Miner was looking for A/C leak when EE's foot slipped from the door way to the track approximately 2 feet. The miner refused medical treatment at the time of injury stating EE's ankle was just sore. The miner stated EE's ankle had not improved on 11/21/2018 and requested treatment, the MRI indicated EE's left Achilles tendon was torn.

IEE was exiting the crusher/hopper. He fell 11.5 feet to the ground on his right side. Co-workers called 911 and 25 minutes later he was gernied by the EMTs then the helicopter showed up and was mediflighted to Amarillo hospital.
Employee was loading prills on the blasting truck. A bag shifted under his foot. He realized he was going to fall so he jumped from the truck bed to the ground. He hurt his right knee. Did not want to go to the doctor and keep on working. The next morning his knee was swollen but he still would not go the the doctor. He said it was all right.
Employee jumped off tail pulley about 2 1/2 feet to the ground and sprained right ankle.
THE GROUND WAS MUDDY. EE STEPPED OFF THE LOADER AND TWISTED HIS KNEE.
