Employee was cleaning the pads around the jaw crusher when they stepped into some mud and slipped causing them to fall into a steel supporting structure causing a small cut on their scalp. Employee stated their hard hat came off as their head went forward.

Employee was steam cleaning a 980H loader and when finished employee was dismounting the machine and the bottom rubber, strap, step broke causing the miner's shoulder to strike a platform above bruising it. Report shows a contusion with restricted use of arm for 5 days.

Employee was standing on the drag bucket to knock the drag cable out of the crow foot on the bucket. When EE tossed a drag chain out of the way it struck EE's foot causing EE to jump to the ground (1 1/2' down) and subsequently twisted knee. Mild strain of knee.

Employee was ascending the catwalk to reach the #2 conveyor when their foot slipped. Employee swung their arm out to stabilize themselves and in doing so struck the 53X chute with a flat head on blow. (No cut, laceration or puncture). Employee said they thought they were fine but condition worsened.

Aggravation of injury reported on 5/1/13. Employee twisted knee while installing silt fence aggravating his knee injury. Went to Dr on 7/17/13 who told him to take 2 days off work, then report to work with no restrictions. This report is submitted to add 2 missed days to the accident.

EE was stepping off of a mini excavator and twisted his knee.No real pain or discomfort at time so he didn't call the accident in. Called 2 days later to say he had pain. Sent him to clinic and the diagnosed a strain. He returned for a follow up exam and they requested a MRI. MRI revealed he had a torn meniscus so today he was scheduled for surgery on 5/9/13.

EE lost his balance when dismounting a screen and when he grabbed a pole, he twisted at the same time and tore the labrum in his left shoulder.

Accident did not become recordable until 7/21/08. EE was ascending the ladder to the pug mill (used as a feeder to the USI crusher), when he missed a rung, causing him to fall to the ground on his feet.

WHILE CLIMBING UP THE LADDER ON HIS ENDLOADER EMPLOYEE HAD MUD ON BOOTS. WHEN HE GOT TO THE 2ND RUNG HIS RIGHT FOOT SLIPPED CAUSING THE EMPLOYEETO STRIKE HIS NOSE AND FOREHEAD ON THE PORCH OF THE MACHINE.
