EE accessed elevator platform using a manlift in order to perform task. Ladder access point door was left open to accommodate ladder access to platform. EE did not close ladder access door before proceeding. EE stepped into the open ladder access door, falling to below platform.

Showing all 3EE was helping another EE clean a material screen on a chute. The EE's did not have harnesses on when at a height of approx. 5 feet. The EE stuck EE's feet between the ladder and the structure, when EE went to replace EE's feet back on the ladder, EE lost balance. EE then fell approx. 4 feet and cushioned EE's blow with EE's right wrist, causing a fracture in hand.
EE carried a bag of flocculent over to mixing tank to fill storage container. EE had a rip in the bag, causing flocculent to fall and react to condensation and water on the ground. EE proceeded to turn around and walk back making it 50 feet before slipping on flocculent that had been dropped. EE felt it was best to go to the hospital due to pain in the arm and get it checked out.
