Employee fell off ladder and landed on their hand, injuring a pinky.

Employee was stung by a bee while traveling in an open cab utility vehicle. Employee is allergic and administered personal epi pen. Employee follow up with doctor and was required to be off work.

Miner was dismounting a 988k Loader and missed the last step fell backwards and cut their head.

Miner was climbing onto a crane to look at the load chart. EE used a pintle hitch as a step, and it rotated as EE put weight on it. Miner fell back wards and tried to brace the fall by putting left arm behind him. Fracture left arm, no cast. Miner released to modified duty.

Gripping Jaw clamp was being used to position conveyor belt. The clamp slipped off of belt and contacted employee's hands fracturing right pinkie and left thumb.

Miner was cleaning under a conveyor belt when EE attempted to stand up, EE struck back on the conveyor support. Miner reported the injury on 07/02/2018. Miner taken to clinic with back pain. Dr. ordered 2 more days of no work, then 11 days restricted duty.

While raising a belt to change a return roller, miner sprained left shoulder.

Mechanic was replacing a steel pivot arm when it slipped and pinched the ends of both thumbs. Left thumb required stitches and right thumb required surgery to place a pin in thumb.

Employee was removing hatch cover on the dredge to inspect the water level in the hold. The employee was using a long pry bar to remove the cover, the pry bar slipped and the employee fell on the deck and broke his right ankle.

Miner was operating a skid steer loader when a 4"" rock fell from the elevated bucket and struck him on the finger causing a laceration and swelling.

Employee reported to management that he felt pain in left knee. No specific recent event identified as cause of knee pain.

Employee was clamping channel iron to forklift fork. Channel iron rotated pinching right ring finger between fork and channel iron resulting in a laceration requiring 10 stitches.

EE was using pry bar to dislodge rock from conveyor frame when bar slipped causing wrist strain.

Employee was manually rotating a belt drive unit of a logwasher and caught the tip of his right middle finger in the nip point between the drive belt and the sheave bruising the finger.

Employee was performing an inspection of the dredge pontoon when he slipped and fell, striking his head on floor.

Employee was cleaning out a plugged chute using a pressure water hose, while leaning over metal railing. After completing the job, employee felt some pain in his ribs on the right side. Employee reported incident on 11/30/2007. Employee chose not to go to doctor until December 3, 2007 after pain did not go away.

EE was grinding on a bolt, holding the bolt in one hand with pliers and holding the grinder with the other hand. The grinder kicked back, striking EE's left hand, causing a laceration.

EE was removing 3""X3"" angle steel from rack, when hornet flew out from hidden nest in pipe below and stung EE in area of left eye. EE was reportedly wearing safety glasses.

EE was cleaning mud from 2C-07 stacker. While pulling 1-1/2"" hose, the hose became tangled. While pulling to untangle the hose, EE reported feeling a pain in his stomach. EE reported incident on 6/19/07. Ee did not lose any time at work until, 8/4/07, when surgery was performed.

Clay fell from bucket while trollying back to digging position, causing splash from hitting water to break lower cabin window. Steel grate above the glass fell and hit operator in head resulting in laceration that required 7 stitches.

Employee replacing skirting on belt. While cutting out notch for bolt holes, the utility knife he was using slipped and he cut himself on his left wrist.

Employee was attempting to remove steel (angle iron) from hydraulic gate. Gate was opened to remove steel. Employee was pulling on steel to remove gate. Pinching steel caused it to jump and corner of angle cut palm of hand through glove resulting in seven stitches.

EE working on screen when he slipped catching his left index finger in the screen causing the finger to break at the first joint.

WHILE WALKING ACROSS UNEVEN FIELD THE EE ROLLED (TWISTED) HIS RT. ANKLE WHILE STEPPING ON RAISED PART OF IMPRINT FROM ROAD GRADER.

EE WAS DISMOUNTING FROM LOADER AND STEPPED ON A ROCK, TWISTING HIS LEFT ANKLE.

EE WAS CUTTING WITH A CONVEYOR BELT CUTTER & WHEN IT RELEASED THROUGH THE MATERIAL TOOK OFF. THE CUTTER WENT OFF THE GUIDE TRACK & EE AUTOMATICALLY TRIED TO CATCH IT. HE CAUGHT THE BLADE WITH RIGHT INDEX FINGER. REC'D 3 STITCHES

HE SPRAINED HIS BACK PULLING A BURIED 1 1/2"" WASH HOSE WHILE CLEANING A BURIED CONVEYOR.

CHANGING SCREENS, PULLING SCREEN OUT OF DECK WHEN ANOTHER EE GRABBED SCREEN TO HELP, CAUSING LOAD TO SHIFT, JARRING EE AND HURTING NECK.

EE SPLICING CONVEYOR BELT WHEN KNIFE SLIPPED AND LACERATED LEFT FOREARM.

EE CLIMBING STATIONARY LADDER, HE HAD ON WET GLOVES & SLIPPED FALLING TO THE GROUND APPROX 3-4' RESULTING IN A LUMBAR SPINE CONTUSION.

EMPLOYEE WAS GUIDING A SET OF SCREEN PANELS INTOPLACE AND HIS BACK GAVE OUT.

ON JULY 2, 2001 EE REPORTED BACK PROBLEMS IN RELATION TO WORK ACTIVITIES & HAS SUBSEQUENTLY BEENOFF WORK. OVER THE LAST YEAR OR SO, DRIVING OVERROUGH ROADS IN THE PLANT WHILE ON THE LOADER, HE HURT HIS BACK. IN MARCH 2000 HE ALLEGES SLIPPINGFROM LOADER DUE TO LOOSING HIS GRIP & STARTED HAVING MAJOR BACK PAINS. ON AN UNKNOWN DATE WHILE FUELING, HE STOOD UP FROM A SQUAT & HURT BACK.

FOREIGN OBJECT ENTERED EYE NOT CERTAIN AS TO HOW.

WELDING ON FEEDER SCREW AUGER AND RECEIVED FLASHBURN.
