Employee claims that his Lanyard got wrapped around boot when squatting to work on cable tray. THIS ACCIDENT DID NOT BECOME REPORTABLE UNTIL 11-3/2005. IT WAS DETERMINED THAT SURGERY WOULD BE NEEDED.

Employee had been working on top of So. Boiler. Plant was getting ready to put boiler back on. As he was coming down the ladder he missed the last rung. Employee states that he felt himself start to fall and grabbed the ladder. By doing this he felt a pain in his back. Did not request medical treatment until 5/4/04.

THERE HAD BEEN APPROX 2-3" OF SNOW FALL DURING THE NIGHT COVERING A THIN LAYER OF SLEET. EE WAS ENTERING THE CRUSHER BUILDING AND FAILED TO NOTICE THE WALKWAY COVERED IN SNOW. EE'S HAD PUT ICE MELT ON THAT AREA AROUND 6 THAT A.M. EE SLIPPED AND FELL TWISTING HIS LEFT ANKLE.

EE'S LAST DAY OF WORK WAS 8 -15-03. HE LEFT OUR EMPLOYEE TO RETURN TO FALL SEMESTER OF SCHOOL. HE WAS EMPLOYED LAST SUMMER AT HEARTLAND CEMENT COMPANY.

EE WAS TRYING TO OPEN STARTER CABINET DOOR. THERE WAS A BUILD UP OF CEMENT AROUND DOOR. HE PULLED ON DOOR, IT WAS STUCK. CLEANED SOME OF THE BUILDUP FROM DOOR. EE PULLED AGAIN AND DOOR CAME OP EN UNEXPECTEDLY. EE FELT A STRAIN IN LOWER BACK RESULTING IN A DIAGNOSIS OF BACK STRAIN AND SPRAIN.
