EE accessed elevator platform using a manlift in order to perform task. Ladder access point door was left open to accommodate ladder access to platform. EE did not close ladder access door before proceeding. EE stepped into the open ladder access door, falling to below platform.

EE failed to follow company Lock Out Tag Out procedure/policy. EE attempted to place a chain back on a sprocket while the motor side sprocket was energized and in motion, EE caught the right index finger tip in between chain and sprocket.

EE was found unresponsive in portable lavatory; County EMS was activated. Paramedics treated EE. EE regained consciousness and refused further treatment. EE was driven home by a non-EE friend. EE has been terminated.

EE was helping another EE clean a material screen on a chute. The EE's did not have harnesses on when at a height of approx. 5 feet. The EE stuck EE's feet between the ladder and the structure, when EE went to replace EE's feet back on the ladder, EE lost balance. EE then fell approx. 4 feet and cushioned EE's blow with EE's right wrist, causing a fracture in hand.

Miner was removing tail pulley guard on a locked out/de-energized conveyor. Miner leaned guard (20 lbs.) on frame of the conveyors tail instead of removing and placing the guard 2' down on the ground. Guard slid cutting miner's right index finger. Wound was treated but the miner was later taken to the hospital to better treat the wound.
Employee was attempting to take off a 3" rubber expansion joint. While doing so the employee decided to kick down on the pipe to remove it. The joint fell off and the employee bumped their knee. The employee did not report discomfort until half way through the next shift before leaving early.
EE carried a bag of flocculent over to mixing tank to fill storage container. EE had a rip in the bag, causing flocculent to fall and react to condensation and water on the ground. EE proceeded to turn around and walk back making it 50 feet before slipping on flocculent that had been dropped. EE felt it was best to go to the hospital due to pain in the arm and get it checked out.
