Showing all 38EE was working on top of a primary sump with an EE wrapping a vertical 18" cpvc pipe with a sealing tape. As EE handed the wrap to EE, EE went to reposition self to receive the wrap and EE's foot slipped thru an opening on the pipe support and fell to EE's left, landing on EE's left forearm. Both EE's were wearing fall protection at time of the incident.
TM fell while descending stairs and while attempting to catch themselves felt a pop in EE's left arm.
TM was exiting car and twisted ankle.
While using a 1 1/2 inch hose to wash down under the tower, TM lost balance and hit EE's left back of head (Above left mastoid) on a cross beam. Additional trauma was inflicted on left eyebrow.
When the team member stepped off the last rung onto the ground the team member planted foot wrong and rolled ankle.
Team member was changing rollers on 4127 conveyor, stepped on uneven ground and rolled right ankle. Originally first aid. EE received medical treatment on 7/1/21.
Team member stepped into a divot causing the team member's knee to jam. Initially this injury was considered first aid. Physical therapy was requested on 9/19/19 making this reportable.
After turning and starting down steps on push frame (blade arm) to dismount dozer, Team Member's foot missed the step causing employee to fall to the ground.
Team Member inadvertently skipped two steps of stairs and fell forward. EE reached the landing feet first but felt EE's knee hyperextend. Team Member immediately felt pain in knee. The injury was classified first aid until 5/2/16 when the doctor requested physical therapy and prescription medication.
Team Member was exiting portable restroom and tripped on a support leg to a light plant. Team Member fell to the ground onto his hands and knees. Did not seek medical treatment until 9/24/15.
Team member injured his left knee when he turned while shoveling in a trench.
Employee reported that while attempting to remove a broken weir board from a sluice box, he slipped and fell down the bank of an empty dredge pond. Injuries were contusions to back and ankle and ankle sprain
Employee was moving loadout flop gate lever with come-along. Lever broke at pivot point. Employee twisted and moved to avoid being struck by lever. Employee fell onto back, hardhat came off and head struck the ground. Also sprained his back.
Team Member was weed wacking and stopped make contact with a passing haul truck. Team Member lost footing on the berm where he was cutting weeds. Slid down slope of berm. Upon stopping at bottom of slope he felt a "pop" and then soreness in right knee.
Employee attempted to get out of his work truck, sliding out to the right when he felt a sharp pain in his lower back as he put weight on his right leg.
EE fell off equipment while performing 125 hour service on aerial lift.
Stepped on uneven ground & twisted his left knee.
Stepped in hole or on rock. Fell to ground. Fracture top of left foot.
reported accident occured in Sept. Requested medical care 11/21/06. Originally 1st Aid no MSHA reportable. Informed by dr on 1/31/07 that injured started physical therapy making injury MSHA reportable.
He missed a step and strained left ankle.
Employee says he strained his lower back when he slipped off lower rung of truck ladder. did not report until 12-21-04 when sought medical treatment. no lost time or restricted work.

FALL/ SLIP FROM LADDER MOVED AND EMPLOYEE STEPPED BACKWARD AND DOWN 27" FROM THE TOP STEP TO THE PAVEMENT, SPRAINING HIS RIGHT KNEE.
WAS STANDING ON A CONCRETE & WOODEN PLATFORM APPROX 4' H LEANED ON A 6FT A FRAME LADDER AGAINST A PIECE OF SWITCHGEAR. WAS ATTEMPTING TO CAULK THE UPPER PORTION OF THE SWITCHGEAR UPON PLACING LEFT FOOT ON THE LADDER TO LAND ON SOFT DIRT, LANDED ON THE GROUND WITH BOTH FEET BUT THE TURNING ACTION CAUSED HIS LEFT FEET TO MOVE OUT OF JOINT
HE REACHED FOR A 3/4 IMPACT GUN, STEPPED FORWARD, SLIPPED AND SPRAINED HIS LEFT ANKLE.
