The employee was inside the onsite job trailer retrieving tools when EE tripped and fell forward. Trying to catch EE's fall, EE caught EE's hand on an old plastic bucket that shattered with EE's weight and force, resulting in a laceration of the right hand requiring several stitches. EE was released to full duty the same day.

EE was stepping onto a metal ladder when their foot slid on the rung from the left to the right side. Employee turned in a report of the incident, but it did not start to hurt until a few days later. The EE worked for several weeks thinking the pain would go away. When it did not EE was taken to the doctor where EE was diagnosed with a strained knee.
An employee was putting a 4" pipe into a warman pump tank when EE's feet slipped causing EE to fall into the tank at which time EE's left hand was smashed between the pipe and the tank.
When EE was stepping down from a LHD right foot got hung up in the step causing ee to step down awkwardly onto left leg. EE felt something pop in the calf of left leg.
EE injured right knee while exiting a RTV, Injury was not reported until 3/29/2016 and deemed work related by physician on 8/11/2016. EE had surgery to repair.
Employee had just left car and was walking across the parking lot when ee slipped and fell causing ee to strain left knee.

Employee missed the step while walking up the stairs into the offices. Employee injured shoulder but did not seek medical treatment until 2/4/2013. EE was placed on restricted duty.
The employee was pulling on a jackleg drill to bring the steel out of the hole when he slipped and strained his back.
Employee drove CAT 950H loader to staging area at the Pond 4 site. He parked the machine at the staging area. He picked up the wheel chocks from the storage area on the loader and threw them to the ground. He then proceeded to begin climbing down the ladder not maintaining the required 3 points of contact. Employee fell from the ladder and landed on wheel chocks.
AFTER EMPLOYEE PULLED A CORE TUBE OUT FROM THE DRILL, HE WAS WALKING OVER TO UNLOAD THE CORE FROM THE TUBE AT THE TUBE RACK AND HE TRIPPED ON THE HELPER'S DECK CAUSING HIM TO FALL WITH THE TUBE IN HIS HAND. HIS HAND WAS CAUGHT BETWEEN THE TUBE RACK AND THE TUBE ITSELF.
When entering the thickener room the employee went to step up onto a landing and slipped causing him to roll his ankle.
Worker returned to work 2 weeks ago after 6 months off due to serious health issues. He was walking across the yard and fell over and made no attempt to break his fall.His face struck the ground. He was unable to assist himself to get up.He does not remember the event. It appears he blacked out and is currently under a physicians care to determine the cause of the event.
EE stepped off a loader and sprained his ankle.
EE stepped over a rock along the new concrete foundation. When his foot came back down a 2X4 brace was left in the ground under some loose gravel, and caused him to loose his footing. EE used his left hand to brace the impact causing his wrist to be sore by the end of the shift.
EE reported injuring his knee on 1/19/10. This was a incident and received no medical treatment at that time. He went to the Dr. on 6/16/2010 because it was bothering him and at that point placed on restricted duty.
The mechanic stepped onto several wooden blocks to reach a water valve. The blocks rolled and the mechanic fell and twisted his knee. A MRI of the mechanics knee showed a tear of a ligament in his knee and he was placed on restricted duty.

A miner stepped off a tractor onto a rock and rolled his right ankle. The ankle was severely sprained with a possible fracture. The doctor placed the miner on light duty.
The miner was in the process of finishing drilling a hole in the back. As he pulled the drill steel out of the hole, it hung up. The miner pulled on the jackleg, lost his balance and fell, striking his right knee on a rock. The impact caused his knee to immediately swell up.
The employee was climbing down from greasing a fan and felt something pop in his hip area. The employee strained a groin muscle and was placed on restricted duty.
Miner was walking down a muckpile and tripped. Cutting his leg on the broken rock. The cut required several stitches.
On 11/16/07, a miner fell 4' from a 6' ladder and landed on a piece of steel ventline. At the time he received bruises on his ribs and did not request or require doctor's care. As time passed his ribs continued to hurt and did not seem to be healing so he requested doctor's care on 11/28/07.
EMPLOYEE WAS BARRING ON A SLUSHER BUCKET TO MOVE IT. THE BAR SLIPPED AND WHEN IT DID, HE ALSO SLIPPED AND PULLED A MUSCLE IN HIS BACK.

Employee was attempting to move a 42" vent bag over to the side, as it was in the way for hanging grade chains. He was attempting this while standing in the bucket of a 2-yd loader. He lost his footing & fell to the ground. The fall caused a fracture of his left leg & a crack just above his right wrist.
Employee was sampling the face of a slusher stope. He slipped on some wet rock and lacerated his finger when he tried to brace himself by putting his hand against the rib wall. The wound received sutures.
EE was walking around a loader at the start of shift and slipped and strained his back.
EE was installing a light over a pan feeder of new conveyer system. He twisted his right knee. This was originally considered first aid. After follow-up visit, it was determined that an MRI was needed. A small tear of the meniscus was detected and will possibly need arthoscopic surgery/ restricted duty.
EMP WAS HANGING VENT LINE FROM A 2 YD LOADER AS WAS HIS PARTNER ON THE OTHER SIDE OF STOPR. PARTNER HAD KINK IN VENT LINE & ALL AIR RUSHED TO THE OTHER SIDE AND VENT LINE KNOCKED HIM OFF LOADE R TO THE SILL. HE LANDED ON THE LEFT SIDE OF HIS BACK & FRACTURED 2 RIBS.
EE WAS HANGING CHAIN LINK FENCING IN THE STOPE. HE SLIPPED & TWISTED HIS KNEE. HE WORKED FOR ANOTHER 1 1/2 HR. & IT WAS BOTHERING HIM, SO WE TOOK HIM TO THE ER. THIS RESULTED IN RESTRICTED DUT Y.
EMP WAS HELPING CHANGE OUT A SIMONS CRUSHER. HE SLIPPED ON SOME GREASE & AS HE TRIED TO BREAK HIS FALL, HE HIT HIS LEFT HAND ON A STORAGE LOCKER & FRACTURED A BONE IN HIS LEFT HAND.
EE WAS HANGING VENT BAG FROM TOP OF 1 YD LHD. HE SLIPPED AND FELL LACERATING HIS LEFT FOREARM. THE LACERATION REQUIRED SUTURES.
EMP WAS STEAM CLEANING ENGINE OFF OF 966 F.E. LOADER. HE SLIPPED ON THE TIRE AND FELL, SPRAINING HIS LEFT ANKLE. THIS RESULTED IN RESTRICTED DUTY FOR 1 WEEK.
EE WAS CLIMBING UP LADDER TO CHECK HEATER IN CONCENTRATE LOADOUT BAY. THE LADDER SLIPPED OUT FROM UNDER HIM & HE FELL LANDING ON THE HEEL OF HIS LEFT FOOT, FRACTURING THE HEEL.
EE WAS WALKING TO THE DRY ROOM FROM THE COLLAR AND STOPPED TO PUT HIS LUNCH BOX AND THERMOS IN HIS CAR. HE SLIPPED ON A PATCH OF ICE, FELL AND LACERATED HIS RIGHT ELBOW. REQUIRING SUTURES.
MINER WAS EXITING MAN CARRIER AND SLIPPED AND FELL, HITTING HIS KNEE, CAUSING A FRACTURE TO THE KNEE CAP.
INJURED HIT BACK WHILE GETTING OUT OF A MAN CARRIER.
HE LAID JACK LEG DRILL AGAINST THE RIB TO CHANGETHE WATER NEEDLE AND TRIED TO PICK IT UP. THE DRILL HUNG IN THE FENCE USED FOR GROUND SUPPORT AND AS HE JERKED THE MACHINE TO FREE IT, HE FELL B ACKWARDS LANDING ON HIS TAIL BONE.
HE LOST HIS FOOTING AS HE WAS LOADING SUPPLIES INTO AN LHD BUCKET AND FELL. HE CUT HIS LITTLE FINGER LEFT HAND ON ROCK BOLT MATS.
