Emp was starting work when he saw a door open to grease room. Shut door to grease room and walked up steps. Grease on the bottom of his boot caused him to slip on steps twisting his leg. Employee did not miss any work until MRI showed torn tendon. Emp had surgery 12/23/13 and became a lost time reportable.

While checking beltline with co-worker, co-worker went up to top of hopper to unlodge a rock that was wedged. Hard hat fell off of co-worker onto beltline below. Emp attempted to grab hard hat and ran, slipping, and falling on his Lt. side. Strain to Lt. Shoulder, continued complaints of discomfort, examination showed tear / Lost time on 01/29/14 when surgery was performed.

Employee was exiting the cab of the excavator holding onto the handle on side of cab with Lt. hand and opening door with Rt. hand. Stepped out onto tracks of machine, slipped on muddy tracks, let go of door and held on handle with his Lt. hand causing all his weight to shift to his Lt. arm. Emp. remained at work until surgery was scheduled to repair torn rotator cuff on 02/29/2012

Emp. completed his task using a bobcat while exiting machine stepped out on uneven surface causing his Rt. leg to twist. Emp. continued full duty work while treating Rt. knee pain over the counter medications. Continued discomfort required further examination showed a Rt. knee meniscus tear. Surgery was on 01/09/2012. Lost time began on 1/10/2012 due to emp. shift.

Employee was climbing ladder to enter the Randon Water Truck. As employee reached the platform he felt he might have slipped or missed the hand rail causing him to fall backwards off the steps/ladder. He struck the ground causing a contusion to his Rt. hip and Fracture to his Rt. wrist.

Employee was entering the excavator climbing up the ladder to the machine. He slipped on ladder bumping Rt. leg and holding on rail/handle with Lt. arm/hand to stop his fall on 06/26/2008. Employee remained at work until surgery on 09/12/2008 for a ligament in his Lt. wrist that was repaired. Employee remains off duty.

Accident occurred on 11/20/07 employee did not seek medical attention until 11/30/2007. Surgery was 01/28/08. Employee was decending stairs on a belt conveyor line when he slipped while grabbing handrail felt strain in his lower back,Rt. side and groin area. It was determined employee had an umbilical hernia. Employee remained at work at his job until his surgery date.

Employee was walking on snow and ice-covered roadway to the rock storage building when he slipped and fell striking his right knee. Employee continued on regular duty. Discomfort in the knee continued; he was examined and doctor scheduled surgery on 1/18/06 to remove torn cartilage. Employee is off work as of 1/18.

EE WAS WORKING AT THE AIRSLIDE. STANDING ON TOP OF AIRSLIDE, FOOT SLIPPED & HE STARTED TO FALL, REACHED OUT TO GRAB HANDRAIL. RT ARM WAS CAUGHT ON THE HANDRAIL & PUSHED BACK CAUSING STRAIN / FRACTURE TO THE BONE IN RT SHOULDER. EE HAS RT ARM IN A SLING & IS ON RESTRICTED DUTY. NO RT ARM USE FOR POSSIBLY 10 DAYS - 8 WEEKS OF LIGHT DUTY - TREATING WITH ORTHOPEDIC DR.

Employee was lowering scaffolding lost balance and bent backwards feeling strain. Reported on 10-28-04 this occurred on 10-27-04. Went to his doctor on 11-3-04. An imbilical hernia was the possible injury, ee decided to be treated by his doctor and have surgery on 12-2-04.
