EE & another employee were carrying a set of steps to load onto a trailer. The ground was covered in frost & icy. EE's feet came out from under him & he fell face first onto the steps he was carrying. It chipped & cracked one of his front teeth.

EE WAS WALKING DOWN THE RAMP TO THE BOAT DOCK. IT HAS BEEN RAINING AND THE PALLET STEPS WERE WET. WHEN HE STEPPED ON IT, THE STEP SHIFTED, CAUSING HIM TO FALL BACKWARDS. HE TRIED TO CATCH HIMS ELF BY PUTTING HIS LEFT HAND DOWN. HE CRACKED A BONE IN HIS LEFT HAND DOING SO.
