Employee was walking the drill pad prior to doing EE's pre shift on the drill. it had snowed 3 inches that evening and EE stepped on the edge of a tire track that caused EE to slip/Trip and fall to EE's knee. when EE came down, EE came in contact with a sharp-edged rock on EE's shin just below the knee cap. we took EE to urgent care where they gave EE 4 Stiches. same day.

Employee was doing a brake inspection of a Trailer when EE was measuring the slack adjuster a small chunk of dirt fell between EE's safety glasses and EE's eye causing scratches on EE's eye. we took EE to urgent care where the prescribed EE antibiotics.

EMPLOYEE WAS WIPING DOWN HUB SURFACE OF THE LEFT FRONT BRAKE ON 13060. ANOTHER EMPLYOEE WAS REMOVING DUO-CONE SEAL AT THE SAME TIME. THE DAMAGED METAL RING SPUNG FORWARD AND HIT THE EMPLOYEE IN THE RIGHT THUMB KNUCKLE CUTTING IT OPEN, RESULTING IN STITCHES.

Electrician was stripping wire with a box knife, when the electrician pulled the wire the knife caught pinky finger requiring 3 stitches.

Employee was picking up a chock block that was froze down and felt discomfort to their left shoulder. They continued to work and did not want to be seen by a doctor. On April 18th they reported that they were feeling a lot of pain and would like to be seen by a physical therapist. They then received dry needling.

On 11/18/2024 an Employee was getting into a pickup truck when employee slipped on the ice and caught themself on the door. Employee felt a twinge in lower back. Employee was taken in for a check up. Doctor gave a full release to go back to work no restrictions. We found out on 3/5/2025 the employee was using chiropractic care to treat the injury.

Employee strained their left wrist and forearm back on 08/08/2018, pushing on the door handle of a 785 CAT haul truck. Surgery had to be performed on 04/21/2021 to remove some scar tissue from the wrist.

Employee was exiting a 785 haul truck. Stepped off ladder rolling their ankle. Minor first aid was administered. Employee felt like could return back to work. Two days later employee started feeling more discomfort. We then requested employee seek evaluation from a Doctor. It was found that employee had fractured fibula. This injury went lost time on 02\24\2021

Operator was attempting to access the cab of a CAT D10 dozer. Slipped and fell onto dozer push arm. Employee was transferred to hospital for observation. After x-rays were performed it was determined by the doctor that the employee had fractured ribs.

An operator in the mill was running a loader on the mill stockpile ore pad, feeding the mill hopper. When operator was getting out of loader at the end of shift, employee felt a pain going down right leg. Employee later contacted a doctor, where an MRI was performed. On March 16 a cortisone injection was administered. Minor Injury report was filed Feb.14 2020

Employee was shoveling frozen mud from the track frame of a D-10 dozer when EE felt a strain in right shoulder. Employee went to the doctor for observation two days later. EE was referred to an orthopedic surgeon. This injury went LTA on March 14, 2019.

Employee was removing a Lock Ring from a 785D wheel assembly. The Lock Ring began to fall off the wheel assembly. The employee tried to stop the Lock Ring from falling resulting in the employee getting thumb smashed.

Employees were replacing teeter tubes inside the hydrosizer. There were filings left inside tubes from drilling out nozzles. While lowering teeter tubes filings fell onto employee getting into left eye. Employee flushed eye and thought debris was cleaned out. On 5/31/18 employee was taken to the doctor and a small fragment of metal was removed.

Two employees were removing a pivot shaft seal using a pry bar and a utility knife. While one employee was prying on the seal the other employee was cutting the seal when the pry bar slipped off and struck the employee that was cutting the seal. Employee received three stitches in nose.

Employee was using a Hilti caulking gun to dispense epoxy into loose bolts. Employee failed to puncture the seal on end of tube and pumped pressure into the tube. When nothing came out, employee turned tube to look inside the end, the seal burst shooting epoxy onto safety glasses and into employees eye.

Employee was lowering a piston into a ram of a track adjuster for a D 10 dozer. When piston was in place employee grabbed it and was lowering the overhead crane to unhook lifting straps. The crane hook came in contact with left pointer finger, requiring stitches.

Employee reported to work on 6/23/16, knee was swollen so employee left to see doctor. Employee was admitted into the hospital with a staph infection. Employee had some old welding burns on arm and possibly one on leg. It's unclear as to what caused the infection and waiting for doctors report.

Employee was using a pocket knife to cut a hole in and vent the bottom of a 1 gallon RV antifreeze bottle used to fill the water system of a blast hole drill. The bottle slipped from his hands, employee tried to catch it and in the process the knife cut his left thumb requiring seven stitches.

Kenworth blast truck was being used to load holes on a shot on the G haul road when driver lost control and rolled down a steep ramp tipping truck up onto its side at the bottom of the ramp. The driver was pinned under the steering column for approximately 2 hours. Employee was transferred by helicopter to Eastern Idaho medical center.

Employee was helping install Hilfickers around the inlet and outlet of a fifteen foot culvert. Employee stepped over the four foot rise to retrieve a rake that was stored inside the culvert. Employee lost footing and slipped puncturing upper leg with a 3/8 inch wire located on top of the hilfickers requiring two to three stitches.

Person had is arm and hands between a fusion machine carriage using a crescent wrench. He was using this tool to wrench out the liner to help the liner pipe line up. staff member closed the carriage on his arm. This was caused from miscommunication between the two staff members. Tyrone was checked out a medical facility and had no breaks or cuts. He did have a bruise.

Employee was heading up the stairs to the upper control room of the mill to answer the phone. Employee was just reaching the top of the stairway and caught their toe on the step causing them to fall onto the grading. Employees thumb went through the grading breaking their thumb.

Employee was walking in the mill area between the flock tanks on main floor. Employee was going to get hose to finish the clean up at shift end. Employee slipped on the wet concrete, falling and bruising their right forearm. Employee went to doctor for ex-rays, there was no fracture, however doctor sent employee home.

Employee was walking up the stair way into the lubrication planning office, slipped on some black ice and was not able to catch themselves, landing on right shoulder. It had snowed the night before, snow had been cleaned around the steps but there was a small patch of ice that employee did not see. Injury went lost time on 03/29/2012.

Employee was under a D-10 dozer doing maint. When employee crawled out they felt a pain in right knee (employee has had prior issues with same knee). Surgery was done on 2/6/2012 and it has not been determined whether this is preexisting or a new injury.

Employee was using a pry bar to dismantle and repair a light duty vehicle drive shaft when he sustained right shoulder pain. Was reported as a minor strain and employee would monitor improvement. Smoky Canyon received notification 12/12/11 from workmans comp that this injury was work related after an investigation was completed.

Employee was Using an air arc, had face shield and safety glasses on. Employee lifted shield, had not pushed the air release button on gouging torch which blew debris into the air. Employee did not feel anything in the eye until that night and did not seek medical attention or report until the next day. The eye doctor removed a small particle of rust from the eye.

Employee was cleaning work area at close of shift, was using a pressure reducer on an air hose to clean up work bench. There was an accumulation of dust and metal shavings that was hidden between items on the bench; employee had a foreign object taken out of left eye as a result.

Employee was removing roller pin with a porta power from the under carriage of a D10 dozer. Pin became wedged,employee was tapping it with a 3lb. hammer. When the pin gave free it hit employee in the thigh bruising it. The next day employee's leg hurt, we sent him to the doctor. Examination showed possible blood clot, employee was admitted into the hospital for observation.

Employee was using a porta power to push a pin out of track frame, the pin slipped and porta power fell on employees right shoulder. There was damage to a tendon and surgery was done on 11/22/2010, first lost day was 11/23/2010.

Employee was in the shuttle bus ridding back from pit area to the main mine parking area. The bus hit a rut in the road and as a result the employee hit left elbow on window frame, needing to see the Doctor. The Doctor could not verify the elbow was fractured but recommended light duty for a few days.

Employee was opening a can of welding rods by pulling on the pop top tab, it hung up and employee grabed the can to get a better hold. Employee was not wearing gloves and caught right index finger on edge of lid, cutting index finger. Required four stiches.

Employee was using a saws-all to cut a 2x6 stud with a plywood sheet attached to the back. The saw kicked back when it hit a knot or a screw in the wood, cutting left 5th digit and requiring stitches.

Employee was using a drill press to drill 1-1/2 inch hole, bit was slipping so employee was using a chuck to tighten. Employee stepped to the right to get a better position and hit the foot actuator, turning the drill bit and chuck. Employee had two stitches in hand as a result.

Employee was operating a 992G Cat front end loader when a hydraulic hose on the tilt ram sprayed fluid inside the cab. Operator had safety glasses on but did get some over spray in eyes. For precaution, employee went to hospital to be evaluated. Flushed eyes and presciption eye rinse was prescribed, employee returned to work.

Employee was working in the B pit area on the date of incident and around 2:00 PM. He started to have some irritation in left eye, he filled out an accident packet at end of shift. The next morning he was taken to the hospital and a small foreign body was taken out of eye and antibiotic was prescribed in eye drop form.

Employee was using a pressurized grease system on 2085 lube truck to get a small amount of grease to put on the o-ring on a hyd. filter while servicing a D10 dozer. The grease system injected grease through the gloves and into the right hand, 3rd joint of middle finger.

Employee was testing a hydraulic pressure line and was using a pressure hose guage with a ball valve. The guage or ball valve was rated as high as the pressure in the line. The ball valve came apart and cut employee's hand, requiring three stitches.
