Seen at clinic, put off work. Orthopedist confirmed on 05/02 incident aggravated pre-existing arthritis. WC investigation complete 05/03, confirmed occupational rt.knee strain.
Ee was exiting bus and foot caught, causing ee to fall to the ground, landing with/on left arm. Ee was seen at the ER and diagnosed with a fractured humerus. Put off work and referred to a specialist.
EE and another ee were using a "big gun" to hydroblast scale from the walls at the # neutralization tank in the Barrick Roaster. The big gun was unsecured when pressure was introduced to the system. EE was pushed down onto the grating ee was standing on by the force of the big gun moving away from the railing.
Ee was adjusting the mirror on grader when ee pushed hard and the mirror moved causing ee to lose balance and fall. Ee cut left shin and bumped head. Ee was released to full for next scheduled shift but required stitches on the shin.
Ee on nights was walking across deck of track hoe. Hatch on the deck was left open from previous maintenance and was shadowed. Ee stepped through the opening, fell, injured left elbow, left leg. Leg continues to have pain in the thigh area; ee was referred to physical therapy on 08/17/18 making this reportable for treatment. Ee continues on full duty.
Ee was cleaning window of service truck. Ee was walking with the wand and tripped on a rock, and the tip of the wand struck Ee's boot, and Ee gripped the trigger trying to catch self, sending hot water/steam into boot. Ee transported to the burn unit in SLC for treatment of burns to the right foot.
Ee was exiting bus at work area, missed the first step, and fell down the steps onto the ground. Hit right elbow, jarred right shoulder, hit head and hit lower back. Ee was put off work.

An electrician was stepping into a tractor mounted basket platform and slipped in the mud, twisting knee. The ee was seen at the clinic, diagnosed with a sprain to the left knee, and put off work.
Employee slipped on a patch of ice and fell to the ground, landing on back side. Employee was seen at the ER, diagnosed with a coccyx contusion and lumbar strain. Xrays were negative for fracture. Employee was given meds and put off work.
At approximately 1300 hrs operator was washing down area with a water hose. As operator was pulling slack on the hose they stepped backwards and their left foot caught on a ladder landing and they lost their balance. They used their right arm to catch fall and injured right shoulder.
Ee bent down to pick up a piece of metal and EE's left foot rolled in. EE twisted in order to keep self from falling, and strained EE's back. EE was seen at the ER and released to full duty. Subsequent follow up on 03/02 resulted in restricted duty effective 03/03/16.

Security Officer went to Entrance Lane 3 to retrieve paperwork from truck driver, as she stepped up on the small berm and reached for the paperwork she fell to knee. She stated she was fine, later she was sore and went to the Emergency Room. She only had a bruised left knee and hand.

Officer was exiting the porta-pottie, behind the Main Gate Entrance at Barrick Goldstrike. She tripped, lost her balance and fell to the ground. Officer landed on both knees and hands, jamming her right arm. She finished her shift and once in town, taken to hospital for further exam. She fractured her right elbow.
Ee was hosing down in the autoclave, she slipped on the wet floor and fell, landing on right knee, left knee hyperextended. Ee felt pain in left knee. Referred to orthopedist and was put restricted duty and an MRI is ordered.

Ee was climbing an access ladder when he bumped his head on an overhead pipe, jarring him and making him lose his grip on the ladder. He fell backwards onto the concrete. He did not lose consciousness. He was evaluated by a physician and diagnosed with a fractured rib.
Employee was performing pre-op and slipped on step of an AD30 and hit his elbow on step. Employee did not want medical treatment; said it was just sore. On Dec.17,2013 employee requested to go to doctor because elbow had swollen up during his days off. Diagnosis: Olecranon Bursitis and Right Lateral Epicondylitis. Employee on restricted duty.
