Received a CWP diagnosis on 5/21/2018 for EE. EE last worked on 2/21/2017.

Struck by flying object

Employee was assisting in maintenance when EE was struck by a small piece of metal that was cast from a hammer strike by another employee. Employee was approx. 20' away from where the other employee was using the hammer. The small piece of metal was removed and the employee received two stitches.

Fall from machine

EE was coming down in bucket to get equipment, leveling cable broke and bucket went forward and dumped EE out. EE has 4 broken ribs, broke collarbone, broke EE's glasses and laceration near right eye.

Struck by flying object

An employee was striking a chisel with a 3 pound hammer to spread top of roller stand open after taking bolts out. Injured was standing about 10' away from stand when a piece broke off the stand hitting injured on left arm resulting in a puncture hole in arm requiring stitches. Injured was supervising the work being done.

Employee was using a 1"" air impact wrench to remove bolts from a cutting edge lip protector on a 993K CAT loader. EE was removing a bolt and it began to rotate. EE pushed up on the bolt and bumped the rotation of the air impact hoping the bolt would lock back into position but instead the bolt spun causing a laceration to EE's left index finger. This resulted in 17 stitches.
