Employee slipped and fell while stepping over hose. Employee received restrictions for this injury on 02/16/2026.
Employee was cleaning out the bottom level of the feedend and removing unused metal from feedend area. As EE was dragging the piece of metal out, EE tripped over another piece of guarding EE had previously moved into a pile to dispose of. EE fell onto a piece of angle iron on the pile and cut the back of EE's knee.
An employee slipped and fell while descending the wooden stairs near the office trailers, landing on their arm and injuring their shoulder. During the fall, their foot slipped forward, causing them to land backward on their arm near the elbow, resulting in pain extending to the shoulder. A medical evaluation confirmed a small fracture in the left shoulder.
Employee walked down staircase, stepped in uneven surface rolling left ankle.
Employee was descending the MROT storeroom stairs carrying boxes. Once EE got to the bottom of the staircase, started walking to leave storeroom, and tripped over pallet that was in walkway causing EE to fall on EE's arms/shoulder. Employee had an MRI that showed a broken bone in the shoulder as well as a torn labrum after follow-up with orthopedic on 4/17/2025.
Scale Attendant fell on ice going to car. Employee has been performing normal job duties but has had consistent discomfort. Employee became reportable on 3/14/2025 after a follow up doctor's visit indicated a nondisplaced fracture of the left tibial spine requiring restricted duty.
Employee was stepping off of a ladder from building to go to rail when EE felt a sharp pain in the left knee. The employee received restrictions during the doctor visit on 9/9/2024 that will not allow EE to perform their job duties.
Employee was performing the tasks of lime loader helper, moving and loading railcars. Employee believes that EE hit EE's knee after tripping and landing on the knee while entering/exiting the top of a railcar. The employee stated that EE didn't report the incident initially, believing that there was no injury of concern. However, it appears the injury may have become infected.
Employee was walking to exit the preheater. As EE was going to exit and started to adjust to get out, EE's foot slipped off the top of the horizontal dome cooling pipe EE was standing on and started to fall. Employee stuck out EE's arms to try to stop falling. Employee noticed afterwards that EE's right shoulder was hurting. MLC was notified 10/28/2024 of medical care.
Employee stood on chair to reach smoke detector, overextended and fell hitting head against door hinge. Employee cut ear requiring stitches and was returned to full duty.
The employee stated, was cleaning an elevated walkway along a beltline and stepped off the walkway onto the ground. Fell approximately 4-5 feet and injured EE's lower back and pelvis. The employee was taken to a local hospital. The employee is currently off work pending evaluation from an orthopedic specialist which was found there was a fracture 1-20-2023.
The employee was operating a fork truck, emptying coal dumpsters into the waste pile next to the Glass Stone Plant. As the employee stepped out of the fork truck to dump the dumpster, EE twisted knee. After twisting their knee, the employee realized that the ground was uneven due to frozen material. The employee was taken to Occ Med and received an x-ray.
Employee stated, Employee was unloading a tire from the service truck with one foot in the bed and stepped on the bumper with the other. EE had stepped on an impact gun on the bumper and lost EE's balance. EE then fell to the ground.
EMPLOYEE STATED EE went to the bulk bagger to check on orders. While there a piece of pallet that someone had leaned up against the shelf had fallen over onto the bagger. EE turned to walk away and tripped over the wood and fell on EE's knee and hand. WITH DOCTOR EVALUATION, FOUND THAT WRIST IS BROKEN.
Employee tripped on a mat in the lab prep room and received a cut requiring stitches when EE's head struck the corner of a metal grinder cabinet.
Walking down an incline, slipped and caught self with arm, tearing rotator cuff.
Employee stated EE was working on cleaning the windshield of a mechanical scaling machine and slipped on the tracks. The employee slipped from atop the tracks striking both hands on the tracks. The employee suffered a laceration to one of EE's hands and stitches were used to close the wound.
During installation of Nash blower, employee was working off a ladder removing 16 inch blind on CO2 line when they aggravated their back. Employee returned to break area and notified supervisor about what happened. Safety was contacted and employee was taken to Occupation Medicine for screening and observation.
Employee contacted safety and stated that EE's knee was acting up. This is the same knee that was previously injured. Treating physician contacted and a follow up was scheduled.
Employee was carrying boxes down the stairs and stepped too far forward and slipped and fell
Employee slipped on stairs and twisted left knee. Reported to supervisor on 2/15/2020 and not seen until 2/17/2020. Was given full release by Occ Med Dr. but increased difficulty in movement persisted. We were no longer able to accommodate and became lost time on 2/24/2020
Employee fell while cleaning the top of a rail car. Fell into car and fall protection was worn and used. Employee was rescued and seen by the doctor and was returned to work full duty. Later appt on 2/17/2020 it was determined EE would need surgery and was given modified duty and recommended off.
EMPLOYEE STATED THAT THEY WERE SHOVELING OFF EXCESS BUILD UP OF MATERIAL ON TOP OF WEIGH BIN, WHEN EMPLOYEE SLIPPED IN MATERIAL CAUSING EE TO FALL AND LAND WRONG ON RIGHT ANKLE. EMPLOYEE WAS TAKEN TO ER 12-2. EMPLOYEE WAS SEEN BY A SPECIALIST ON 12-4-2019. GIVEN TREATMENT PLAN OF CAM WALKER. EMPLOYEE BEGAN MISSING WORK 12-5-2019 FOR LOST TIME. EMPLOYEE HAS FOLLOW UP 12-18-2019
EMPLOYEE STATED THAT THEY TRIPPED ON A PIECE OF ANGLE IRON. THEY AFFIRMED AS THEY WENT TO CATCH THEMSELVES THEY LANDED WRONG ON THEIR LEFT LEG CAUSING PAIN IN THE KNEE. EMPLOYEE WAS TAKEN TO A LOCAL ER WHERE EE WAS EVALUATED. EMPLOYEE HAD RESTRICTIONS PLACED UPON EE THAT REQUIRED MODIFIED DUTY. THE EMPLOYER IS ABLE TO ACCOMMODATE THESE RESTRICTIONS.
Walking at the water treatment plant when EE slipped and fell against railing onto right shoulder. Put on limited duty but returned to work.
Employee climbed the ladder to enter the cab of the crane EE was operating. When EE attempted to step into the cab, EE misstepped and fell to the ground breaking right arm
While reducing a rail car, the bar slipped causing employee to lose balance and fall breaking Right Ankle
Employee stated Employee was working at the HR64 plant by the rail blow off point on #2 track. EE was climbing down a 5' stationary ladder to a concrete platform. EE opened a safety gate and walked through and closed the gate. Lost grip and fell about 5' against a railing on the other side of the platform.
EMPLOYEE STATED THAT THEY HAD TRIPPED & FELL AND LANDED ON THEIR RIGHT SHOULDER. EMPLOYEE WAS TAKEN TO OCCMED & MRI SHOWN A TEAR IN THE RIGHT SHOULDER. WITH MRI RESULTS, IT SHOWN THAT THERE WAS A PRIOR TEAR IN THE SHOULDER. SURGERY WAS ON 9-18-2018. ON 9-28-2018, THE DOCTOR STATED THAT DR DETERMINED THAT THE FALL WAS THE PREVAILING FACTOR FOR THE ROTATOR CUFF TEAR.
EMPLOYEE STATED WAS ATTEMPTING TO CLEAN A SCREEN AND WHEN REMOVING THE WEIGHT OFF THE SCREEN EMPLOYEE'S FOOT SLIPPED CAUSING EMPLOYEE TO LOSE BALANCE CAUSING PAIN IN SHOULDERS AND PAIN IN THE STOMACH AREA. EMPLOYEE WAS DIAGNOSED WITH A HERNIA AND WILL HAVE SURGERY 4-11-2018. BEGAN MISSING WORK ON 4-2-2018.
Injured employee stated that ee fell from the deck of a rough terrain crane when it became unstable while the Operator was moving the boom as it was chained to a transport trailer.
Employee stated that ee was walking, tripped over a railing and twisted left knee.
Employee was cleaning the burn floor of the plant. As employee pulled the air hose to the next area to be cleaned, employee tripped over a 4" vacuum hose that had fallen down into the walkway. Employee fell to the concrete floor striking right shoulder.
Employee was assisting in opening the tarp on a truck to load by standing on the fuel tank. The tank was covered in ice causing the employee to slip and fall to the ground landing on both feet but spraining the right knee upon landing.
Employee stated while climbing down a ladder and slipped causing them to miss the bottom step. Employee further stated they overextended their right knee and twisted left knee. Employee went to their personal doctor for treatment. On 01/18/2017 employee contacted me stating it was work related and requested treatment. Employee is currently missing time.
Employee was dismounting the locomotive and twisted left ankle.
Employee stated while stacking bags of R-2 stone EE was standing on a pallet and a board on the pallet broke causing EE to fall forward. Employee was taken to the clinic and was diagnosed with a right knee strain and given light duty.
Employee stated that while EE was releasing a brake on a railcar with a brake stick EE stepped into a hole, twisting their right knee. EE was taken to the emergency room where EE was given a splint and light duty.
Employee was cleaning on a steep and uneven roof and turned left ankle. Employee did not request medical until 04/26/2016. EE was placed on light duty and given a splint.
While exiting a train, employee slipped and fell on his left knee that required stitches.
Injured employee stepped down from a kiln pier to the walkway and twisted his left ankle.
Employee stated he jumped out of the service truck he was operating and stepped on a rock with his left foot. He felt a pop and fell to the ground. He was later diagnosed with a torn ACL and underwent surgery on 05/30/2014.
Employee stated he was trying to un-choke the RK2 trash screw with another employee. After being unsuccessful in getting it un-choked the other employee went to get a supervisor. The employee suffered a laceration to his head and an abrasion to his right eye. The laceration required 2 staples.
Employee stated he was unchoking a rock reclaim tripper when he slipped and hit his rib cage on a piece of angle iron. He received one fractured rib and was placed on light duty.
Employee stated he slipped on a toe in the cross cut and his right hip popped followed by pain. Employee refused medical treatment at the time. Employee received surgery on 01/14/2014. Safety department later learned the surgery was work related.
Employee stated he was walking on a step and turned feeling a "pop" in his right knee. The injury required surgery and he is currently missing time.
While climbing up the ladder of a front end loader he fell approximately 3-4 feet on his right hand. The hand is fractured and he has been placed on light duty.
Employee stated he slipped on the step of the crane that had diesel fuel on it. He fell to the ground and hit his elbow on the door frame. He miss 2 days from work.
Employee stated he slipped in some wet material at the bottom of the steps and caught himself on a handrail. He was placed on light duty.
Employee stated the he was washing the basin down with water and was leaning up against the hand rail and the rail broke and he fell to the bottom of the basin. The employee suffered a fracture to his pelvis and a contusion to his head. He is currently missing time.
