in left shoulder since this incident happened.
Employee was retrieving CO cable from 2-A belt head for installation on the 2 section belt move. EE placed right foot on the fresh water pipe to gain height to reach the cable. EE's foot slipped and caused right knee to buckle. EE sought medical attention after the shift and was placed on restricted duty.
Employee was walking toward the section and slipped in a Scoop rut twisting right ankle. The mine floor was wet and muddy at the time of the accident. Employee returned to work the next day (6-1-18) with no restrictions. Employee returned to the Doctor on 6-8-18 and was put on restricted duty for 2 weeks beginning on 6-9-18.
Employee was washing down the 3rd floor of the prep plant with a wash down hose. Employee stepped on a slick spot causing employee to strain right knee. ***Employee did not start missing work until 11/15/2017. ***The WC Claim was deemed compensable on 11/30/2017.
Employee stated employee was walking in #4 entry Inby the feeder line and employee rolled employee's ankle causing employee to fall onto the mine floor.
