Showing all 44Originally went to ER. Was released to return to work. After follow-up with ortho DR. was placed on restricted duty on 11/06/24. EE was walking to their work truck when they tripped on some uneven concrete and fell. Injuring their right shoulder.
EE was walking routine for Preventative maintenance. EE came down a flight of stairs. At the bottom, EE turned to the right and started walking. EE stepped on an uneven joint in the concrete and twisted EE's left ankle.
EE was inspecting RM-306(raw mill) roller deck. EE tripped on a support bracket for the lubrication system lines. The tripping motion caused the EE to fall forward into the rocker arm assembly, causing EE's right arm to strain backwards causing shoulder pain. Originally reported as a first aid, but due to continued discomfort medical attention was sought on 5/10/23.
Employee tripped over material discharge hose while operating gunite machine, landing on left wrist.
EE was exiting the quarry office building and fell onto the platform/steps outside the doors. EE cut EE's right shin and was sent for medical evaluation. EE received 5 stitches to close the wound and was returned to duty without restrictions.
EE was pulling a grease line to the Primary Crusher when he missed a step and reported straining his neck while catching his balance. EE was taken to the Dr., diagnosed with a neck muscle spasm and released to work with restrictions.
EE was coming down the stairs from the second level on R-202 and reported losing his footing and feeling a "pop" in his right shoulder as he held on with his right hand to steady himself. EE reported as a First Aid Only case on 06/21/14 and did not seek medical evaluation until 06/27/14. He was diagnosed with a "right shoulder strain" and released to work with restrictions.
Walking on platform and slipped on klinker and fell forward twisting ankle.
EE reported falling on 29Mar13 but did not seek medical attention until 22Apr13. EE was opening a door to a storage shed when wasps flew towards him. As he was stepping backwards to get away from the wasps, he tripped over some pallets and fell to the ground. EE was diagnosed on 22Apr13 with a contusion to the right shoulder and released to work with restrictions.
EE was walking down the stairs of the 992G loader while carrying a bucket and slipped from one step to the next step down. The steps were wet due to rain and his heel slipped from the edge of the traction control on one step to the next step down. (8"drop) He was using the handrail and did not fall. EE was diagnosed with a back strain and had work restrictions for 1 week.
EE was lubricating a tail pulley and reported slipping in mud created by recent rain storms and twisting his left knee. EE reported the incident to his supervisor the following day 9/29/11 as a first aid only and did not request medical attention until 10/05/11. EE was diagnosed with a "strain" to the left knee and released to work with restrictions.
EE was exiting the cab of the skid steer and felt a "pop" and sharp pain in his back as he stepped down to ground level. EE was sent to the company physician for examination and diagnosed with a "strained" back muscle. EE returned to work the same day with minor restrictions.
As he went to return the fuel hose to the stand, he twisted his ankle and fell.

As he was walking down the stairs at the feed end of the kiln onto the ground level there is a 2" drop from one concrete slab onto another and as he stepped off he twisted his left ankle.

"RAIN & MUD CONDITION" EE WAS RETURNING A TROUGHING ROLLER SET TO THE BACK OF THE MAINTENANCE TRUCK. HE THEN BACKED AWAY FROM THE TRUCK AND SLIPPED IN THE MUD. WHEN HE STARTED TO SLIP, HE TRIED TO CATCH HIS BALANCE. HE THEN FELT AND HEARD HIS BACK POP AND INSTANTLY HAD LOWER BACK PAIN.
WALKING DOWN STEPS WITH BOXES IN HAND, SHE MISSED A STEP, FELL INJURING RIGHT HIP. SHE WAS TREATED & RELEASED WITH NO RESTRICTIONS. APPROX 3 MONTHS LATER SHE HAD PAIN IN LEFT HIP. HER PHYCIAN SAID IT WAS FROM PREVIOUS INJURY. AFTER RE-EVALUATION OUR DR PUT HER ON RESTRICTICTIVE DUTY. ON 7/1/03 THIS ACCIDENT BECAME REPORTABLE.
WHEN TRAVELING FROM MAIN PLANT UPPER LEVEL TO BLENDING AREA LOWER LEVEL. EE WALK DOWN AMBANKMENT AS OPPOSED TO USING 1 OF 3 ACCESS STAIRWAYS. EE TRIPPED AT BOTTOM OF EMBANKMENT HITTING HEAD ON RAIL FOR RAILCARS. A CUT WAS SUSTAINED ON TOP OF HEAD.
EE WAS CLIMBING A LADDER AND SLIPPED. WHEN HE GRABBED TO CATCH HIMSELF HE PULL (OR TORE) THE MUSCLE IN HIS RIGHT ARM. THIS IS A LOST TIME ACCIDENT.
WHILE WORKING ON A BELT LINE, EE SLIPPED ON A CLINKER BALL ON THE WALKWAY - HE TRIED TO CATCH HIMSELF BUT PULLED SIDEWAYS.
WALKING DOWN HALL OFFICE, SLIPPED ON SLIPPERY AREA OF FLOOR , BENT RT LEG/FOOT BACKWARDS. FELL DOWN.

EE WAS WORKING ON BC-340, KNOCKING DOWN SKIRT RUBBER WHEN HE SLIPPED ON A CLINKER BALL. HE FELT HIS KNEE BUCKLE. INITIALLY SAW DOCTOR ON 4/26, NO RESTRICTIONS, NO MEDICATIONS. WENT BACK 5/2/02 AND WAS GIVEN PAIN MEDICATION.
EE WAS WALKING AND FELT A SHARP TEARING PAIN IN HER RIGHT FOOT.
EE WAS LOADING 10'X11/2' CONDUIT ON A PIPE RACK,EE OVER LOADED ONE SIDE OF THE RACK, THE RACK HAD NOT SHOE PLATES & THE LEGS SANK IN THE DIRT CAUSING THE RACK TO FALL OVER. THE EE WAS STRUCK BY THE CONDUIT KNOCKING HIM OVER. HE PUT HIS LEFT ARM OUT TO BREAK THE HIS FALL & LANDED ON HIS WRIST. THE INCIDENT OCCURRED AT GROUND LEVEL
WHILE WORKING AT KILN #1, EMPLOYEE TRIPPED ON A PILLAR AND FELL, HE USED HIS HAND TO STOP THE FALL AND INJURED HIS WRIST.
WORKER WAS ON TOP OF A DUST COLLECTOR SETTING A 90 DEG HOOD IN PLACE. WORKER WAS TIED OFF TO VESSEL. WHILE SETTING THE HOOD, IT SHIFTED & PUSHEDTHE WORKER OFF THE SIDE. HIS HARNESS SWUNG HIM I NTO THE SIDE OF THE VESSEL CAUSING BRUISES. THE HARNESS STOPPED HIM FROM FALLING TO THE GROUND HOLDING HIM ABOUT 3 FT OFF THE GROUND
EE COMPLAINED HIS ANKLE WAS STILL BOTHERING HIM ON THE 18TH EE WAS ON LOADING COAL CARS, HE WAS WALKING DOWN THE TRACK TO SPOT THE CAR WHEN HE STEPPED OFF THE RAIL IN A LOW SPOT AND TRIPPED AN D FELL.
WHILE WALKING ON CLINKER BALLS, EE SLIPPED, FELL BACK AND LANDED ON A BERM.
THIS WAS A NEW PCC FINISH MILL STRUCTURE THE FALL PROTECTION LIFELINES HAD NOT BEEN INSTALLED EE AND HIS CO WORKER WERE 21' HIGH INSTALLING FLOOR GRATING 4'X20' HE WENT TO STEP OVER SMALLER FL OOR GRATING 4'X8' DURING A LIGHT DRIZZLING RAIN , IT SLIPPED AND HE FELL 21.
OFFICE CLERK WAS WALKING UP OFFICE STEPS CARRYING MAIL, PARTS AND HER LUNCH BOX. SHE WAS NOT USING HANDRAIL. SHE SLIPPED ON STEPS AND FELL FORWARD, HITTING HER LEFT KNEE, AND LEFT WRIST.
PARTLY CLOUDY DAY. HE SLIP ON SOME LOOSE GRAVEL, LOST HIS FOOTAGE AND HIS KNEE CAME DOWN ON A PIECE OF IRON, CUTTING A SMALL SLIT ON TOP OF HIS KNEE. THE DOCTOR GAVE HIM A TETNUS SHOT AND PUT 5 STITCHES ON THE CUT. HE THEN RELEASED HIM TO COME BACK TO WORK ON MONDAY 7/9/01.
WORKER STATED THEY WERE SETTING A PULL BOX COVER. HE JUMPED OFF THE PULL BOX (APPROX. 4') AND INJURED LEFT ANKLE.
EMPLOYEE WAS CARRYING A PIECE OF EXPANDED METAL WHEN HE TRIPPED. HE REACHED OUT TO CATCH HIS FALL AND FELT A SNAP AND GRIND IN HIS LEFT WRIST.
WORKER STEPPED OUT OF CONEX ONTO UNEVEN ROCKY SURFACE AND TWISTED RIGHT ANKLE.
WHILE HEADING BACK TO #1 MCC ROOM TO PULL A LOCK, EE FELL ON GRATING ALONG SOUTH SIDE OF #1 KILN. HE CUT HIS KNEE VERY DEEP, NEEDING STITCHES.
AT PARK AREA FOR HAUL TRUCKS, EE WAS WALKING TO SHUT OFF THE WATER TRUCK WHEN HE SLIPPED ON A PIECE OF SLAG FROM THE SLAG PILE & PULLED OR TORE A MUSCLE IN HIS LEG.
WHILE REPAIRING #3 SIDE DRAW GATE SMASHED FINGER, REFLEXED BACK MADE HIM LOOSE BALANCE WHICH CAUSED EE TO FALL.
WHILE WALKING DOWN THE STEPS AT BELT CONVEYER 203 DUMP POINT, HE SLIPPED ON SNOWY STEPS AND FELL. HE PUT HIS HAND DOWN TO HELP CATCH HIS FALL.
WHILE EMPLOYEE WAS CLIMBING INTO FORD 4 X 4 TO HELP UNLOAD TOOLS, THE TAILGATE POPPED OPEN JUST AS HE PUT HIS FULL WEIGHT ONTO IT. THIS CAUSED JOE TO FALL ABOUT 3 FEET AND HE LANDED ON HIS LO WER BACK.
EE WAS LOOSENING A NUT ON MOTOR FOR THE ROATRY COMPRESSOR IN THE MILL BUILDING WHEN THE WRENCH SLIPPED OFF CAUSING EE TO FALL BACKWARDS STRIKING HIS MIDBACK ON A PIPE.
