Checking oil on 998 loader, slipped on steps due to mud on shoes due to recent rain
While exiting a front end loader at the rear step ladder, lost footing right foot got stuck as ee exited and ee twisted right knee
Employee was reaching down to ground to pick up pipe wrench with right hand when EE's left foot slipped on flat ground with a thin layer of gravel causing EE to strain lower back. Employee had good soles on boots
Employee went up stairs using hand rails and missed a step. He twisted his right knee when he slipped. Employee reported injury on 6-1-15. Doctor did not issue restrictions until 6-23-15. Supervisor did not learn about restrictions until 7-14-15.
Parking area had material added to it in the a.m. raising the truck level so that final step to ground was about 3'.
The lab Tech was walking into the lab and tripped on rubber floor mat. We repositioned the mat away from the entrance to reduce the trip hazard. EE fell onto concrete floor straining or spraining left wrist. He was placed on modified duty.
Loader was parked on pressure wash pad, employee was climbing down ladder and slipped
Employee was clearing a rock plug from the blue rock plant's C1 conveyor discharge chute and slipped when dismounting the work area. He fell onto the hand pulley metal skirting and lacerated his left buttock. Medical treatment was adminsitered at a local emergency room and the employee was placed on modified duty.
The EE was exiting a Loader #71-926. As he climbed down the ladder his hand slipped on the handrail. He twisted his left knee. Saw company doctor on 4-30-09. No restrictions that prevented him from doing his job. Returned to doctor on several occasions. Surgery done on 7-29-09. Reportable as of then.
EE was walking on discharge chute. As he was climbing out his foot slipped causing lower back pain when he landed. The screen discharge chutes were on the ground while maintenance was being performed.
Employee was dismounting loader and missed step. Injury was diagnosed as a lower back strain and employee was placed on restricted duty.
Hands full of tools, slipped and could not catch himself, iron was wet.
EE was removing hammers on the impeller at the AB Plant when the wedge preventing the impeller from rotation broke loose. The impeller rotated throwing the EE against the backwall(curtain). EE attempted to break the impact with his arm and struck his left elbow. He worked the rest of the week but pain and inflamation worsened and he went for medical treatment over the weekend.
When climbing on to rubber tire dozer left foot slipped through first step of ladder causing left leg to be pinched between fuel tank cover & step. This caused bruising & swelling to left shin.
Jump starting equipment( haul truck). Stepping across from truck to truck, lost footing, grabbed handrail, fell against the truck and fell to the ground.
Stepped off loader into hole.
Sprained left ankle while stepping off of service truck.
WHILE EMPLOYEE WAS EXITING THE MACHINE HIS LEFT FOOT SLIPPED OFF THE STEP. HIS LEFT LEG WENT BETWEEN THE DRILL & MACHINE. HIS MOMENTUM CAUSED HIS BODY WEIGHT TO SHIFT TO THE LEFT CAUSING PAIN IN HIS KNEE.
INSPECTING SLINGS, TURNED TO WALK AWAY & SLIPPED ON GREASE OIL.
GETTING DOWN OUT OF A WATER TRUCK TO FILL IT. STEPPED OUT & FELT PAIN IN LEFT KNEE.
WAS HEADED TO HIS VEHICLE - GOING HOME. STEPPED IN HOLE FROM STEM IN GROUND, TWISTED LEFT KNEE.
EE was carrying a 8' ladder along the side of a silo scale when he slipped on a slight slope falling backward landing on a 3" puc pipe injuring his mid to upper back and tearing ligaments and muscles around his left side rib cage.
Climbing up and down equipment ladders, strained groin.
WHILE CLIMBING BACK ONTO THE DOZER EE GRABBED AT A HANDLE AND MISSED. HE TRIED TO JUMP TO AVOID LANDING ON HIS BACK AND LANDED ON HIS LEFT SIDE INSTEAD.
EE SLIPPED OFF THE BOTTOM RUNG ON LADDER WHILE CLIMBING DOWN FROM THE MACHINE.
PLACED LADDER UP AGAINST WALL, CLIMBED UP AND STARTED UNSCREWING SCREWS LADDER SLIPPED OUT AND DOWN CAUSING EE TO FALL.
EMPLOYEE WAS CUTTING ARUNDO WHEN HE TRIPPED AND FELL ON HIS RIGHT ARM. THE ARUNDO STRUCK HIS ARM AND PUNCTURED THE SKIN.
EMPLOYEE WAS CLEANING CHUTE WHEN HE LOST HIS BALANCE AND STARTED TO FALL, WHILE TRYING TO JUMP CLEAR OF CONVEYER BELT HE STRUCK HIS RIGHT FOOT ON THE RAIL CAUSING FOOT INJURY.
EE SLIPED AND FELL USING HAND TO CATCH HIMSELF HE FELL ON BOLT STICKING UP FROM DECK. THEY HAD COVER OFF AND WORKING UNDER COVER.
CLIMBING UP 988B LOADER WITH FUEL NOZZLE IN ONE HAND, SLIPPED AND FELL UP AGAINST TIRE THEN FALLING TO GROUND.
HE WAS BENT DOWN WELDING FOR ABOUT 15 MINUTES, THEN WENT TO CLIMB UP LADDER. FOOT SLIPPED OFF, THEN WENT TO PUT FOOT BACK ON LADDER AND FELT SHARP PAIN IN LOWER BACK.
HE SLIPPED ON TRACK WHILE GETTING ON DOZER AND CAUGHT HIS RIGHT LEG ON GROUSER.
WHILE SERVICING ROCK TRUCK, EE SLIPPED AND FELL BETWEEN THE LEFT FRONT TIRE AND THE MUD FLAP FRAME, INJURING HIS LEFT KNEE DURING FALL.
EE WALKED UP STAIRS TO WATER TANK AND STEPPED ON SOME LOOSE ROCKS WHICH CAUSED HIM TO TWIST HIS KNEE AND FALL.
SLIPPERY SURFACE - FEET SLIPPED OUT FROM UNDERNEATH. LANDED ON BACKSIDE WHILE ON TOP OF SCRAPER.
EMPLOYEE WAS HELPING DRILL CREW CHANGE DRILL STEEL. WHEN STEPPING DOWN FROM DRILL RIG, HE TWISTED HIS KNEE. HE STATED HE WAS WEARING HIS BIFOCALS LOOKING THROUGH THE LOWER PART AND THE GROUND WAS ACTUALLY FARTHER AWAY THAN HE REALIZED. CAUSED INJURY TO HIS LEFT KNEE.
