Employee was spraying off concrete pad under Jaw Crusher. He said he slipped and sprained his ankle. He said he didn't need to go to the doctor, he just went on home. On 12/01/13 he reported to work and worked for several hours, then said his ankle was still bothering him, so he went to doctor the next day.

Employee was using a water hose to wash a concrete pad. He stepped off the concrete pad (3" elevation change) and lost his balance. He fell to the ground and struck his hand on a sharp rock causing a small laceration which required stitches.
