Showing all 27Employee was walking the drill pad prior to doing EE's pre shift on the drill. it had snowed 3 inches that evening and EE stepped on the edge of a tire track that caused EE to slip/Trip and fall to EE's knee. when EE came down, EE came in contact with a sharp-edged rock on EE's shin just below the knee cap. we took EE to urgent care where they gave EE 4 Stiches. same day.
On 11/18/2024 an Employee was getting into a pickup truck when employee slipped on the ice and caught themself on the door. Employee felt a twinge in lower back. Employee was taken in for a check up. Doctor gave a full release to go back to work no restrictions. We found out on 3/5/2025 the employee was using chiropractic care to treat the injury.
EE was descending a set of stairs when EE felt a pop in left knee.
Employee was descending off a Kenworth T-800 when employee lost 3 points of contact landing on butt on the ground. Causing pain to lower back.
Employee was exiting a 785 haul truck. Stepped off ladder rolling their ankle. Minor first aid was administered. Employee felt like could return back to work. Two days later employee started feeling more discomfort. We then requested employee seek evaluation from a Doctor. It was found that employee had fractured fibula. This injury went lost time on 02\24\2021
Operator was attempting to access the cab of a CAT D10 dozer. Slipped and fell onto dozer push arm. Employee was transferred to hospital for observation. After x-rays were performed it was determined by the doctor that the employee had fractured ribs.
An operator in the mill was running a loader on the mill stockpile ore pad, feeding the mill hopper. When operator was getting out of loader at the end of shift, employee felt a pain going down right leg. Employee later contacted a doctor, where an MRI was performed. On March 16 a cortisone injection was administered. Minor Injury report was filed Feb.14 2020
Employee was heading up the stairs to the upper control room of the mill to answer the phone. Employee was just reaching the top of the stairway and caught their toe on the step causing them to fall onto the grading. Employees thumb went through the grading breaking their thumb.
Employee was walking in the mill area between the flock tanks on main floor. Employee was going to get hose to finish the clean up at shift end. Employee slipped on the wet concrete, falling and bruising their right forearm. Employee went to doctor for ex-rays, there was no fracture, however doctor sent employee home.
Employee was walking up the stair way into the lubrication planning office, slipped on some black ice and was not able to catch themselves, landing on right shoulder. It had snowed the night before, snow had been cleaned around the steps but there was a small patch of ice that employee did not see. Injury went lost time on 03/29/2012.
Employee was under a D-10 dozer doing maint. When employee crawled out they felt a pain in right knee (employee has had prior issues with same knee). Surgery was done on 2/6/2012 and it has not been determined whether this is preexisting or a new injury.
EE was tightening a bolt on the ROPS of a dozer when he slipped and fell to the ground. Approximately 5' hitting lower back.
EE was climbing ladder to perform engine service. EE slipped on the second to top rung of the ladder. While falling to the ground EE grabbed the handrail and felt something pop in right arm.
EE TWISTED KNEE CARRYING A WATER BOTTLE INTO THE MILL, CAUSING POSSIBLE CARTEDGE DAMAGE. STATE WORKMAN COMPENSATION REPORTED IT AS WORK RELATED ON 12-8-03.
EE WAS WORKING IN A CONVEYOR DRIP PAN. WHEN CLIMBING OUT OF THE DRIP PAN THE EE TRIPPED & FELL ABOUT 3' ONTO WALKWAY, HITTING SHOULDER ON STEP.
EMPLOYEE JUMPED FROM DRILL PLATFORM TO GROUND (ABOUT 3') AND FELT A PAIN IN LEFT GROIN. EMPLOYEE WAS TAKEN TO TOWN FOR EXAMINATION, THE DOCTOR RELEASED EMPLOYEE WITHOUT ANY RESTRICTIONS. THE E MPLOYEE WENT OFF WORK ON 4/7/03 FOR A HERNIA OPERATION. WE RECEIVED CONFIRMATION FROM WORKMANS COMP ON 5/5/03 THAT THE INJURY WAS WORK RELATED.
EE WAS CLIMBING OFF DOZER & MISSED STEP OR SLIPPED ALLOWING FULL WEIGHT OF BODY TO PULL ON ARM PULLING MUSCLE.
CLIMBING OFF TRUCK SLIPPED ON STEP AND FELL, THEN CAUGHT HERSELF WITH RIGHT ARM DURING FALL. EE WENT TO DR ON 6-3-02, BECAME REPORTABLE ON JUNE 7, 02.
INDIVIDUAL WAS WALKING ACROSS THE PIT FROM ONE PIECE OF EQUIPMENT TO ANOTHER WHEN HE TWISTED HIS FOOT. PIT WAS COVERED BY GLAZED SNOW.
INDIVIDUAL WAS CARRYING 2" CONDUIT UP A LADDER WHEN HE FELT PAIN IN LEFT HEEL. INJURY WAS REPORTED 1/15/02 AFTER IT WOULD NOT GET BETTER. ON 1-16-02 INDIVIDUAL WENT TO HOSPITAL AND HE WAS DETE RMINED THAT A PIECE OF BONE AND TENDON HAD BROKEN LOOSE FROM THE INDIVIDUALS LEFT HEEL UNKNOWN LAST TIME.
INDIVIDUAL STEPPED THROUGH FLOOR GRATING, INJURY TO LEFT KNEE / LEG - BRUISE/SPRAIN. (AGGRAVATION / RE-INJURY OF PREVIOUS INJURY.)
EE TRIPPED ON FLOOR GRATING, FALLING AND CUTTING HIS HEAD ABOVE THE RIGHT EYE FROM THE RIM OF HIS SAFETY GLASSES. REQUIRED SEVERAL STITCHES.
OPERATOR SLIPPED ON A ROCK ON A BERM AND LANDED ON A SHARP ROCK ON HER KNEE, REQUIRING MEDICAL TREATMENT.
EE WENT TO TURN ON STEAM CLEANER IN WAY BAY TO CLEAN HIS BOOTS. HE FELL THROUGH THE SUMP GRATING AND SKINNED HIS LEFT LEG SHIN. SUFFERED AN ABRASION. MEDICAL TREATMENT INCLUDED PRESCRIPTION AN TIBIOTICS TO PREVENT INFECTION.
EMPLOYEE WAS WALKING OFF A BERM. A ROCK ROLLED UNDERFOOT CAUSING THE EE'S KNEE TO TWIST. THE EE WENT TO THE DR ON 12/18/00 AND WAS SCHEDULED FOR SURGERY. EE ALSO HAD A PRE-EXISTING CONDITION F ROM AN "OFF-THE-JOB" INCIDENT.
EXITING CAB OF 2593 AND MISSED LOWER STEP CAUSING HIM TO FALL TO THE GROUND, BRUISING HIS HIP.
