While walking downstairs, team member slipped exiting the last step which made them fall against the handrail.
EE was working on top of a primary sump with an EE wrapping a vertical 18"" cpvc pipe with a sealing tape. As EE handed the wrap to EE, EE went to reposition self to receive the wrap and EE's foot slipped thru an opening on the pipe support and fell to EE's left, landing on EE's left forearm. Both EE's were wearing fall protection at time of the incident.
TM was verifying side by side heating issue when they were burned by Gator radiator steam.

TM reported, that while shoveling material in the reclaim tunnel their knee started hurting.
Team member injured right hand and elbow while installing radiator hose on D11.
Team member was loading material at the end of the cut when the team member's scraper was struck twice from behind by another CAT 627F scraper resulting in neck and upper back pain. This injury was considered first aid until the team member's first day of treatment on 8/1/23.
While using a 1 1/2 inch hose to wash down under the tower, TM lost balance and hit EE's left back of head (Above left mastoid) on a cross beam. Additional trauma was inflicted on left eyebrow.
Employee was holding an idling chainsaw when the chainsaw caught the outer seam of EE's PPE chaps. The saw kicked up and hit EE's left knee causing a 2.5 inch laceration on the left knee. Immediate first aid was given, then employee went to an urgent care and received stitches.

Team member was washing area with fire hose in between head chutes. The hose ripped and team member moved to take cover and ran into a portion of the conveyor structure.
TM entered loader work zone in pickup and parked without acknowledgement from loader operator. TM was jarred in cab when loader backed into right side of pickup. Originally first aid. TM received medical treatment on 8/11/2022.
A wildfire broke out on lands adjacent to the A.R. Wilson Quarry to the north side of the overburden conveyor system. A laborer in the area called emergency services. The fire and cause are still under investigation by responsible regulatory agencies and Graniterock.
TM was in the process of cleaning up after a job. TM threw used liner over handrail. Handrail gave way causing team member to fall to the ground, approximately 20 feet.

Team member was lifting materials & operating a skid steer to haul junk wheels to a dumpster. Felt pain in back while operating skid steer.
Contractor was cutting an opening in a rubber lined chute when the rubber liner caught fire. Contractor attempted to extinguish the fire with a water hose. The plastic screen media above the chute proceeded to catch fire which then ignited a conveyor belt overhead, damaging the chute, screen, belt and nearby components.
Team member was changing rollers on 4127 conveyor, stepped on uneven ground and rolled right ankle. Originally first aid. EE received medical treatment on 7/1/21.

Team member was shoveling aggregate spillage. When they went to take another scoop they hit something hard, breaking the shovel and injuring their left groin area.
Team member was hammering a tooth into an adapter with a single jack style hammer. The hammer impacting the tooth caused a metal chip to fly up resulting in team member retaining a metal fragment in chin. Injury was originally first aid. Medical treatment was not provided until 8/11/2020.
D-6 Dozer made contact with 740 Water truck during washing activity. Water truck was parked and as dozer moved forward the blade made contact with the front right tire. The contact caused the tire to pop and drop the water truck. The event led to EE seeking medical attention for back pain.

Team member was on hands and knees moving a screen. When the team member moved, felt a sharp pain in lower back.
Team member stepped into a divot causing the team member's knee to jam. Initially this injury was considered first aid. Physical therapy was requested on 9/19/19 making this reportable.
While attempting to manually flip the spindle of 988F Loader with a fellow worker to drain the oil from the brake packs, it slipped off the forklift forks and struck EE's arm.
Team Member reported eye irritation resulting in conjunctivitis from exposure to airborne foreign particulates following the use of a grinder while wearing safety glasses and face shield. Examination by ER physician did not find a foreign object or corneal abrasion in the affected eye.

Removing rear differential fill plug from a 777G. Tried pushing down on breaker bar plug and it didn't move. Felt popping in right shoulder and shooting pain down right arm.
Team Member was cleaning the screen chute by hand. While unloading rock out of 4209 discharge a large rock shifted and hit the back of Team Member's right hand. Originally first aid only. Team Member did not seek medical treatment until 11/26/18.
Team Member was operating loader in the sand yard. Team member turned on the AC and a foreign object flew out of the AC vent and into right eye.
Received confirmation from audiologist on 6/21/18 that team member had hearing loss in right ear due to cumulative trauma while performing work as a Heavy Duty Repairmen/Welder.
Team member was changing a roller with the help of another team member. As the team members turned the frame sideways on its back to change the roller out, the frame fell and caught the team member's thumb bending it over.
Team member was shoveling 4"" rock when team member's right foot slipped and twisted when the rock under foot shifted causing pain in team member's right knee.

After turning and starting down steps on push frame (blade arm) to dismount dozer, Team Member's foot missed the step causing employee to fall to the ground.
Team Member felt pain in right elbow while attempting to start weed eater. This was initially reported as first aid. On November 13, 2017, ee was given prescription medication for the continuing pain in elbow making this injury reportable.
Hearing loss in right ear due to cumulative trauma while performing work as truck driver.
Team Member was loosening coupling bolt when handle on pipe wrench broke resulting in strain in right shoulder.

Team Member picked up several pieces of steel with both hands while wearing protective gloves, but the steel cut through the gloves and lacerated the left thumb.
Team Member was standing near the tail of the 4157 conveyor resting their hand on top of a horizontal steel member. A 4"" rock fell from the 4153 conveyor approximately 12 feet above. The rock struck EE's left hand causing a fracture to the fourth metacarpal of EE's hand.
EE was removing torque converter from engine that was still installed in the 775 rock truck. EE had EE's left hand on the oil fill neck of the torque converter. EE was using a pry bar in EE's right hand to help remove the converter. When the torque converter came loose, it popped to the side and pinched EE's left hand against the truck frame.
Team Member inadvertently skipped two steps of stairs and fell forward. EE reached the landing feet first but felt EE's knee hyperextend. Team Member immediately felt pain in knee. The injury was classified first aid until 5/2/16 when the doctor requested physical therapy and prescription medication.
Operator inadvertently lowered the loader bucket while approaching the Krupp Crusher feed hopper. The loader was too close to the hopper causing the loader boom to contact edge of hopper jarring the operator in the cab.

Team member stepped on a shackle and twisted his ankle.
Team member didn't seek doctor until 12/8/14. He was dumping material from floor sweeper hopper into dumpster. Hopper (21""x10""x10"") started to slip from team member's grasp. He felt pain when he tried to regain control of the hopper.
Team member reporting feeling pain in lower back when pull cord halted or recoiled while starting trash pump motor. Did not seek medical attention until 4/10/14. Returned to work with restrictions same day.
The contractor EE was repairing a conveyor belt. The EE was pulling back a section of rubber/top cover from the conveyor belt's damaged area. A section of the top cover broke loose abruptly resulting in a backwards motion of the contractor EE's Right Arm/Elbow, subsequently the Right Arm/Elbow made contact with a piece of metal which was part of the structure in the work area.
Team member reported to supervisor on March 6, 2014 that he had been exposed to poison oak while repairing a damamged pipe. Team member was sent to doctor on March 6, 2014. He was diagnosed and treated for poison oak dermatitis. Team member was released to work on March 7, 2014.
Team member was driving the utility vehicle from the Blue House area to the office. While driving downgrade near the Recycle area, the vehicle began to slide and turned into the berm on the right side of the road. The front of the vehicle contacted the berm then rolled over onto the passenger side. Team member received a concussion and laceration to the left side of the head.
Team member injured his left knee when he turned while shoveling in a trench.

Injured worker lacerated the index and middle fingers of his left hand when fingers were caught between the bearing housing being lowered onto a pallet by an overhead hoist and a nearby maintenance workbench when the load shifted. Lacerated fingers required sutures.
While opening transmission service door away from his body, Team Member's left middle finger became caught between door handle and surface of service door, resulting in fracture to tip of middle finger.
While handling a light fixture, it started to fall. The injured worker caught it with his right hand. The fixture had sharp edges which cut his index finger and bruised his middle finger.

Employee reported he was working under gear box when dirt fell into left eye. He was wearing safety glasses. Dirt entered in gap between safety glasses and face. He flushed eye with water about 20 minutes later. Eye became irritated over the next two days. Employee reported eye irritation at start of shift to supervisor on 12/20/2012. Corneal abrasion to left eye.
Team Member was climbing ladder to access head pulley drive area of conveyor. When he reached for ladder's railing, he jammed his right little finger into ladder's rung support iron. His finger was fractured.
Employee reported that while attempting to remove a broken weir board from a sluice box, he slipped and fell down the bank of an empty dredge pond. Injuries were contusions to back and ankle and ankle sprain
Job title - Driller. Was placing a drill hammer into the bed of a pick up truck. He pushed the hammer into the bed of the truck. He received a cut on his hand from a sharp edge of the hammer. The injured employee was wearing leather gloves at the time.

While bending down to retrieve parts out of a deep box, employee felt pain in his side.
Removing bend pulley from conveyor. Injured worker grabbed onto lifting strap that was hooked to the pulley that was hooked to crane. Pulley shifted and his left hand was caught between strap and beam. Left ring finger was lacerated, requiring 7 stitches.
EE was replacing teeth and edges on loader bucket in welding shop. The last two edges were flat on the floor and could not be hooked onto overhead hoist. EE attempted to lift an edge up but the other edge slid out and caught his left index and middle fingers. Lacerations on both fingers requiring several stitches on each finger.
Injured worker stepped on a rock as he exited a pick-up truck. When he stepped on the rock, he twisted his ankle.
Employee reported that while surveying in remote area of the quarry, she was struck by a bee and stung. Antibiotics were required to resolve the injury.

Employee was moving loadout flop gate lever with come-along. Lever broke at pivot point. Employee twisted and moved to avoid being struck by lever. Employee fell onto back, hardhat came off and head struck the ground. Also sprained his back.
Employee reported accident occurred while clearing plugged starter steel on drill rig. Compressed air had been used unsuccessfully to clear debris. Air was bled off Workers waited 2 minutes before trying to clear plugged steel with a pry bar. Plugged material came free, releasing residual compressed air, water, mud and debris onto worker's right hand and coveralls.
Team Member was weed wacking and stopped make contact with a passing haul truck. Team Member lost footing on the berm where he was cutting weeds. Slid down slope of berm. Upon stopping at bottom of slope he felt a ""pop"" and then soreness in right knee.

Employee attempted to get out of his work truck, sliding out to the right when he felt a sharp pain in his lower back as he put weight on his right leg.
While cutting a hole in metal plate, metal ""popped"", causing slag to enter clothing at open neck. Hot slag ran down chest and abdomen, causing burns.
Come-along holding belt up slipped, causing roller to move and catch finger between roller & belt structure.
EE fell off equipment while performing 125 hour service on aerial lift.

Backed overpile of material, felt jolt up through seat into his back. Started to feel pain early last night, reported at 10:00 this morning did not feel well and requested medical care.
Employee notified company on 9/15/08 he wanted to open old claim for cumulative trauma to left wrist.
While trying to clear the chute using a pry bar. EE lost his grip on the bar. The bar hit the belt-which was still running. The bar bounced back, hitting him in the chest and mid-section. Diagnosis at clinic is minor fracture of sternum.
While turning over the plate, it slipped, smashing left ring finger between the plate and the welding table. Advised by doctor on 5/2/08 employee was authorized to return to full duty duty, no restrictions, on 4/16/2008
Apparently grit on EE's clothing fell into EE's lunch. While eating lunch he bit down onto fine sand material (grit), which EE spit out. While eating a snack during break time, EE's back tooth fell out.

Tugged on case cover when it was hung up and would not lower. Cover went into closed position at the same time employee put hand in pinch point on the cover to maintain his balance. Cut hand which required sutures. Treated and released to regular work with no restrictions.
Smashed index finger between the side plate and the other half of the gate. Fracture of right index finger. On modified work. Released to regular work on 8/10/07.
EE was using a snap-on filter wrench with a 24"" breaker bar to loosen a filter the breaker bar slipped out of the filter wrench socket and his left elbow struck a strut housing. No reportable medical treatment until 7/12/07.
Reported pain after the job was completed. Returned to regular duty on 4-26-07.
