Employee passed out inside mill. Had lack of sleep due to issues at home. Also possible potassium deficiency and several layers of clothes.

Autoclave operator went to field to change over valves on Geho strainers. A technician was using a wrench on a stuck valve when the valve broke free and threw the wrench, which ricocheted and struck the autoclave operator standing nearby in the face. Fractured nose, facial lacerations, and put off work.

MD624 had an electrical grid box fire which took more than 30 minutes to extinguish. Truck has been removed and isolated for investigation purposes.

Employee was stepping off the bus and twisted an ankle. Employee reported to supervisor, was seen at the clinic and put in a walking boot and off work for 2 shifts.

Ee was using a one inch impact to loosen a nut. As the nut spun loose, the gun hung up and twisted the impact, pinching the ee's right middle finger between the gun and casing. The ee was diagnosed with a fracture of the fingertip and released to full duty.

Ee was sitting in haul truck being loaded. The shovel hit the truck, jostling the ee in the cab. The ee did not report incident until two days later. Investigation by workers' comp determined compensable, claim accepted on 07/02/19. Low back strain/inguinal contusion. Reportable for lost time.

Employee was at the face cleaning the lifter hole with a fin hoe preparing to install collar pipe. As EE was using the hoe a section from near the center of the face sloughed down towards EE striking EE to the ground on the left rib.

Ee was using a wrench to loosen a bolt, when the bolt came loose the wrench came back and struck the ee in the mouth, fracturing two front teeth. Released to full duty, tooth numbers 8 and 9 will need repair and caps.

EE was cleaning windshield of loader when ee twisted and leaned, reaching to clean the middle of the windshield. EE felt a sharp pain in shoulder which progressively got worse. EE was seen at the ER, diagnosed with a strain to the shoulder. Follow-up resulted in the ee being put off work.

Ee was removing a pipe plug from a strainer housing using a pipe wrench. When torqueing on the wrench, EE felt a sharp pain in the low back which worsened throughout the day. Seen at the ER, diagnose with a muscle spasm, and put off work.

Ee walking down stairs carrying tools. Stepped down, twisted right knee, felt pop. EE reported this the following morning, as knee swelled up and began hurting overnight. Seen at clinic, put off work. Orthopedist confirmed on 05/02 incident aggravated pre-existing arthritis. WC investigation complete 05/03, confirmed occupational rt.knee strain.

Ee picked up a charged fire hose and the nozzle came off causing high pressure water to hit ee's eyes. Seen at ER and diagnosed with contusion to eye, released to full duty. On follow-up on 04/15, ee was given a prescription for eye drops.

Ee was cleaning Geho strainers. After isolation, the strainer housing did not fully depressurize. Ee used a wrench to poke through a layer of mud at the top of the housing, causing the housing to depressurize rapidly, releasing hot slurry that struck the ee on the left side of ee's face, and ee's left wrist. Seen at clinic, diagnosed with 2nd degree burns and released to full duty.

EE was lifting a heavy object when they felt a pop in the right calf. They reported the incident to a supervisor and worked out the shift. EE was seen the next day at the clinic, given crutches, and put off work.

Employee was hooking up a trailer. The employee pulled the tongue of the trailer over to align the pintel when the trailer dropped onto the pintel the tongue smashed the employees finger between the tongue and the block the trailer had been setting on.

Employee installing chute on concrete mixer truck. Finger caught in pinch point between two chutes. Open fracture (non displaced) to right index finger.

Unplanned roof failure at the 3535 Level. No one was in the vicinity at the time of the failure. Found failure and reported to MSHA. The failure was due to a fault line in the vicinity.

EE was on scissor deck when the wings were folding up. One of the wings crushed their right foot. EE transported to ER, then flown to University of Utah for surgery. Surgical repair to foot on 02/03, EE released to home on 02/04. Lost time. *Investigation confirmed EE dismounted scissor deck while it was in motion. 2/15 found infection. 2/21 amputated to ball of foot.

Ee was replacing hoist cylinders on a CAT 777. The cylinder was hooked to a crane. Slack in the crane attachment allowed the cylinder to move, crushing the ee's right hand between the hoist cylinder and the fuel tank. The ee was diagnosed with a fracture to the right ring finger.

Ee was grinding on hard nose of MG753 when the grinding disc came apart. A piece of the disc struck the ee on the upper left chest, causing a small wound. The ee was seen at the ER where stitches were required to close the wound. Ee released to full duty for next scheduled shift.

Ee was exiting bus and foot caught, causing ee to fall to the ground, landing with/on left arm. Ee was seen at the ER and diagnosed with a fractured humerus. Put off work and referred to a specialist.

Task was to change leaf springs on a lube truck. While adjusting jack stand height, adjustment tube slipped and caught the right thumb of the tech. Minor laceration/nerve damage.

Module in hoisting system failed causing the hoist to stop between levels. Miner was trapped for more than 30 minutes. No injuries.

Ee was holding a belt with both hands and watching a co-worker move weight from the belt. When the weight was moved, the belt quickly pulled the ee's hand up and it was pinched between the metal and the belt. Ee was seen at the ER, x-rays were negative for fracture, the wound was stitched and the ee released to modified duty.

Ee experienced a non-occupational health issue that required transport from the mine. MSHA considered this a reportable event.

Power bumped and caused the Rodeo shaft to be down for more than thirty minutes. No one was on the cage at the time of power bump. Power bump was caused by outside source and beyond the control of Goldstrike UG. A courtesy call to the MSHA hotline was made with a follow up call to Gary Hebal the Elko field office supervisor.

Ee was walking through thick slurry material in a containment area. When ee went to pull foot out ee felt a pop in low back. EE was diagnosed with a low back strain. Put off work pending a follow-up on 09/17/18. EE missed one scheduled shift.

Ee was standing talking with an electrical contractor supervisor about schedules. When the conversation was over, the contractor supervisor turned to leave and saw the ee fall over. Bystanders began care, EMTs continued care, and ee was flown to NNRH ER. Ee received care at NNRH and was transferred to UofU Medical Center. Ee died 08/08/18. Please see attached email.

EE was shoveling out a conveyor drop chute when they bumped their arm on a railing, dislodging some fine dust/lime, which blew into their eye. Seen at the ER and diagnosed with a corneal abrasion. Follow-up with eye doctor diagnosed inflammation and irritation, prescription eye drops given. Released to full duty.

Ee sustained a reportable standard threshold shift in hearing. Confirmation received from AuD, CPS/A, on 07/18/2018. Hearing conservation retraining completed.

Ee was pulling a pump spool and got some hot slurry on the left side of face, neck and left arm, and strained left leg. Further exam on leg on 7/19/18 revealed a torn hamstring and the ee was put off work. Referred to orthopedist.

Ee was cutting a piece of angle iron using a handheld grinder with a cutoff wheel. Completed the cut, shut off the grinder and reached with left hand, and wrist contacted the wheel which was still spinning. Seen at the clinic and sutures were required to close the cut. Released to full duty.

EE and another ee were using a "big gun" to hydroblast scale from the walls at the # neutralization tank in the Barrick Roaster. The big gun was unsecured when pressure was introduced to the system. EE was pushed down onto the grating ee was standing on by the force of the big gun moving away from the railing.

EE and another employee were using a "big gun" to hydroblast scale from the walls at the #2 neutralization tank. The big gun was unsecured when pressure was introduced to the system. EE turned to run away from the big gun and ran into a deck mounted motor. This caused deep bruising to EE's right leg.

A reportable standard threshold shift in the ee's hearing was identified during annual hearing test and confirmed by Barrick's Professional Supervisor on 6/5/18.

Ee was adjusting the mirror on grader when ee pushed hard and the mirror moved causing ee to lose balance and fall. Ee cut left shin and bumped head. Ee was released to full for next scheduled shift but required stitches on the shin.

The Mechanic was cutting out a seized through shaft from a Kenworth differential. The Tech was above it when ee was cutting, then got under it to see where else needed to be cut. As the Mechanic was getting out from under, the cut broke free and the driveline swung into the Mechanics Left Cheek

Ee on nights was walking across deck of track hoe. Hatch on the deck was left open from previous maintenance and was shadowed. Ee stepped through the opening, fell, injured left elbow, left leg. Leg continues to have pain in the thigh area; ee was referred to physical therapy on 08/17/18 making this reportable for treatment. Ee continues on full duty.

Ee was cleaning window of service truck. Ee was walking with the wand and tripped on a rock, and the tip of the wand struck Ee's boot, and Ee gripped the trigger trying to catch self, sending hot water/steam into boot. Ee transported to the burn unit in SLC for treatment of burns to the right foot.

Mechanic working on the rebuild CBD pump split EE's lip. The employee was repacking the pump with a 2 foot bar on the packing pusher to push the packing ring further into the gland. The bar slipped off the gland and the other end of the bar contacted the employees lip. Seen at clinic and stitches were required, released to full duty.

EE was unhooking hoses and when they stooped down by the rib to unhook air hose, employee caught an ear on a small piece of wire sticking out of the rib. Seen at the ER and given stitches to close the wound. Released to full duty, must keep ear clean, and work in clean area for 3 days.

EE sustained a reportable standard threshold shift in hearing. Confirmation received from Dr. Brent Black, MD on 03/13/2018. Hearing conservation retraining complete.

Ee finished loading a round and when the went to remove the remote from mine belt, finger contacted open box cutter causing a cut. Stitches required to close the wound. Ee released to full duty on next scheduled shift.

Ee was exiting bus at work area, missed the first step, and fell down the steps onto the ground. Hit right elbow, jarred right shoulder, hit head and hit lower back. Ee was put off work.

Ee was walking down stairs, stumbled, and caught self on the handrail, hyperextending left ring finger. Seen at ER, diagnosed with a fracture that required surgery on 12/04/17. Ee released to restricted duty.

The freight elevator at the roaster was commanded to travel from the third floor to the twelve floor. The elevator traveled approximately 15 feet in the opposite direction toward the ground floor and stopped in between levels. There was an employee on the elevator during this incident. The employee remained on the elevator for 2.25hrs before being removed.

Ee reports was driving haul truck down ramp and swerved to avoid a rock, striking the rib, causing ee to hit left rib/chest wall on the armrest of the truck. Seen at clinic, diagnosed with a chest wall contusion and put off work.

Ee repairing broken feed change. Operator rotated the feed motor, chain moved and pulled the ee's hand, hydraulic driven clamp closed, crushing hand in bolter. Ee seen at ER, diagnosed with fracture to right finger, released to restricted duty. Site could not accommodate so put off work.

Ee was mounting tractor, reached for handrail and missed, causing ee to miss the step and fall. EE has a fractured left wrist and right thumb. Seen at ER, put off work and referred to ortho.

Process operator cleaned off level transmitter for RIL feed hopper. EE noticed the level in the vessel rising and exited the platform using the access ladder. EE made it to the ground and the vessel overflowed, causing hot slurry to hit them on the right side of the body, partially filling their right and left boots with hot slurry. The hot slurry burned their bilateral lower legs.
