Was unloading saw table, backed over saw rail, and tripped. Fell on 4" x 4" rock and cut back. Resulted in 4" to 5" gash approximately 1" deep on lower back. Required 16 stitches inside and out. Back at work next day.

EMPLOYEE HAD STEAM-CLEANED THE DOZER BUT DIDN'T LAY THE STEAM WAND DOWN. HE CLIMBED DOWN OFF THE DOZER TRACKS WITH THE WAND AND SLIPPED AND FELL BACK AGAINST THE BLADE CYCLINDER GUARD.
EE WAS PUTTING GREASE ON THE 7' SAW BAR, STANDING ON SAW TABLE, SAID HE TURNED TO STEP DOWN OFF TABLE & HEARD A POPPING NOISE IN HIS KNEE.
EE HAD BEEN MOVING ROCK FROM LOCATION TO ANOTHERLOCATION FINSHED-STARTED TO GET OFF FORKLIFT, GOT TANGLES UP IN THE SEAT BELT BUCKLE STRAP & FELL DOWN SIDE OF FORKLIFT AND ONTO FLOOR
