Employee fell off ladder and landed on their hand, injuring a pinky.

Employee was stung by a bee while traveling in an open cab utility vehicle. Employee is allergic and administered personal epi pen. Employee follow up with doctor and was required to be off work.

Miner was dismounting a 988k Loader and missed the last step fell backwards and cut their head.

Fall from machine

Miner was climbing onto a crane to look at the load chart. EE used a pintle hitch as a step, and it rotated as EE put weight on it. Miner fell back wards and tried to brace the fall by putting left arm behind him. Fracture left arm, no cast. Miner released to modified duty.

Gripping Jaw clamp was being used to position conveyor belt. The clamp slipped off of belt and contacted employee's hands fracturing right pinkie and left thumb.

Miner was cleaning under a conveyor belt when EE attempted to stand up, EE struck back on the conveyor support. Miner reported the injury on 07/02/2018. Miner taken to clinic with back pain. Dr. ordered 2 more days of no work, then 11 days restricted duty.

Mechanic was replacing a steel pivot arm when it slipped and pinched the ends of both thumbs. Left thumb required stitches and right thumb required surgery to place a pin in thumb.

Employee was removing hatch cover on the dredge to inspect the water level in the hold. The employee was using a long pry bar to remove the cover, the pry bar slipped and the employee fell on the deck and broke his right ankle.

Employee was cleaning out a plugged chute using a pressure water hose, while leaning over metal railing. After completing the job, employee felt some pain in his ribs on the right side. Employee reported incident on 11/30/2007. Employee chose not to go to doctor until December 3, 2007 after pain did not go away.

Struck by powered moving object

EE was grinding on a bolt, holding the bolt in one hand with pliers and holding the grinder with the other hand. The grinder kicked back, striking EE's left hand, causing a laceration.

EE was removing 3""X3"" angle steel from rack, when hornet flew out from hidden nest in pipe below and stung EE in area of left eye. EE was reportedly wearing safety glasses.

EE was cleaning mud from 2C-07 stacker. While pulling 1-1/2"" hose, the hose became tangled. While pulling to untangle the hose, EE reported feeling a pain in his stomach. EE reported incident on 6/19/07. Ee did not lose any time at work until, 8/4/07, when surgery was performed.

Clay fell from bucket while trollying back to digging position, causing splash from hitting water to break lower cabin window. Steel grate above the glass fell and hit operator in head resulting in laceration that required 7 stitches.

Employee was attempting to remove steel (angle iron) from hydraulic gate. Gate was opened to remove steel. Employee was pulling on steel to remove gate. Pinching steel caused it to jump and corner of angle cut palm of hand through glove resulting in seven stitches.
