Speaking with co-employee running skid steer. Turned around to move stone stacker and tripped over skid steer attachment. Nondiscplaced fractures of head of right and left radius. Restricted days started on 5/26/2023.
Employee was climbing down from ground ladder on crusher when foot slipped and rolled ankle.
Operator was moving to check a conveyor on spread and stepped in hole. Awkward step caused pain in back, which made the employee seek medical attention.
Employee was climbing down a ladder and twisted his ankle on a stone.
EE was conducting pre-shift inspection on Front End Loader & slipped while climbing down. He caught himself with left hand before he fell. When he caught himself he pulled his shoulder. On the day of injury the EE did not think he was hurt.As time went on his shoulder started hurting quite a bit. The EE reported this injury 1/22/07 & sought medical treatment on the same day.
Driver was exiting dump truck and missed the top step, he fell to the ground and fractured his right ankle in two places.
Ice covered ground, slipped and hit knee on loader frame.
EE CLIMBED OUT OF LOADER, LOST GRIP AND FELL.
FOOT SLIPPED OFF LADDER AND HE HIT RIGHT KNEE ON CRUSHER.
INJURED LEFT KNEE AFTER TRIPPING OVER A CABLE.
