Employee stated EE had unhooked a chain from the MRS on a LoBoy. When EE attempted to step off the LoBoy, EE's foot hit the end of the ramp causing EE to twist EE's left knee. Employee had surgery on 1/21/26 and started PT on 1/22/26. No lost days.
Employee stated as they were exiting the operator's deck. They stepped down onto the mine floor that was slightly uneven and damp to muddy and their foot slipped a little twisting right knee. They continued to work until 11/20/2025.
While walking from AC10 towards power center to put breaker in, stepped in a hole in the travel way twisting left ankle.
Employee was setting bits, turned to go to back of continuous miner and stepped in a hole. EE twisted EE's left knee and fell to the ground.
Employee was tramming roof bolter from the #2 heading to out by the LOCC. EE was walking backwards and holding drill cable slack pulling it back as EE walked and trammed the drill. EE stated EE stumbled over loose gob and catching EE's balance EE stepped on a ledge on corner of the rib at the intersection twisting EE's right knee.
Employee was getting on forklift to check diesel fuel level when EE's foot slipped out from under EE causing EE to fall and twist right knee.
Employee was moving curtain from one rib to another when employee tripped and fell back on right wrist. This resulted in a fracture.
Employee was walking through thick mud when EE's left knee popped and started hurting.
Employee stated EE went to the mantrip and was getting miner bits off the front of the mantrip. EE stated EE reached and got two buckets of miner bits and twisted to go back to the stinger. EE tripped over the waterline and caught self and EE's lower back began to hurt on the left side. Lost time as of 9/3/24.
Employee was carrying 2 bundles of rope bolt plates from scoop to load on roof bolter. EE tripped on power supply cable for roof bolter causing EE to fall into the walkway of the roof bolter. The plates EE was carrying caught EE's middle finger on left hand between plates and floor of walkway. This resulted in a comminuted displaced fracture to EE's middle finger.
Employee was drilling a hole and broke a piece of steel. When EE stood up to get steel out of head, EE slipped on a rock falling into steel with EE's right arm. This resulted in 6 sutures.
EE was helping miner move when EE saw a loose piece of rock. When EE went to scale it down, EE tripped over some loose coal & rock lying on the ground hurting EE's left knee. EE was given a knee brace. EE continued to work until 11/9/23 when EE had surgery.
Employee was walking and tripped on a piece of pallet causing them to fall hurting their right shoulder. Employee continued to work until 1/20/23 when they were taken off work by a doctor.
Technician was working block time for customer, and was installing transmission in a 993k loader. Technician was exiting transmission area on the loader when EE's left foot slipped and caused EE's body weight to transfer to left arm. Tech felt pain in the bicep.
Employee was working on the miner, stepped off of the rub rail stepping on a rock or uneven bottom causing them to twist their right ankle resulting in a sprain.
Employee was loading top roller on trailer and stepped in a hole rolling EE's ankle causing EE to fall. Became LT on 7/10/20.
Employee stated was replacing tailpiece skirts and was positioned on both knees for a period of time. When EE turned EE felt a pop in left knee. EE stated has a preexisting condition with this knee from an injury at home and has been seeing a Physician for that injury.
Employee was crossing belt. When stepping down EE turned EE's right ankle.
Employee alleges was attempting to hang a drill cable and stepped into a hole and hurt right knee. Upon further investigation and interviews of the crew on the active section, no deformation in the mine floor could be found in the intersection the alleged event occurred in.
Employee was stepping across the man trip lift when EE's left foot caught the edge of the lift causing EE to fall into the shelving along the wall. As EE was trying to catch self, a nail sticking out of the shelf punctured EE's right hand.
Employee stepped up onto concrete incline at belt take up on stacker belt to grab hold of a piece of belt when one end flipped up causing employee to slip and lose balance. When employee lost balance and fell employee twisted around resulting in a strain to lower left back area.
Employee was walking behind parts trailer when a non-venomous snake bit EE on EE's right leg above EE's boot top. After being startled by the snake, EE sprained EE's right ankle. EE became lost time on 5/15/18 due to the ankle sprain.
EE was coming down in bucket to get equipment, leveling cable broke and bucket went forward and dumped EE out. EE has 4 broken ribs, broke collarbone, broke EE's glasses and laceration near right eye.
Employee was pulling a miner cable pull rope to attach it to the tail boom of the miner and fell after becoming entangled in the miner water line. Employee fell backwards on to left buttocks. Employee is complaining of pain near tail bone radiating down left leg. Employee continued to work until 6/21/17.
Employee was loading high voltage cable. The employee tripped over the cable causing them to fall landing on the left arm resulting in a fracture of the left radius.
Employee started to step down from a ladder The ladder shifted and pulled rivets from the inside of the ladder that did not appear to have been previous damage. The employee fell from the second step down to the mine floor and injured the right ankle. Employee worked the remainder of the shift with no issues but was later taken off work due to swelling.
