Employee was stepping off the bus and twisted an ankle. Employee reported to supervisor, was seen at the clinic and put in a walking boot and off work for 2 shifts.
Ee walking down stairs carrying tools. Stepped down, twisted right knee, felt pop. EE reported this the following morning, as knee swelled up and began hurting overnight. Seen at clinic, put off work. Orthopedist confirmed on 05/02 incident aggravated pre-existing arthritis. WC investigation complete 05/03, confirmed occupational rt.knee strain.
Ee was exiting bus and foot caught, causing ee to fall to the ground, landing with/on left arm. Ee was seen at the ER and diagnosed with a fractured humerus. Put off work and referred to a specialist.
EE and another ee were using a "big gun" to hydroblast scale from the walls at the # neutralization tank in the Barrick Roaster. The big gun was unsecured when pressure was introduced to the system. EE was pushed down onto the grating ee was standing on by the force of the big gun moving away from the railing.
Ee was adjusting the mirror on grader when ee pushed hard and the mirror moved causing ee to lose balance and fall. Ee cut left shin and bumped head. Ee was released to full for next scheduled shift but required stitches on the shin.
Ee on nights was walking across deck of track hoe. Hatch on the deck was left open from previous maintenance and was shadowed. Ee stepped through the opening, fell, injured left elbow, left leg. Leg continues to have pain in the thigh area; ee was referred to physical therapy on 08/17/18 making this reportable for treatment. Ee continues on full duty.
Ee was cleaning window of service truck. Ee was walking with the wand and tripped on a rock, and the tip of the wand struck Ee's boot, and Ee gripped the trigger trying to catch self, sending hot water/steam into boot. Ee transported to the burn unit in SLC for treatment of burns to the right foot.
Ee was exiting bus at work area, missed the first step, and fell down the steps onto the ground. Hit right elbow, jarred right shoulder, hit head and hit lower back. Ee was put off work.
Ee was walking down stairs, stumbled, and caught self on the handrail, hyperextending left ring finger. Seen at ER, diagnosed with a fracture that required surgery on 12/04/17. Ee released to restricted duty.
Ee was mounting tractor, reached for handrail and missed, causing ee to miss the step and fall. EE has a fractured left wrist and right thumb. Seen at ER, put off work and referred to ortho.
Employee stepped out on the fender to retrieve the wheel choke on a 938M Cat loader. Slipped off the rubber mud flap, landed on right hip, fracturing right femur.
An electrician was stepping into a tractor mounted basket platform and slipped in the mud, twisting knee. The ee was seen at the clinic, diagnosed with a sprain to the left knee, and put off work.
Employee slipped on a patch of ice and fell to the ground, landing on back side. Employee was seen at the ER, diagnosed with a coccyx contusion and lumbar strain. Xrays were negative for fracture. Employee was given meds and put off work.
At approximately 1300 hrs operator was washing down area with a water hose. As operator was pulling slack on the hose they stepped backwards and their left foot caught on a ladder landing and they lost their balance. They used their right arm to catch fall and injured right shoulder.
Ee bent down to pick up a piece of metal and EE's left foot rolled in. EE twisted in order to keep self from falling, and strained EE's back. EE was seen at the ER and released to full duty. Subsequent follow up on 03/02 resulted in restricted duty effective 03/03/16.
Ee was stepping off haul truck ladder, slipped on last step, landing on ground with both feet, jarring EE's right low back. EE was seen at the ER, diagnosed with low back pain, no fracture. On follow-up EE was put off work pending physical therapy and reevaluation.
Ee was mounting a piece of equipment and the step broke. Ee fell jarring right leg and hip and twisting lower back. He was seen at ER and released to full duty. On follow up he was released to restricted duty and ultimately lost time on 12/09 due to surgery. Injury was determined work related on 12/28/15.
EE was walking in to MCC building when he slipped and fell, injuring his shoulder. Seen at ER, given a sling and referred to orthopedist. Returned to restricted duty.
Ee stepped over bench in the dry, tripped and fell, catching self with left arm. Strained hand, wrist, elbow, shoulder. Ee seen at clinic, diagnosed with a chip fracture and possible soft tissue tear, referred to orthopedist and placed on restricted duty.
Ee tripped going through lab door and caught herself when she fell. Seen at the ER and diagnosed with a fracture to the wrist. She was released to restricted duty.
Ee was walking into phase 2 tunnel, slipped on ice, and fell. He hurt his left shoulder and knee. He sought treatment on 01/26 and was released to restricted duty. Diagnostics being done for a possible rotator cuff tear.
Material fell from under truck the ee was cleaning, striking the ee on the head and causing him to fall to the ground. He struck his head on the concrete. He was seen by the doctor, diagnosed with a minor concussion and put off work. Returned to work full duty on 01/23/15.
Security Officer went to Entrance Lane 3 to retrieve paperwork from truck driver, as she stepped up on the small berm and reached for the paperwork she fell to knee. She stated she was fine, later she was sore and went to the Emergency Room. She only had a bruised left knee and hand.
Employee stated that while cleaning the windows of a dozer, slipped on track and strained back, did not indicate injury. Employee laid off that day, company recvd. diagnosis of disc herniation on 11/17/14.
Employee dismounted his drill and stepped onto a rock that rolled under his foot. He started falling, caught himself partially, but his left knee hit the ground. Seen at Golden Health and put off work pending an evaluation by an ortho.
While grinding off paint on a suspended load, employee step back on an extension cord cap, lost his balance and fell on his right knee.
Ee was getting off the bus and stepped onto uneven ground, twisting his left ankle. He was evaluated by a physician and placed in a walking boot and released to full duty. Use of the walking boot has restricted his activity and he is working modified duty.
Officer was exiting the porta-pottie, behind the Main Gate Entrance at Barrick Goldstrike. She tripped, lost her balance and fell to the ground. Officer landed on both knees and hands, jamming her right arm. She finished her shift and once in town, taken to hospital for further exam. She fractured her right elbow.
Ee was hosing down in the autoclave, she slipped on the wet floor and fell, landing on right knee, left knee hyperextended. Ee felt pain in left knee. Referred to orthopedist and was put restricted duty and an MRI is ordered.
While walking from the bus to the autoclave, ee slipped on mud, hyperextending his knee. His knee swelled and became painful. He was seen by a doctor and referred to an orthopedist who has put him off work pending an MRI.
EE had exited the air mover and after he was completely on the ground he turned to his left to move toward the rear of the vehicle. As he turned he felt a pain in his lower back. EE was wearing the required footwear and was not carrying anything when the injury occurred. EE sustained a muscle strain to his lower back and has been released to work with restrictions
Ee was climbing an access ladder when he bumped his head on an overhead pipe, jarring him and making him lose his grip on the ladder. He fell backwards onto the concrete. He did not lose consciousness. He was evaluated by a physician and diagnosed with a fractured rib.
Employee was performing pre-op and slipped on step of an AD30 and hit his elbow on step. Employee did not want medical treatment; said it was just sore. On Dec.17,2013 employee requested to go to doctor because elbow had swollen up during his days off. Diagnosis: Olecranon Bursitis and Right Lateral Epicondylitis. Employee on restricted duty.
Ee was walking through paste when he felt left knee pop, causing the knee to swell. He reported the issue, tried self-care, and sought medical attention on 11/06 at the clinic. Diagnosed with a knee strain and placed on restricted duty.
EE was pulling fuel hose to fill tank, she slipped on a step, causing her to fall, catching herself with her left hand and she felt immediate pain in her left wrist. Seen at the ER, x-rays negative for fracture, she was released to restricted duty.
Ee walking up stairs to lock out equipment, boot caught and he fell into next step, striking right knee. Seen at orthopedist and put on restricted duty.
Ee getting into back seats of van. Started to lose balance, and braced his right leg to avoid falling. He felt pain. He reported to his supervisor on 08/07 and when the pain did not get better he asked to see a doctor. He was seen on 09/06 and referred to an orthopedist. On 09/12 he was diagnosed with a fracture to the tibial plateau and placed on restricted duty.
Employee was using a fire hose to clean in the tunnels. He tripped over the hose that was in the walkway and fell, catching himself on his braced left arm, jarring his left shoulder. Released to restricted duty and told to attend physical therapy.
Employee slipped on ice and fell to ground. Sought treatment for her low back pain on 3/7/2013. Diagnosed with a low back strain and put off work for physical therapy.
Employee was walking across the Barrick parking lot, slipped on ice and injured his right knee.He did not request medical treatment at that time, thought it was just bruised. Seeked medical attention 4 days later, had MRI, results tear of meniscus, wear knee support, restricted duty and referral to Orthopedic Doctor.
Employee was walking toward office when he slipped on the ice and landed on his back. He reported to his supervisor and did not immediately seek treatment. On 01/07/2013 he requested to be seen by a doctor, was seen at the clinic and released to restricted duty.
While securing tires on bed of transport truck with straps associate slipped on ice and injured left arm and hit head. Diagnosis on Nevada form C-4 is concussion due to fall, caused partially by acute renal failure...
EE was walking to the collar, slipped on snow covered ice and fell to the ground, pulling his abdomen and straining his back. Seen at the ER and diagnosed with an umbilical hernia. The hernia will be repaired and we will be filing an amended 7000-1 with updated days.
While disconnecting a hose from the carbon truck, the hose came loose from fitting and threw the employee off balance. He fell to the ground, fracturing the right wrist. Will have surgery on 04/16/12 for repair. Currently working restricted.
Employee was lining up connector on push arm with bar when the connection slipped, he lost his balance and fell back against a tool box. He was seen at the ER and released to restricted duty. Continued on restricted duty on follow-up at clinic.
Ee got off the bus at the welding shop and was walking to the truck shop to begin his shift when he slipped on the ice and fell, injuring his left knee. He was seen at the ER, referred to an orthopedist who placed him on light duty.
At the start of the shift a haul truck operator was walking from the crew van to the equipment ready line. While walking, the operator's leg slipped forward resulting in a strain of the right knee. There were winter like conditions at the incident scene (snow/ice/frozen ground). The ready line area had been bladed the previous shift.
Employee was walking in the drift and tripped over a rock, falling to the ground. She slid on both wrists and left hand. X-rays were negative for fracture, given a splint for left wrist and released to restricted duty.
IW stepped off bolter, planted left foot and turned. Left knee popped and knee gave out, causing IW to fall to ground. Seen at ER, released to restricted duty. Previous non work-related knee problems investigated, MRI revealed new ACL full tear, claim accepted by workers' comp on 07/12/11. Surgery scheduled.
