Employee pinched/smashed two finger tips on EE's right hand when caught between two steel shim plates while in the process of adjusting a Jaw Crusher. The pinching incident resulted in lacerations and contusions to the middle and ring finger on the right hand needing First Aid and stitches.

Employee slipped on icy metal after drizzle (light rain) froze in EE's work area. This was a first aid case until the follow up doctor appointment that occurred on 01-24-2023 at which physical therapy was prescribed.

Employee was operating an electrically powered hand drill, boring out a piece that was secured by a vice. The drill bit caught the metal piece dislodging it from the vice striking the employee on the left side of the face around eye area. The employee was wearing safety eye protection. The employee sustained a slight fracture of the left orbital.

Employee was entering the excavator & had his left and right hands on the handrails, his right foot on the track and his left foot on the bottom step. He removed his left foot to place it on the track; his right foot slipped in the process and he was hanging from his arms. Dislocated shoulder pulling himself into machine.

Triaxle driver was being loaded with R8 size rock. When the rock was placed in the truck bed, driver was bumped out of his seat and into the passenger seat and felt back pop.

Employee was trying to move a 55 gallon barrel out of the oil building. His figer was pinched between the barrel and the buildings door frame. He received seven sutures. There was a management change at this site on 10-11-04. In preparing the 4th quarterly mine report for 2004 this injury report was discovered.
EMPLOYEE WAS HELPING CHANGE SCREEN ON SECONDARY SCREW DECK. HE WAS OUTSIDE ON A SHORT LADDER TIGHTENING BOLTS WITH HIS RIGHT HAND. HE PUT HIS LEFT ARM UP OVER HIS HEAD TO HOLD ONTO THE CATWALK AND WHEN HE DID THIS HIS SHOULDER CAME OUT OF PLACE.
EE SAID 30 DAYS ATER HE HAD BACK PAIN THAT HE THOUGHT RUNNING HAUL TRUCK THAT THIS IS WHAT HURT HIS BACK, BUT THIS INCIDENT WAS NOT REPORTED TO ME UNTIL 30 - 40 DAYS LATER.

THE EXCAVATOR WAS LOCATED @ BASE OF A SHOT THAT WAS BLASTED THE PREVIOUS DAY, W/THE BOOM POSITIONED PARALLEL TO THE MUCK PILE & THE CAP RIGHT NEXT TO THE PILE. THE EE WAS USING THE HYD HAMMER TO BREAK SOME OVERSIZED ROCKS WHEN A ROCK SLID OFF THE SHOT & ROLLED INTO CAB OF MACHINE. CAB OF EXCAVATOR SUSTAINED DAMAGE & EE RECEIVED 6 STITCHES IN RIGHT LEG BELOW KNEE.
TEMP. WAS 10 DEGREES, COLD AND SNOW AND ICE. WE PUT DOWN ANTISKID, EE WAS HELPING CARRY A PIECE OF STEEL, WEIGHT WAS ABOUT 40 LBS.
EE WAS GETTING OUT OF DOZER AND GOING TO USE CAT LOADER WHEN HE SLIPPED OFF TRACK OF DOZER AND HIT HIS LEFT SIDE AND BACK ON THE TRACK. THE WEATHER WAS SNOWY AND ICEY.

THE EMPLOYEE WAS ASSISTING IN THE REPAIR OF A 48"" CONVEYOR BELT FOR THE PRIMARY CRUSHER. AS HE WAS PULLING THE BLET INTO PLACE, HE FELT A DISCOMFORT IN HIS LOWER BACK WHICH GOT PROGRESSIVELY WORSE AS THE SHIFT WENT ON.
EE WAS OPERATING A FRONT ENDLOADER TO LOAD TRUCKS OUT OF A STOCKPILE. AT SHIFT END, HE PARKED THE LOADER AND STOOD UP TO EXIT THE MACHINE. WHEN HE STOOD UP HE NOTICED A SHARP PAIN IN HIS BACK.

CHANGING A WEAR PLATE ON A CAT 980 BUCKET. THE BELT HEADS WERE WORN DOWN AND HAD SHARP EDGES ON THEM. THE EMPLOYEE TOOK HIS GLOVES OFF AND CUT HIS RIGHT WRIST. HE HAD MINER BLEEDING, BUT RECEI VED SUTURES.
