Showing all 23Employee arrived for the shift, while walking across the wet limestone parking lot EE slipped and fell onto knees, causing contusion on L knee. At time of this injury on 9/3/19, EE thought was okay. On 10/24/19 EE reported knee still bothering EE. Was sent to Dr for 1st visit. On 11/4/19 EE had follow-up & Dr put EE on restrictions.
Employee was going up steps of skid steer to pull the keys from the ingnition for the day. After retreiving the key, EE pivoted on EE's foot causing the boot tread to get caught in the step of the machine. Employee was thrown off balance and fell approx 2' to ground on right shoulder.
Employee was working in the primary feeder hopper. He was standing on built up material in the hopper, clamping a piece of belting up so access to the rotor was open. He turned to climb down and his foot slipped, causing him to fall sideways about 2 ft into the bottom of the feeder trough, impacting his right shoulder.
Employee was walking on the grizzly bars inside the feeder hopper to enter the impactor housing to set up wire feed welder on hammers. Employee's foot slipped off top of bars and slipped in between causing top of foot to over extend.
EMPLOYEE WAS ON DOZER PLATFORM AND DROPPED HIS PENCIL, BENT OVER TO PICK IT UP AND BUMPED HIS HEAD ON THE MACHINE WHICH CAUSED HIM TO LOSE HIS BALANCE, LOST HIS GRIP AND SLIPPED DOWN ONTO HIS PICKUP TRUCK HOOD WHICH WAS PARKED NEXT TO HIS MACHINE, THEN SLIPPED DOWN TO THE GROUND.
Employee was getting off his excavator when he noticed there was mud on the step. To avoid slipping from the step he jumped from the track approx 3' landing on his left heel, which in turn caused pain in his back and leg.
employee was checking on unlocked door at accounting office when he fell off the step onto concrete floor.
EMPLOYEE WAS STEPPING OFF A NEWLY INSTALLED WALKWAY AT # 2 CRUSHING PLANT WHEN HE STEPPED WRONG, OVER-EXTENDING HIS RIGHT KNEE BACKWARDS. EMPLOYEE'S KNEE HURT A LITTLE AT THE TIME BUT WAS STIFF AND SORE THE NEXT MORNING.
Employee was coming down the ladder of a 65 ton Euclid haul truck when his right ankle twisted.
Employee was climbing off a D8R Cat Dozer and lost his footing. Employee's right ankle hit the dozers blade arm and a step welded to the blade arm , causing bruising and swelling.
employee was turning a piece of 1" plate over to gring off the cutting torch slag when he stepped back on a power cord causing his foot to slip. he lost his lifting momentum causing the weight of the plate to come back at him at which that point he lowered the plate.
EE WAS CHANGING SCREEN CLOTH AT THE CRUSHER, STEPPED ON THE DISCHARGE LIP CAUSING EE TO SLIP AND LOSE FOOTING. EE SLIPPED DOWN THE DISCHARGE SHOOT APPROX 6' LANDING ON THE CONVEYOR BELT.
EE WAS WALKING TO HAUL TRUCK TO START SHIFT. WALKED INTO A SMALL GULLEY MADE BY RECENT RAINS AND TWISTED KNEE
EMPLOYEE WAS GREASING THE PLANT. HE LEANED BACK ON THE GUARD RAIL & IT BROKE. HE LOST HIS BALANCE & GRABBED AHOLD OF A PIECE OF ANGLE IRON, CUTTING HIS HAND ON A JAGGED EDGE.
EMPLOYEE WAS COMING DOWN THE LADDER OF HIS FRONT END LOADER WHEN HE MISSED THE STEP CAUSING HIM TO HOLD MOST OF HIS WEIGHT WITH HIS ARMS HOLDING ONTO THE HAND RAILS.
EE WAS CLIMBING THE LADDER TO GET IN HIS TRUCK WHEN HIS HANDS SLIPPED FROM THE HAND RAILS, CAUSING HIM TO FALL TO THE GROUND.
EMPLOYEE COMPLETED FUELING HIS HAUL TRUCK. HE CLIMBED UP THE TRUCK TIRE INSTEAD OF THE LADDER. THE TIRE WAS MUDDY CAUSING HIM TO SLIP HITTING HIS KNEE ON THE TIRE.
EE WAS UNCLOGGING CONE CRUSHER. HE STEPPED BACK ON THE WALK WHICH WAS PILED WITH DEBRIS OUT OF THE CRUSHER WHEN HE MISSTEPPED, FALLING OFF WALK DOWN ONTO SAFETY GUARD OVER THE TOP OF THE PIT L ANDING ON HIS FEET. HE THEN SLID DOWN THE GUARD FALLING TO THE GROUND BELOW APPROX. 4'
EE WAS STEPPING OVER WELDING LEADS & TRIPPED. HE WENT HEAD FIRST INTO THE CATWALK HANDRAIL.
EE WAS CLIMBING THE LADDER TO HIS HAUL TRUCK WHEN HIS FOOT SLIPPED CAUSING HIM TO SLIP DOWN LANDING ON THE NEXT LADDER RUNG.
EMPLOYEE WAS EXITING WABCO HAUL TRUCK CAB WHEN HE TRIPPED ON LEDGE. INSTEAD OF FALLING HEAD FIRST, HE JUMPED FROM THE PLATFORM, LANDING ON HIS LEFT HEEL.
EMPLOYEE SLIPPED IN MUD ON GROUND LEVEL AT THE BASE OF STAIRS FALLING ONTO METAL STEPS.
