EE was walking out of The office and took a bad step. EE fell landing on EE's knee and wrist. EE notified EE's supervisor in which EE decided to go to the doctor to get checked out. The doctor diagnosed a sprained wrist.

Operator was loading haul trucks. Mechanic showed up to address an issue on the loader that had occurred. Operator briefly stepped out of loader to talk with mechanic about issue with loader. Using 3-point contact to step off loader, the operators hand slipped causing EE to hurt EE's knee. Operator did not fall off ladder.

EE was desending the ladder. At mid-point when he stepped down, his weight shifted pushing the ladder to the right. His right foot became entan geled in the step of the ladder.

EE WAS HELPING CHANGE OUT THE SCREEN DECKS OF THE WET SCREENING PLANT. HIS FOOTING CAUSED HIM TO FALL BACKWARD, LANDING ON HIS BUTT. EE DID NOT REPORT THE INCIDENT OR MISS ANY WORK FOR ONE AND ONE HALF MONTHS.

MAINT. CREW WAS REPAIRING A CEDAR RAPIDS CONE CRUSHER. TOP OF CRUSHER HAD BEEN REMOVED EXPOSING A HOLE IN THE WORK DECK, WHERE THE CRUSHER ACCUM. HAD BEEN. THE INJURED PERSON THOUGHT SHE WAS S TEPPING INTO THE SMALL OPEN HOLE & OVER CORRECTED HER POSITION BY REACHING FOR A NEARBY LADDER. A FEW HOURS LATER SHE COMPLAINED OF A BACK PAIN & AT THAT TIME WAS TAKEN TO A MEDICAL FACILITY.
