EE was servicing a cat 775 E haulage truck, EE was removing the left rear final drive oil plug, the employee slipped and his face struck the tire when the oil plug broke free. EE sustained a broken tooth when his jaw struck the truck tire.

EE LOST FOOTING ON CONVEYOR CATWALK. HIS LEFT HAND WAS HOLDING HANDRAIL AND RIGHT HAND HOLDING GREASE GUN. WHILE TRYING TO RESTRICT FALL, THE FORCE OF EE'S WEIGHT OVERPOWERED HIS LEFT ARM STRENGTH. EE'S LEFT BICEP WAS RUPTURED AND REQUIRED SURGERY TO REPAIR.

THE EUCLID 74TD 35 TON HAUL TRUCK WAS BLOCKED, LOCKED AND TAGGED OUT FOR REPAIR. A WIRING FAULT WAS BEING TRACED AND ACCESS INTO AND UNDER THE CAB WAS REQUIRED. EE WAS INSPECTING THE WIRING UN DER THE CAB WHILE STANDING ON THE EXTENSION LADDER, HIS FOOR SLIPPED FROM THE LADDER RUNG. HE FELL APPROX 4 FEET TO GROUND LEVEL. HIS INJURIES CONSISTED OF BRUISES TO THE LEFT LEG, THIGH & HI

WHILE WALKING TO THE BREAK ROOM, EE TRIPPED ON A PLASTIC TRASH BAG AND FELL TO THE GROUND. HE SUSTAINED LACERATIONS AND ABRASIONS TO HIS RIGHT WRIST AND ELBOW. A WALKWAY GRATING USED FOR BOOT SCRAPING WAS PRESENT IN THE FALL AREA AND CONTRIBUTED TO HIS INJURIES. FIRST AID WAS ADMINISTERED AND EE WAS TRANSPORTED TO CLINIC WHERE HE REC'D SUTURES IN HIS WRIST & ELBOW, XRAYS, AND SHOT.

EE WAS OPERATING A WATER TRUCK. HE WAS THEN TOLD TO HAUL OVERBURDEN MATERIAL. AT APPROX 11:45 A.M. HE WAS TOLD TO OPERATE THE WATER TRUCK THROUGH THE DINNER PERIOD & REMAINDER OF THE DAY. HE D ROVE HIS PERSONAL VEHICLE, A JEEP WRANGLER, TO THE WATER TRUCK. AS HE WAS GETTING OUT OF JEEP HE FELL STRIKING HIS HEAD ON THE GROUND. HE WAS UNCONSCIOUS BREIF PERIOD AND TAKEN TO HOSPITAL.

A TEAMSTER WAS DELIVERING DRINKING WATER TO VARIOUS WORK AR EAS AT THE MINE. WHILE CARRYING A 5 GAL. KEG TO HIS PICK-UP TRUCK, HE STEPPED OFF THE EDGE OF A CONCRETE PAD. THIS MIS-STEP CAUSED HIM TO TWIST HIS LEFT KNEE. SURGERY SCHEDULED FOR 12/14/00.
