Showing all 8Miner was climbing onto a crane to look at the load chart. EE used a pintle hitch as a step, and it rotated as EE put weight on it. Miner fell back wards and tried to brace the fall by putting left arm behind him. Fracture left arm, no cast. Miner released to modified duty.
Employee was removing hatch cover on the dredge to inspect the water level in the hold. The employee was using a long pry bar to remove the cover, the pry bar slipped and the employee fell on the deck and broke his right ankle.

IN MARCH 2000 HE ALLEGES SLIPPINGFROM LOADER DUE TO LOOSING HIS GRIP & STARTED HAVING MAJOR BACK PAINS. ON AN UNKNOWN DATE WHILE FUELING, HE STOOD UP FROM A SQUAT & HURT BACK.
