Employee was climbing up the ladder on a DM45 drill. His hands slipped off the handholds and he fell forward striking his face on the upper part of the ladder. He did go to the E.R. and received 7 stitches.

On 03/17/08 EE was walking down the stairway by the welding shop in the mill, he slipped and fell down the stairs. He initially didn't go to the DR and felt fine. By the end of the week his back was still sore, so on 3/21/08 he went to the DR and was placed on light duty work.

EE had been recovering from an off the job, when he was watching a bumper being repaired on a straight truck when he stepped back to look at the bumper, stepped back, he stepped on a rock, caused him to lose his balance and fall onto his right hand, fracturing his wrist.

Employee was walking from one pickup truck to another. The ground was frozen and rutted. She rolled her ankle and felt a sharp pain. She was seen at the emergency room and diagnosed with a small fracture to the ankle.

IMPROPER RIGGING OF 70# BEARING CAPS; FAILURE TO WAIT FOR SIGNALS; MISJUDGEMENT OF PROPER CLEARANCE; FAILURE TO CLEAR PERSONNEL BELOW. VICTIM FELT HE WAS IN A SECURE POSITION AND FAILED TO CON NECT TO PROVIDED SAFETY BLOCK. RESULTED IN DISLOCATED RIGHT ELBOW, SCALP LACERATION, POSSIBLE RIB FRACTURES, ABRASIONS AND CONTUSIONS. HOSPITALIZED.

EE WAS WORKING ON A TRANSFORMER IN THE MINE AREAAND SLIPPED ON ICY GROUND. HE FELL LANDING ON HIS LEFT SHOULDER. HE WAS DIAGNOSED WITH A DEEP BRUISE. ON APRIL 29, 03 HIS PAIN HAD NOT SUBSIDED AND HE HAS BEGUN PHYSICAL THREAPY. HE HAS NOT MISSED ANY WORK.

EE WAS DESCENDING THE LADDER OF THE CAT HAUL TRUCK AND WHEN SHE WAS ON THE BOTTOM 2 RUNGS, MUDDY CONDITIONS CAUSED HER TO LOOSE HER FOOTING AND FALL. HER SOFT SIDED LUNCH BOX WAS SLUNG OVER HE R HEAD & SHOULDER AND SHE SAT ON THE LUNCH BOX WHICH HAD A CAN OF SOUP IN IT.

EE TRIPPED WHEN DESCENDING THE STEPS IN THE MILLAND FELL TWO STEPS TO THE LANDING STRIKING HIS RIGHT SHOULDER, ELBOW AND KNEE. HE PULLED MUSCLESIN HIS NECK. HE WENT OT A CHIROPRACTOR ON 5/8/01 FOR AN ADJUSTMENT.

EE WAS WORKING IN FRONT OF THE CRUSHER DUMP BOX ON A "CRUSHER DOWN DAY". A BERM OF MATERIAL HAD BEEN PILED IN FRONT OF THE BOX TO PREVENT DUMPING. EE WALKED ON THE BERM AND SLIPPED, CATCHING H IMSELF WITH HIS RIGHT ARM. HE JAMMED HIS RT SHOULDER AND ROTATOR CUFF. NO MEDICAL TREATMENT UNTIL 6-19-01.

THE EMPLOYEE WAS PLACING 6" DRISCO PIPE WATERLINE ON A BERM. HE STEPPED OFF THE BERM AND FELT A POP IN HIS LEFT KNEE. HE WAS SCHEDULED FOR SURGERY ON 5-16 TO REMOVE CARTILAGE FROM THE KNEE JOI NT. SURGERY SCHEDULED ON DAYS OFF.
