EE was leaving his work location in the Shipping Tower, decending the steps from the 3rd level. Prior to reaching the 2nd level landing, he looked back up at the 3rd level and noticed he had left the Load Out Chut in the down position. He was not paying attention to the steps and and missed the bottom step, twisting his right ankle, examined by MD, put on restricitons.

The weather before the start of the employee shift was humid and there had been previous heavy rains. The employee was decending on a flight of stairs. The employee slipped causing the employee to fall down the steps. The employee had bruising of the rt. hip and rt. forearm.

We had a coal bin fire and were in the process of dumping the coal into loader buckets for disposal. The injured party was opening a dump valve when a small piece of floor grating shifted and allowed the employee to fall through to the ground below. The employee sustained a laceration to his head approximately 2 inches long requiring 10 stitches.

THE EE WAS CLIMBING DOWN OFF OF THE TRACKHOE. WHEN THE EE PUT HIS FOOT DOWN HE THOUGHT HE WAS STEPPING DOWN ON SOLID GROUND. BUT THE GROUND WAS EXTREMELY SOFT. WHEN THE EE STEPPED DOWN INT HE MUD, HIS FOOT SANKED IN THE MUD AND HIS FOOT TURNED TO THE SIDE. THIS CAUSED HIM TO BE OFF BALANCE AND TO PUT TOTAL WEIGHT ON ANKLE.

THE EE WAS FEEDING THE HOPPER AND NOTICED THAT THE FEED WAS NOT GOING UP THE BELT. THE EE GOT OFF THE LOADER AND RAN TO THE HOPPER TO CUT OFF ALL THE CONTROLS. IN THE PROCESS OF THE EE RUNING TO HOPPER. HE TWISTED HIS RIGHT ANKLE.
