Showing all 18EE was exiting the operator's cab of a tri axel facing forward and missing the first step and fell to the shop floor striking the torch cart with EE's right hand as EE fell.
EE was descending the Water Truck with both hands free. EE stopped on the bumper step with both feet to relocate lunch box to the front bumper with left arm while maintaining 3 points of contact. EE slipped / fell 32" to the ground landing on both feet in the catchers position. EE fell forward hitting right shoulder on the lower step support bracket.
EE was climbing the right rear steps of 992K loader. Their right foot slipped off 3rd step as their left foot was reaching for 4th step. EE fell 11 inches to the second step which they caught with their right foot. This incident resulted in a strain in the EE right hamstring. On April 26, 2016 Doctor prescribed Physical Therapy. Up until now, this has been First Aid case.
The entire plant area was covered in a skiff of snow, with some small icy areas. Employee was walking from secondary plant area to primary area into an area that had not been cleared due to limited access. He slipped on a patch of ice injuring his left wrist.
Exiting excavator foot slipped off step. Both hands were holding onto hand rails. Felt pain in lower back when foot slipped off.
The driver climbed up the side of his tri axle dump truck and slip and fell off the ladder resulting in broken ribs and a separated shoulder.
Employee tripped on hose for propane attachment. Employee lost balance and struck right knee on catwalk. Forman was notified. One hour later, pain in right knee was severe. Company HR was notified. Employee was given a 2PM appt. with physician. Result was a sprain and employee was given a knee brace. Returned to restricted duty next day. Released to full duty on 1-14-11.
Employee was leaving the cab at the end of his shift. As he walked down the steps he had his left hand on the hand railing and on the right side was the motor compartment of the machine. When he stepped on the platform his feet went out from under him. Resulting in him falling backwards onto the steps.
Weather conditions were snow fall and icy surface at the place of the incident. Individual slipped while crossing a parking lot, falling on his right knee.

Employee was cleaning around tail pulley of conveyor. He tripped on a rock, catching himself with his left hand and injuring his left ring finger.
Employee was entering the excavator & had his left and right hands on the handrails, his right foot on the track and his left foot on the bottom step. He removed his left foot to place it on the track; his right foot slipped in the process and he was hanging from his arms. Dislocated shoulder pulling himself into machine.

EE WAS GETTING OUT OF DOZER AND GOING TO USE CAT LOADER WHEN HE SLIPPED OFF TRACK OF DOZER AND HIT HIS LEFT SIDE AND BACK ON THE TRACK. THE WEATHER WAS SNOWY AND ICEY.
