On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.

Showing all 5Employee was using a water hose to wash a concrete pad. He stepped off the concrete pad (3" elevation change) and lost his balance. He fell to the ground and struck his hand on a sharp rock causing a small laceration which required stitches.
Inspecting a screen he twisted his knee.
